    Thank you- that is exactly how I lost the weight- make protein, limit carbs- and brought down the fat macro as I lost the weight. Lost 80 lbs the first year, another 10 the second year, another 5 this year- and I am near my goal weight. Great lecture!!

    Thank you Dr Gerber for doing something which is seldom done in the low carb community. You have come up with an amicable comprise that acknowledges the validity of both the calorie hypothesis and the hormonal hypothesis of weight management. From a personal perspective, I can vouch that the most effective means to achieve fat loss, and most importantly, maintain that weight loss for the long run is to be mindful of both the macronutrient composition of food and the calorie content of said food. Far from being diametrically opposing ideas, the former facilitates adherence to the latter. A diet comprised of a low carb approach tends to satiate the body to such an extent that it makes achieving caloric restriction achievable for the long term. However, if one simply thinks that low carb, even with its established metabolic advantage, gives one carte blanche to indulge in food to hedonistic excess to the point of significantly exceeding ones caloric needs, then you are destined to never reach your weight loss goals or maintain them in the long run.

      My speculation , indeed, is that weight regain, is due to poor food quality despite being on keto, wrong omega ratio, seed oil usage etc... So, once again, keto alone means nothing, margarine and chicken breast not equal to butter and t-bone steak.

      @@JeffryGerberMD I still am seeking a solid answer. My health has dramatically improved and I've lost weight I'm 6 ft 4 in a way about 220 (was at 270) trying to get down to 200 pounds that 20 lb or so is stubborn. I do only eat one meal a day so I am fasting 18 to 20 hours a day. I wonder if my iced coffee with for cream and 3 Splenda is stalling the belly fat. But I doubt it. Any thoughts if you get this?

    To be fair, Dr. Gerber's talk focused on why weight loss plateaus. In his conclusion, he does seem to point to reduction of fat intake (thus reducing total caloric intake) will increase BMR and jump start weight loss again. Not sure why he wasn't more purposeful with this point?

    Thank you Dr. Gerber! I have seen this in my own journey. Although my carbs were extremely low, my weight loss was stalled. Once I started tracking calories, and lowered them, I started losing weight once again. Since I stayed LCHF, I didn't experience "starvation" as I have in the past on a normal calorie diet.

      Every one has a diet that works for them.i prefer carnavoir. But I'm a strong believer in blood type diets.the 0 people do better on low carb diets and keto and carnivour diets. Blood type a and b may do better on higher carb

    Well done! Perhaps the reason for the "checkmark" phenomenon with weight regain over time is a sign we need to follow the examples of our ancestors and insert fasting days to maintain our healthy weight indefinitely and mimic the periods of food scarcity in the midst of our abundance.

      If you eat the right foods there is no need to fast, the caloric density and volume is matched to our ideal weight.

      @@TB1M1 If you don't want to clean your cells with the autophagy process, if you don't want to boost the production of your growth hormone and your stem cells to repair organs, bones, muscle or skin, no, there is no need to fast. For me weight loss during fasting is more a side effect of my rejuvenation cure than an aim!
      And for people with a T2D to reverse or a hundred pounds to loose just "eating the right foods" won't be very efficient...and not very fast either.

      He's saying you naturally eat a higher protein level and lower fats(satiety) thus consuming fewer calories. He's saying decrease calories as you get smaller.

      Through a mathematical point of view he is saying: maintain constant carb and protein intake (that you started with, mind you we are on a lchf diet so 50g net carbs, 100g protein, 160 fat from diet) and than just reduce daily (let's not exaggerate - monthly) your fat intake. Basically: follow the grams not the macro percentages. The logic is: if you stick with the macro percentages the only thing you achieve by reducing calories is: decrease carbs amount (ok... that's not bad), decrease protein amount (oh no... that's extremely dangerous... don't ever do that) and....well.... same amounts of fat (as this is how math works, due to the fact that fat holds such a large share, it is almost inelastic to your reduced calories). What follows is: you don't allow your body to tap into the stored fat...thus...you plateu. Voilà! Now you understand? :)

      And just to be sure you got it:
      What is 10% carb from 2000 calories (=200 calories = 50g).
      What is 10% carb from 1000 calories (=100 calories =25g).
      What is 70% fat from 2000 calories (=1400 calories = 156g).
      What is 70% fat from 1000 calories (= 700 calories = 78g).
      "Uhmmm.... I don't get it...if i follow macro percentages i am reducing in fact fat intake, from 156 to 78!”
      Well...yeeees...but.... how about you stick with 50g of carbs even on 1000 calories (if you were in ketosis with 50g...why change?)....if you do that 50g carbs = 200 calories, 100g protein = 400 calories (we keep this constant also, why change... protein is essential)....we add it up...200+400 = 600 calories...that leaves only 1000-600 = 400 calories for fat = 45g !!!! How about that ...even less fat from diet thus more burned fat from storage. In conclusion...follow quantity (grams) not percentages which are fine in the beginning but stupid on long term. Yes you can decrease carbs by following that...but you also decrease protein which is never a good thing (protein gives us essential amino acids which are the building blocks of the human body... carbs/fats are only the fuel to run it!!!!!!!)

    It's amazing that doctors still recommend low fat, high carb diets for weight loss, it's hard to believe! Low-carb diets have long proved themselves superior.

      That day I was sitting in a lounge doing Sheesha, when a common friend i met for the first time, and he is a Doctor in the most prestigious hospital in india ( AIIMS) and I thought of talking to him about low carb ( could not help myself when he told he has specialization in metabolic stuff 🙄 , I should have stopped there)
      So I tell him about obesity is hormonal and about low carbs, guess what he says , we should eat all macros in equal quantities, that's balance diet .. when I talked about insulin he started eating as if I am some unimportant person talking, but atleast I tried 😋 also btw, his client is 130kg ( my friend) with whom he , the prestigious doctor came, and my friens is struggling and not loosing, sad but slowly I guess these stereotypes will improve

      Depends on activity levels. For non-athletes, low carb seems superior. Higher carb works better for higher activity levels and/or regular supplement, hormone, or drug use.

    I can't help but notice that all of these charts just say weight regain and not fat regain in the "check mark" so this doesn't account for the idea that people, once they get to a weight they are happy with could easily be commencing to gain weight in muscle. It's also a relatively small gain over a somewhat long period of time which could be accounted for by people dialing in the weight they're happy with maintaining. There doesn't seem to be any indication whether compliance might be becoming lax towards the end because it's much easier to be swayed to eat just one small piece of cake when you're 150 pounds and think you can now get away with it than when you were 215 pounds and determined to play the game to the letter. Also, I might be dense but it seems like he is saying consume less calories at a point when you're potentially getting less energy from fat stores so doesn't that suggest upping the calories (from external fat sources) in order to keep up with the metabolic demands of the body and not cutting them because of some notion that keeping calories low is somehow vital maintaining weight when they didn't matter as much on the way down while LCHF?

    i started on keto and doing fasting, and lost about 35-40kg, then some life happenned and i had to go on an anti anxiety drug called olanzapine, and even though i was on keto, i started putting weight on again becasue i was hungry all the time, eating loads of steak and basically drinking more than a litre of cream a day, probably around 4-5000 calories total per day. just couldn't handle the hunger, so i know it's not just carbs that make you gain weight, they're just a crazy accelerant for gaining weight.

    That is very interesting. Never thought about it like that. I add extra fat to ensure ketosis, but notice over time my protein content increases. Carbs only ever about 10gm. My problem was I was losing way too much weight and had to put back on 5kg, which inevitably was fat. So now I am a bit confused about my diet mix. But I gather if I keep my Carbs and Protein constant I use fat content as the accelerator/break to manage weight. So I will give this hypothesis a try. But the end target I think is to find a standard way of eating where you don't have to think out each meal.

    Nicely explained, I have been saying this for Years. It’s not just hormones or calories.
    It’s both! :) my keto program has a 95% success rate because of it :)
    I lost 90 lbs and have maintained it now for 3 years and counting :)

    excellent presentation but then he misses its purpose. He starts why people regain weight after a year despite following the same diet. And he doesn't really conclude on his initial question. He vaguely suggest reason maybe too few calories intake... and slowed down metabolism?

      I believe the point was...there's a point, on a low carb high fat diet, where calories do begin to matter. I've seen this in my own experience. My carbs might be extremely low, like the carnivore diet, but I lose no weight. Once I drop my calorie intake, weight loss begins again. But, one nice thing...I don't feel "starved" as I have on normal low calorie diets.

      He did answer but it was not obvious. After losing weight your metabolic rate changes. In order to keep losing weight (if that's what you want) you need to adjust calories IN accordingly. If you don't, at some point you'll gain again...as what was once a deficit now it's a surplus. His explanation was in the tables. Maintaining carbs and proteins (in grams, not in percentages) constant...one should reduce fats directly connected to his (resting) metabolic rate. Your calories OUT are decreasing so you need to adjust the calories IN, by tweaking fat intake (what is slightly confusing is his body fat consumption column, but he was just trying to make a point on a 2k calorie vs 1.1k calorie diet)

      Listening from 27 minutes again, here is what I understood. The person starts of ingesting 2040 calories. As the person becomes more "fat" adapted, to allow for weight loss, we need to reduce the number of calories we get from fat. If we do not, then the body will stop burning fat and we will stop losing weight. Observe that the total calories out remain the same as the amount of fat ingested is reduced. In the last chart, he removes references to the fat taken from the body to make a point that we need to reduce calorific intake.

    So, if I understand well, after a period of LCHF diet where you can eat Unlimited, you should continue with LCHF but introducing less fat and calories.

      @TSD TSD the point is that you should stop eating automatically if you introduce enough fat. sure that we live in an abnormal adundance historical period, so that it's not easy to self-control.

    Yes, calories matter, from my experience. Keto for about 8 years. But darn, I love the food. A lot of butter and thick cream, I had it in my head that I'd stay slim no matter what I ate on account of the idea that it's raised insulin that generates fat creation. Turns out, that's not the only pathway. Spotted my reflection in a shop window at the beach, was surprised at the paunch.
    Years ago I'd noticed I had no appetite for breakfast, like a lot of low-carbers, and I had just started noticing that I had no appetite for lunch either. Leptin I expect, who cares? So, easy solution, skip lunch too.
    Now I'm OMAD (one meal a day), for the past five months. Lost about 8kg, I think, paunch all but gone. I don't weigh or measure, I do a once-a-month check taking a video of myself in a swimming costume, the picture tells the story, and I'm looking lean, probably about two months to go and I'll have to worry about under-eating.
    (I've figured out that my lunch used to be about 1200kcals pd. One thing I've learned, don't compensate with bigger suppers. My problem is that I enjoy food too much. Speaking of which, I can't understand the argument that keto is unsustainable - I enjoy food more on keto than before.)
    MORAL OF THE STORY: 1. Calories matter. 2. But low carb makes calorie management easy (probably impossible on high carb). 3. Low carb makes Intermittent Fasting easy (23/1 every day in my case) for those who buy the IF story (not including me, I surmise that IF works because it reduces carb effects). 4. On low carb, appetite reduces - this creates problems because it can be tricky to know if you're over- or under- eating, in my case I had to alter living habits and check with monthly video selfies.

      You know it sound corny but after eating food cooked in Lard, you will never ever go back to seed oils. It's pure, clear, it has no odor and it makes anything you cook really crisp. What's more doesn't smoke at any temperature and last forever in the fridge.

    interesting presentation but although title for the presentation mentions lipedema I did not hear Dr. Gerber make any reference whatsoever to lipedema so I was a bit disappointed.

    nice lecture but sadly it does not resolve the initial problem that you showed at the beginning. my weight loss curve looks exactly like that. i lost 25kg in the first year of keto. the second year the weight stagnated and now in the last 7 month the weight kreeped up 1kg per month.
    i am still on keto but my metabolism is down. the only way out that i see now is getting some levithyroxine from a doctor to speed up my metabolism with thyroid medication. and maybe also supplement with some good 5-htp.

    There are several solid LC advocates out there (eg Ted Naiman) who correctly state that HF will make you fat or at least prevent weight loss. keto was first used to help children with epilepsy. That extra fat may have helped them in some way, but it’s not beneficial for most of us.

      Dr. Naiman's P:E Diet and online calculator show you the effect of adding "energy" (fat & carbs) to your fat-losing capacity. Keep your P:E ratio above one and you should lose weight again.

    Curious if mitochondria will change the number of pumps on their cell membrane in the face of different fuels? I.E. if you eat low carb high fat for a year will your mitochondria have many more fat pumps on them than glucose or is the ratio of glucose and fat pumps fixed?

    Amen! I have been on the same thing for 1 year. Keto lost 45 pounds in the first 6 months but put back 20 pounds in the last 6 months. Fasting, OMAD. IT IS MADDENING!!!

      I only lost 40 lb and have regained any. From the start I've done a 36 hour fast once a month only. Then ate little the next day. Then back to something like keto not counting anything, but vastly improving the quality of my food. Hopes this helps someone. The quality, I believe, is important.

    He never explains why the KETO dieters gain weight. Are they still KETO? I would think the u shape curves need input from the subjects. I also think that pushing past some personal weight threshold is possible under KETO but your body needs some protective lower bound mechanism.

      Mary Harper you can still gain weight on keto if you are eating too many calories. Eating large amounts of dairy, for example, is keto, but adds extra fat above what is needed.

    After listening to just his introduction and identifying 100% with what he's saying about the checkmark, I have to say my response to my failed efforts to keep off weight, losing it can be easy - the only conclusion that makes sense - is...
    MY BODY WANTS ME FAT!
    Now I'll return to the video to see if this doctor has something helpful and effective to teach me.
    P.S. I'm diabetic and have been on keto for over five years and carnivore for the past 7 months. I lost 53lbs. Gained back 47lbs. Lost 37lbs. Gained back 15lbs. I am back on keto because of my elevated fasting blood sugar and I've stopped losing any weight.
    My fasting blood sugar went up between 30-50 points on carnivore, without overnight insulin. My fasting glucose was lowest with juicing, mostly cruciferous veggies, celery, and cucumber. No high sugar fruits at all.
    I am the exception to nearly every nutritional rule of thumb you have
    Ha! I truly hope not.

    Damn! He’s talking about What I just discovered and I am sooooo sad!
    I’m eating carnivore these days for muh reasons and recently discovered how satisfying and satiating BACON can be!
    But guess what?
    If you eat up to a pound and half of bacon, that’s OVER 2,000 calories! So unless that’s all you eat all day you are very likely going to eat TOO MANY CALORIES to lose weight.
    Like I said, DAMN!

    1tb ground psyllium husk, 2 tb activated charcoal, drip warm water while kneading. Smear mixture on gauze and then tape to body part needing attention whether a bee sting, inflammation, over the liver ect. Leave on no longer than 8 hrs. Melts swelling and edema away!

      @@thalesnemo2841 I have a book "Charcoal Remedies" but there's no peer reviewed proof . My life has been saved by these therapies. Internal and external charcoal. Water and dry fasting coupled with a low to keto diet. It's always funny until its your neck on the block when the health fails and all the docs can offer is another miserable diet / medication plan. It seems some of us need an extra punch of detox and cleansing in addition to diet , exercise. 👍

      @@ImplyDods I used to mock and laugh at the very idea of a poultice. Now that it's changed my life I'm more than willing to be laughs at. Just remember the joke so if you ever find yourself desperately needing help that main stream medicine can't offer , maybe you'll give it a try. I thought "This is so stupid" while I was putting it on the first time. I put it right in the middle of my chest for breathing issues and heart palpitations. After 4 hrs I took it off. Cleaned up. Layed down to sleep and was amazed as my heart was a whisper and my breathing was deep and easy. No amount of beta blockers , pain killers , or any number of other therapies has done for me what this has done 👍
