I found a vlog from a German pilot. I am not sure which airline she works for, but I think it's Lufthansa. She showed the approach to Madeira and mentioned that pilots have to have a certain amount of time with the airline before they are allowed to fly to Madeira.
Yes they need a special license for Madeira airport
Special certification needed, and only captain landing and takeoff
