That Doctor that was "saving" Jinx life is the same Doctor that made the shimmer for Silco. He looks different now because he got burned during the "incident" in episode 3. The "mutation" of the "salamander" (Rio) is that it turns those purple flowers into shimmer. That's where shimmer comes from. The doctor (now named "singed" in the captions) knocks out Silco before he works on Jinx so that he can work in peace without a concerned father hovering over him. Perhaps he also does some sort of experiment on Jinx too. Singed, in the game, is known for being an unethical scientist who experiments on people.
I agree with you on the horn that sound is just chilling. the first time it went off in episode one when the kids exploded the apartment it seemed so out of place and gave me such an ominous feeling, I feel like that horn was one of the first subtle hints that the series was going to have a major tone shift
