Hi Dr. Barbieri, thank you for the informative video. If I may ask, is using Sarecycline acceptable for patients whose PD is not due to utilizing topical steroids? I haven't been able to locate too many papers discussing the efficacy of Sarecycline for PD. The papers I have found utilized a small sample size/have been focused on patients with SIR. Also, is using Sarecycline considered a novel approach to treating PD? Trying to gauge how open primary care doctors might be to giving it a shot with their patients given seeing a specialist can be a long wait. Thank you so much!
Great question! There are unfortunately few high-quality trials in perioral dermatitis and it has no FDA approved therapies. There are some case reports that suggest sarecycline can be helpful in perioral dermatitis (pubmed.ncbi.nlm.nih.gov/38912196/) and I would expect that it does not need to be steroid-induced for it to be helpful. However, due to cost, access can be a challenge (though there are some patient assistance programs for those who are eligible).
Tacrolimus is generally considered to be a safe treatment to use for prolonged periods of time. If perioral dermatitis is persistent or recurs, it often responds to the same general treatment strategies.
