    I can tell by the look in this lady's face she has had the pain for a long time. It's similar to the way I'm feeling after 6yrs of pain due to C6 nerve compression! It's actually soul destroying!

      Graham, Absolutely Correct .. I'm worse than that : C4-C7 disc bulges and degenerated Discs. And T1-T2 T3-T4 have aerothelioethesis (basically like dislocated /misaligned Thoracic Vertebrae with Stenosis into the spinal cord. For many years, on a progressive basis. I just now started with a new PT (after trying many others AND 6 different Chiros). There is no Prescribed Surgery per my 6 differnt Consultations. meaning the success rate would be less than 75%. By today's standards according to an Ortho NP I met, Surgeons will NOT do the Surgery on your spine unless they find an indication that the Success rate is over 75%. If you end up worse, or with pain and discomfort that returns in a year, it reflects on Them and the associated Hospital. I scratch my head everyday because We can send a Rocket to Mars carrying a Remote Controlled Rover vehicle there , control it form millions of miles away with precision, Yet we don't have better way to Fix Spinal Issues in the Human Body??

    I have seen I would say 10 seperate specialists, ED doctors, surgeons etc for my multiple fractures of my thoracic spine and multiple herniated discs in my cervical spine and had 2 spinal fusions. Only once did I have a proper assessment like this after suffering from a suspected TIA by a student Dr. Not one other dr has gone into this amount of depth in any assessments, the worst was a neurologist, who didn’t even write a follow up letter to me after paying 500 dollars or a consult. During the exam I couldn’t even walk in a straight line and my balance was terrible! I am in so much pain. My kyphosis in my thoracic spine is currently at 89 degrees and lordosis in my lumber and cervical spine are way off too! I’m so sick of health professionals not doing there job properly. Well done to this doctor who takes his job seriously and obviously cares about what he does and knows what he is doing. Thank you for the video. :)

    Really enjoying the way you are describing what you are doing, how you are examining your patient. Really a must see for any medical student, physio, orthopedic surgeon, general surgeon, GPs, etc... Very few people are actually so talented to present a neurological examination " that makes sense".     I noticed you asked about ringing in the ears. I have cervical rad, and C6-C7 disc disease, paresthesia in right arm, index, middle finger and actually on my face a little too. I only had an XRay. MRI's in a week. I developed Tinnitus in 1 ear about a month ago. Can the Tinnitus be related cervically?

    I have C5/C6 foraminal stenosis or narrowing and I have pain in my shoulders when I sleep/lay down on my back is that only from a herniated disc or could it also be with foraminal narrowing (DDD)? That is why I am here, you show "C6 Nerve Compression" at the top left in green in the early part of the video?

    Awesome. I love how you checked the reflexes which I don't see many do. Also how you first ruled out the red flags. I suppose we need not do VBI test if the red flags are ruled out. Should we?

      Correct. NRC is usually only one sided and only hits upper or lower not typically both

    I have brachioradial pruritus and suffer from insanely intense itching in my left forearm and outside bicep.
    I have looked this up and it appears to be caused by the nerve root in C6 being compressed due to degenerating disks in my neck. Is there any procedure that can stop this itching?

      Do you think it might be more of a "pins and needles" rather than an itch? If so, it can be coming from your neck.

      I have this problem too. Definitely an itch. Absolutely unbearable. Only ice packs help a bit.

    Sir, can you please please advise my aunt regarding her disc prolapse at C5/C6 causing moderate C6 root compression and mild , posterior disc bulge at C6/C7 causing minimal sac compression. The MRI also says early cord ischemia. Will she really need a surgery or physical therapy will heal her ?

      This is definitely a question for a neurosurgeon which I assume she has met with. If there is cord ischemia she may need surgery.

    Thank you for posting this very informative and helpful video. I was helping my girlfriend move. In 2017, I turned 51. I was using a single hand truck to move a double door refrigerator. I had to dead lift it up one large step and over the doorway entrance of the kitchen area from the back of the house. I am left handed. Later that night, the next, day, and for weeks, I started having pain on my left side around my left lower shoulder area where the shoulder blade flexes near the spine. I used a tennis ball and lay on it to try and massage the area. It helped a little, but I ended up using liquid Fire and Ice to help with the symptoms. Well, since that time, September, 2017 til now, February 2020, I have had numbness in my left hand, left index finger. It sometimes improve a little when I tried a few exercises I saw about a year ago, but the problem has never gone away. It almost seems like I tore a nerve or something because the problem never went away.
    Best guess from this video, it seems as if I have a C7 Pinched Nerve. Can you give me any guidance on how I can return normal feeling to my Left Index Finger? I would appreciate any response you can provide, as I can go and see my chiropractor if that can solve the problem. Thanks in advance. Mark, Fresno, California. :)

      I would start with an MRI of the neck and see if there is significant nerve compression. Maybe a little traction could help.

    Hi there. Can you please explain how this patient's pathology is a cervical herniated disc if her symptoms reduce with passive mid cervical flexion in the supine position? Anatomically, the pulposus should displace posterolaterally to her left side with cervical flexion, as a result impinging on C6 nerve root. Her symptoms seems to be caused via EXTENSION of the cervical spine, which resembles more of a lateral foraminal stenosis/degenerative disc type of pathology. As a physical therapist, this is important because it guides the treatment plan more thoroughly the treatment in regards to a disc herniation and a foraminal stenosis differ greatly as far as directional preference.

      Hi Vincent, I just typed a huge response and lost it. I will give it another shot.. So, when the disc is herniated the nucleus polyposus takes up space in the lateral foramen along with the nerve root. When you extend the foramen closes and the root gets pinched. Now, when the patient is supine and you flex the neck the NP does not migrate posteriorly because the neck is not being loaded axially. I know we were taught that neck extension "pushes" the NP back into place but I completely disagree with that. You squeeze jelly out of a jelly donut and see if you can push it back in place. I think extension can be helpful if there is a bulge only. When you extend the neck, the anterior disc separates and has a "suctioning"effect. I have treated hundreds upon hundreds of people as in this video and I have come up with the same conclusion. The foramen is crowded with the nerve root, swelling, inflammation, disc material and often a hypertrophied facet and it does not like extension. With that being said, I do not force flexion unless it is an isolated facet issue. I hope this helps. I would have made the same comment 20 years ago but learned from experience that I was heading in the wrong direction. I'm big on traction and posture.       Do you tend to see a patient age trend in regards to the likelihood of the presence of herniations? In regards to the thought that loss of water, loss of glycosaminoglycans, and loss of waterbinding elements tend to make it less likely to be a herniation in more particularly older, geriatric patients with a recent onset of cervical radiculopathy.
      Great cervical spine special test by the way! I will be sure to attempt that and document as the Marquis Maneuver.

    Hello I have been having the same symptoms as the lady on this video but I also have rotator cuff problems and shoulder blade pain. It started with should blade pain and I continued boxing which led to the pain and tingling down my arm. I also noticed major bicep function loss. This is all along my left arm. Any help I keep talking to doctors but they don’t seem to help. I got an mri of my shoulder and nothing showed up. I keep telling them it’s a disc in my spine or neck but they don’t listen. Too my luck I can’t afford health insurance because I’m only 21 and still going to school. Any suggestions ? I was thinking of an mri of my spine or neck but I don’t know which to do. Also where are you located ? I am in the city of Los Angeles.

      Yes MRI and I had a injectionon on c5 but I don't remember the name of the injection, but it didn't work for me now I'm going to see a specialist, but for you it may be different

      I literally have the same exact symptoms as yours. Mine started months ago and was only a very mild discomfort in my right shoulder blade, but about a week ago, it started on my neck, all the way down to my right arm and into my finger tips. Pain gets worse as soon as I lay down in bed. My sister is a dr and by me telling her that over the phone, she immediately said it was a neck Cervical desk prolapse. From what I’ve read so far and comparing it to my symptoms, i believe she’s right. Best luck to ya.

    Hello Paul,
    I am facing a similar issue. MRI says I have Mild diffuse disc buldge ar C5 to C6 causing spinal canal narrowing and causing right mild neural Foraminal narror.
    C6 to C7 Diffuse disk bulge with right para central disc protrusion cause Spinal Canal narrow and right severe, left mild Neural foraminal narrowing.
    Ortho suggested rest with intermittent Traction and some medicines like miofree A and Pregalin along with mahacort.
    Please advise on this

      I agree with the traction. Often these will resolve over time but watch out for weakness. If you become weaker in the arm, contact your doctor.

    Sir i have some problems in C6-C7: annular tear, right posterolateral disc extrusion with right lateral recess and neural foramen compression with exiting and traversing nerve root compression on right side. Now i have pain during laying/ sleeping. So i am laying on medical bed which is lifted up around 60degree. I cannot lay as straight. When i try to lay on bed straightly i feel pain on my right hand. Please suggest any medical help

    Hi sir my name is sateesh. I am 19 years old. Graduate studying. I workout most of the time.I had pain in the back one morning(16/12/2018). Near the left side of the neck.Numbness all over the body, weekness in hands and legs.coldness of under the foot,etc.we went to hospital and consult neurosurgeon doctor.He checked mri test and blood tests.doctor said this condition is cervical transverse myelitis.we have done mri, blood,and csf tests.Doctor give steriods and antibiotics for 1 week.He was later discharged from hospital with prescribed medicine.now that's my situation is my legs are fine walking but hands are not working properly, tightness of abdominal,
    shaking of hands,muscle weekness,my hand fingers are not working properly,at least not writting my fingers etc..iam visit serveral doctors and using medicine. But my problem is not cure.my parents depends on me.sir please Help me

    I haven’t had feeling in any of my fingers for 5 years after hurting my neck nor can I feel anything on my skin on my c6. I can put a knife in it until blood comes out and nothing but a light pressure sensation

      Why would you put a knife in it at all, let alone until it bleeds? You have more serious issues than your C6.

    Dear Sir...i was doing weight training and i started feeling pain in my sloulder and delts while doing pust movements and lateral raise...initially the pain was only at the time of lifting weights but now it remains for 24 hrs and it came in triceps region also but no numbness or tingling...i stopped weight training and went to orthopaedic....got xray done...he told me that i have degenerative disc desease at c5 -c6...plz suggest me...is it really becoz of that.....?

      degenerative disc disease is very common and can be associated with nerve root compression.

    Thank you for sharing. I've had a MRI and showed cervical herniated discs. My major pain is left arm top of forearm when pulling motions. Chin ups and pull ups, choking someone in MMA and curls especially hammer curls and even holding a gallon of milk. It's only pulling motions. No pushing motions hurt at all. I've tried PT and meds such as Gabapentin with no relief so quit them after a couple weeks. I'd like to continue my activities in training a nd weight training but it seems like surgery is only option and not thrilled about that. Hoping for some new ddirection to get this fixed up without surgery. Any recommendations?

      She needed to undergo an anterior cervical disc replacement. She is doing very well.

    Dear Sir. Thanks for your videos. U are doing an amazing job and Demostration - with deeply explanation :) I have a question as I get confuced ! :) Bracioradialis muscle is C6? As I am aware of. Triceps is Just predominal C7 but also - Triceps C7 (Note: Some references include C6 OR C8, however C7 is predominantly involved.) and Also Biceps tendon reflex is C5/ C6 I think is C6 more involed? Please correct me If I am wrong. Biceps and Brachioradialis C5/C6 I thought that Biceps is more C6 and Bracio more C5?/ OR THE OTHER WAY around??? I am a bit confused!! Thanks for your video! Always amazing!

      Yes you are correct that there are combinations of nerve roots that affect each muscle. I have found though that C5 nerve root compressions exhibit themselves more with biceps weakness and especially weakness of supination.

      That space never increases with time. Cervical traction can help separate that space but only temporarily. Disc space only gets smaller in time.

    I have started to have shoulder pain, also numbness and tingling on the right index and thumb. I was about to go to the ER ! Who should I go to ? I went to physiotherapy and they only gave me median nerve gliding exercise

      Dizziness is not generally nerve related. Could be an inner ear issue.

    I have just gone through the same thing this lady is going through. After working on some shelves in my garage, I strained my neck and woke up the next morning with agonizing pain in my left shoulder and arm. The doctor tells me I have problems with C6 and C7, but I believe mostly C6. I'm finally starting to feel better, but I still have numbness in my index finger and thumb and a little bit in my middle finger. My forearm and triceps muscles are much weaker in this arm. My question is... is this weakness in my arm permanent, or will I eventually regain my strength?

      @@Nina-jc3be Yes, it has improved significantly since then, but I still feel a bit of numbness in my index finger and thumb. I had a steroid injection in my neck about two weeks ago and it helped a lot. I’m satisfied with how it is now.

      I haven’t had feeling in any of my fingers in almost 6 years. Sadly I don’t think it just heals on it’s own

    I thought c6 was the right side. The symptoms I have are only right side down to the thumb
