A glomerular filtration rate (GFR) test is a blood test that assesses how well your kidneys are functioning by measuring how much blood they filter each minute. If the kidneys are damaged by disease, their filtering capacity diminishes. While GFR can be measured directly, this is often complex and time-consuming, so healthcare providers typically estimate GFR (eGFR) using the levels of certain waste substances in the blood, primarily creatinine.
The eGFR test is a generally reliable method for evaluating kidney function, although it may not be accurate for those under 18, pregnant, very overweight, or very muscular. To find out your eGFR, a blood test is conducted to measure creatinine levels. The result is then used to compute eGFR, which will be reported to you along with a lab report. Discuss your results with your doctor to understand their implications.
Routine eGFR testing is crucial as early-stage kidney disease typically lacks symptoms. It is also recommended for individuals at high risk for kidney disease, such as those with diabetes, high blood pressure, a family history of kidney disease, heart disease, over the age of 50, smokers, or those with obesity. Symptoms indicating later-stage kidney disease include swelling in extremities, changes in urination frequency, dry skin, itching, fatigue, muscle cramps, nausea, and loss of appetite.

The eGFR (estimated glomerular filtration rate) ranges and corresponding stages of kidney disease are as follows:
    1. eGFR: 90 or higher: Indicates normal kidney function but may have signs of kidney damage, such as protein in the urine or physical damage to the kidneys. This is classified as Stage 1 kidney disease.
    2. eGFR: 60-89: Represents a mild loss of kidney function with potential signs of damage like protein in the urine. This is classified as Stage 2 kidney disease.
    3. eGFR: 45-59: Indicates a mild to moderate loss of kidney function, possibly with symptoms such as swelling, back pain, and changes in urination. This is classified as Stage 3a kidney disease.
    4. eGFR: 30-44: Reflects moderate to severe loss of kidney function, with symptoms similar to Stage 3a. This is classified as Stage 3b kidney disease.
    5. eGFR: 15-29: Signifies severe kidney damage with symptoms like high blood pressure, anemia, and bone disease. This is classified as Stage 4 kidney disease.
    6. eGFR: less than 15: Indicates kidney failure or end-stage kidney disease, where patients often feel very sick and may need dialysis or a kidney transplant. This is classified as Stage 5 kidney disease.
