  • Upper extremity sensory complaints include a differential diagnosis of nerve root entrapment or irritation vs. peripheral nerve entrapment. Succeful treatment is based on an accurate diagnosis. The motor, reflex and sensory testing is always supported by orthopedic tests and, potentially, electrodiagnosis.

    That is brilliant. Using MRS for both Spinal Nerve Root & The Peripheral Nerve & differentiating SNR/Dermatome from PN/Pure Patch & their INDIVIDUAL MRS's is just the best manner I have ever seen if differentiating the two possibilities in a differential dx. Thank you.

      Holy hell. You didn't know​ this? You might be doing unnecessary cervical spine surgery on someone with a peripheral nerve entrapment!!! Oh my God, this is hugely disturbing!

    The radial n does supply the back of the hand. Understanding the pure patch location allows for differntiating a nv root problem from a peripheral n problem. C6 overlaps the radial area as well. Pure patch sensory loss suggests the peripheral n lesion as opposed to the n root, but in clinical context, you would confirm this with motor and reflex texting. I hope this helps...

      NeuroDocUniverse I feel like I am missing something. Radial is exclusively sensory on the dorsal aspect of the hand.I also found useful for nerve pain is Skyarza Neuropathy Treatment Star - if anyone wants to know more search on google.

      Nusrat Prodhan The radial nerve sensory area is the dorsal area of the thumb, index,
      and middle finger. The radial nerve extend to the posterior aspect of
      the forearm and to the lateral aspect of the lower upper arm.

      @@neetikagaur1082 Sure! A nerve root refers to the mixed (sensory and motor) spinal nerve located next to the spinal cord. Peripheral nerves are structurally distal to the nerve root.Typically, nerve roots are numbered (C5, T1, L4) and peripheral nerves are named (ulnar, tibial, long thoracic). Peripheral nerves can carry motor and/or sensory information. Let me know if you need any further clarification.

    While your considerations of the anatomical relationship between nerve roots and peripheral nerves is admirable, clinically it might be simpler to regard nerve roots and peripheral nerves as separate entities. This will help you with the M-R-S evaluations.

    Can you explain the MRS of the the Radial Nerve, with the sensory deprivation of the Pure zone on the dorsal aspect of the hand with a little more detail?? I feel like I am missing something. Radial is exclusively sensory on the dorsal aspect of the hand, correct?

    Hello, these are my issues: right arm. Does not swing when move, dead weight. Pinky finger and ring finger weak tingle. Hand stiffness. Bicep smaller than the left. Muscle control lost, or losing. Constant ache in arm and hand. XrAy negative, mri of neck apparently showed “pressure” on disc. Family doctor said tennis elbow, but I doubt it. I need help. I think I need a nerve conduction test. I’m 40, adult ice hockey. Ive had dozen of collision to my head..swip lash etc..

      GoPro Goalie Uzi could be compression of C8 at the spinal root level since u mentioned the MRI. Have you had a cervical spine screen done yet? I would also recommend looking up how to do a tension test for the ulnar N, and maybe radial and median too just to rule those out. If you tested positive in more than one peripheral nn, could possibly be thoracic outlet syndrome. Hope this helps a bit! Feel free to message me if you’ve got further questions.

    i thought a reflex is monosynaptical e.g. the c6 root , so this wouldnt be significant for the n.musculocutaneus nerve (c5-7 , same as the n. thoracicus longus ) but my english is not the best , so maybe i missed something important in your vid,

    Doc do you believe nerve entrapment can be healed naturally if disc is not herniatied but you had a pinched nerve with relief from all symptoms I slept wrong one night had some trouble for a bit have an mri coming soon just to make sure of no herniation, chiropractor told me just a pinched nerve

      II cannot make a call in your case since I have not examined you or seen your films. However, impingement of a cervical nerve root can come from a number of issues. Some of the causes can resolve on their own (chemical radiculitis, edema, etc.) and some causes are unlikely to resolve on their own (ligamentum flavum hypertrophy, osseous foraminal encroachment, etc).

      Hi! Pure patches contain a large number nerves from one peripheral nerve and is a sensory area that can be used in exams to help distinguish one nerve deficit over another. It's not exact and people vary due to the number of overlapping areas. I hope this helps!
