Crash Truck Dash Cam: AA 383 Fire at O’Hare (Fire Trucks Nearly Collide)

  • Every video of the response to the uncontained engine failure of a Boeing 767 (American Airlines Flight #383) on departure from O’Hare International Airport contains a wealth of information about the incident and the fire department’s response to it. While the overall theme can be appropriately summarized with “job well done” there are certainly lessons to be learned. (And they have been.)
    It’s not often that an unscripted video contains sound effects that adequately convey a sense of what’s going on, but the scene beginning around 3:25 in this particular video sure as hell does. (Watch your speakers.)
    One thing which is particularly noticeable in this video is the effect of wind on firefighting operations. Even flowing well over 1,000 GPM with enough force to throw a straight stream over 200 feet, the wind has no trouble interfering with foam/water/Purple-K application. (The Purple-K flow is quite evident in this video, and even more visible in an upcoming video. It’s pretty neat, it's expensive, and it's also the source of an amusing story about this response - to come.)
    Shortly, this channel will be posting additional videos of the response to AA 383. They will include views from other crash rigs along with new and interesting information about the incident itself. (Hey, you didn’t know about the near-crash, right?)
    One light-hearted teaser: The AA 383 fire occurred the day prior to one firefighters scheduled retirement. As of that morning he had not been on shift for the response to an aircraft fire. Ever. (Yes, an airport firefighter. Planes are safe.)
    There isn’t much that an ARFF firefighter enjoys more than playing with their toys - and they rarely get to flow foam during training - so it’s perfectly understandable that you can hear that very firefighter happily saying “I got my fire!” on the ride back to the station for a tank refill.

      When you're task saturated...AKA "Overloaded"...the first sense to go is hearing. They literally didn't hear it as their total concentration was on the preparation and execution of their mission. Don't know why they didn't reset it once they were RTB though...maybe the adrenaline was still pumping or they were thinking about all the paperwork and clean up!lol!

    Amazing job by all involved that day. A lot of lives saved. From the quick response of the flight crew, airport dispatchers, fire crews , flight attendants evacuating etc. Things work like they were supposed to in an emergency situation like that

    I read the interviews from the flight crew and it sounded like they did a fantastic job getting the idiotic passengers to safety. It's ridiculous that people were concerned with their luggage while the plane was melting around them. Also congratulations to the idiot in 20B who learned about jet blast the hard way. I bet he'll follow the flight attendant's instructions next time.

      They weren't responding on a building fire or smoke in a building, they were going to an aircraft on fire. PASS devices are great for a unconscious firefighter in a building with zero visibility.

    The approach in basic ARFF is 45 degrees on the up-wind and up -hill side of the emergency? The two pieces of appartus that almost collided was classic! I love the PASS alarm going off, like its part of the warning system, or is it that someone spazzed out and doesnt know how to shut it off?? The conversation in the cab is rich!!!! Bascially, lets keep shooting foam, till we run out and don't utilize aggressive turret/deluge placement. ARFF is a very tricky business. You shine, when you're training pays off because the "big one"comes in on you're shift. You need more practice gang!!

    I loved the wipers keeping perfect rhythm with the alarm....
    I noticed that the firefighter that were working on the ground seemed not to be wearing the silver fire suits (I'm at a loss as to their real names). Why? Is there a different suit that's used now?

      Just normal structural gear. Truck technology has come a long way so most of the heavy lifting is done with the wheels. The proximity gear is probably still in use somewhere but I'm not sure it's in use that often in NA anymore.

      Those sliver suits (called proximity suits btw) are rarely used for many reason. Those suits are only really used for chemical tank/high heat long burning fires, and oil field fires. Structural turnouts are sufficient for ARFF.

      The elevator might be composite but the wing, which holds the engine is aluminum. It looks like the elevator is de-laminating....quite the sight.

    Looks like a job well done close call with truck passing in front like that, l understand it can be a bit of excitement in the moment. But you still have to be aware of trucks etc on fire ground still fire was put out so job done.

    The pilot and co-pilot having 23 years combined active military flight servive and 40,000 combined flight hours, almost 6,000 of them being behind the controls this exact plane, explains why this evacuation went so smoothly and no lives were lost.

      This evacuation did not go smoothly, for several reasons. First, the left engine was not shut down during the pax egress, causing a serious injury when an evacuee was knocked over by jet blast. This could have been much worse. Second, the attendants could not speak to the flight deck because the interphone on this 767 was different to the one they had used in training. And third, many passengers, despite specific instructions from attendants, attempted to take their carry ons during the evac. The first and third of these are not uncommon during ground evacuations, leading the NTSB to make specific recomendations as a result of this incident.

    Going off memory from several years ago, seeing that right wing burned up is alarming given that the 767 holds around 5000 gallons in the wings and not counting the fuselage tanks. Yikes. Neat video

      I actually have the numbers from that flight. On takeoff the plane had 42,500 pounds of fuel onboard, 6,259 gallons pretty evenly divided between the wing tanks. The estimated amount of fuel lost to fire is 2,040 gallons.
      (NTSB calculated that approximately 350 gallons were used for the aborted takeoff roll, and after the incident 2,917 gallons were removed from the left wing and 950 gallons removed from the right. I added the page with those to the photos and documents at tiny.cc/AA383Pics.)

      Heavies, including the A330,A350,787,747,A380,777s usually carries fuel in wing-tanks Only for the "short" Atlantic hops. The Wing-Box "the empty center fuel tank" is filled with an inert gas since the TWA flight 800 tragedy.
      And if a Heavy does have fuel in the center wing tank.....on-board nitrogen (in tanks) or created from the Fan-Jets...is automatically pumped in the center tank as soon as the flight crew throws a couple of switches.

      PASS stands for Personal Alert Safety System. It is designed to sound off an alarm when a firefighter is not moving for a certain amount of time.

      Actually the audio of the firefighters talking to each other has been turned down a bit. Also they did the right thing cause as you can see the foam was laid on the ground to stop fire spreading on the leaking fuel and than foam was sprayed onto the wing and engine. But there is always additional learning that can be done with this video

    I watch a lot of these fires on here I don't understand why firetrucks don't have a front nozzle like the firetrucks at the airports have.Then they could be putting water on a fire a lot sooner and it could even save someones live. Because from what I've seen on here it takes the firefighters some time to hook up the hoses and start putting water on the fire.

      timothy prescott the reason why we don't put water on a structure fire is what is called the lobster effect. You put water on a fire it becomes steam. The life you are trying to save now becomes a steamed lobster. Interior crews have a line. As they clear an area they can spray water. Again they aren't immune to this. The bunker gear shields us from direct flame. We can still get steamed. Hope this helps to explain it. A lot of rigs have a deck gin we use in a surround and drown situation.

    Interesting to note how long it took to cool the area down sufficiently to prevent re-ignition. Does foam have a much lower cooling effect than water ?

      John Murrell usually water just spreads burning fuel. Foam suffocates the fire to put it out. Cooling isn't an option with the high octane of jet fuel

      Hello Brian,
      I understand that one cannot use water but what struck me was how poor the foam was at cooling the hot material to below the re-ignition temperature. The fire must have relit about 6 times when they stopped pouring the foam on. It was a good thing the passengers had gort out and the fire was not that close to the fuselage.

      Yeah i was under the same understanding as well. It almost seemed like the foam was watered down and wasnt coating the aircraft like it should. I do know the aircraft turbine has many different chambers in it that the fire could easily rekindle from built up heat, but youre right that the foam should have been more effective

      The first fire engines left at speed before the situation was fully under control. I presume they had run out of foam & water and had to go for additional supplies ?

      Airport crash trucks use Type B Foam in their water supply. It's meant to coat liquids and cut off the oxygen supply to the fuel.
      The reason it's hard to cool the engine is due to all the compartments and having a fuel supply that the engines can't coat.
      Fuel shutdowns only work if they haven't melted and failed.
      You'll see the crew blasting water in both the inlet and outlet of the engine. That's the most effective way of extinguishing an engine that's out of control. Meaning it's burning fuel without the control of the pilot.
      Except for the engineer that almost collided with the truck, this was a text book call and all around good job.
      That engineer will be reprimanded after reviewing the footage.

    Someone should’ve had a FLIR heat thermal camera to see through the smoke to aim the foam on the fire. It look like you were shooting in the blind

    Just a suggestion from a pilot - you got the harpoon just sitting there. The fire keeps reigniting because one of the fuel bladders is compromised. Jam than harpoon into the wing and fill that bladder up with foam and water the the Kerosene won't burn anymore.

      But then you are just forcing the fuel out of the already leaking breach by adding foam into the tank. The inside of the tank isn't burning, it's the fuel leaking out of the tank.

    I can’t believe that they allowed that pass alarm to go off for the entire response, I would have ripped that airpak out of its holder and reset it or thrown the goddamn thing out of the rig. That near collision by the that other ARFF rig was intense as well.
