Midra would be fucking horrifying, seeing a human man with the eye of Sauron for a head who impales people in the most painful way possible, and the flame of frenzy will either make you, tear your eyes out, be a zombie, or straight up turn you to smoldering yellow ash. Plus he is 9 feet tall and you can see him from a distance because the frenzied flame is the brightest flame in the game (based on Nanaya's torch being a dying flame of frenzy)
For me it has to be Midra. It’s not about scary looks but rather the concept of frenzy itself. Just standing near him makes you loose control over your life so that the only option is to end it.

He literally becomes a personification of entropy, an avatar of an outer god that is capable of melting away even souls, and has the express purpose and ability to undo ALL OF EXISTENCE. Every other boss would still leave a world behind them. A few might leave it as a lifeless rock, but there would still be a planet, and reality itself, at the end of the day. Midra would melt it all away with the yellow flames of Chaos without a second thought.

I doubt he would have had the time to learn much more of it during the Shattering anyways, although tbf, we're never quite told how long it went on for.
No. It's because in the DLC Radahns spirit has been put into Moghs body by Miquella. He couldn't convince Radahn to be his consort in life so forced him in death. He first send Malenia to take him out but she ultimately failed. This is why You must defeat both Mogh and Radah to access the DLC as well as why, as told to you by Ansbach, that Miquella stole Moghs body.

Miquella is potentially the most terrifying boss in any Souls game. Imagine someone who could take away every single bit of your agency. Your will to live, your will to fight, your very freedom of thought. I would argue that that is just as scary

Not only that, but said rot can form new life that is poisonous/dangerous and has the potential mutate people/animals with super rabies. Other bosses may be more lethal in that they can incinerate you with magic fire, obliterate you with sorcery, and crush you in the blink of an eye with pure strength, but that is far better than suffering Scarlet Rot IMO. Alternative is Metyr for being so creepy and the idea of large dog sized hands jumping me is...
I actually think mesmar would be much higher because of the snake and the story of the garden and eden. And also it would be easy for people to say he his the devil. Plus because he does look human it would be very unsettling. Of course he can put the snakes away. But the fact that he can control how scary he wants to be could the most scary if u think about it because most people couldn't imagine that. At least to me he is the most scary well top 3 for sure.

AFTER further thought, I think the ‘scariest’ would be the dual chariots that grab you. Like seeing that thing out in a corn field and slowly rotates to you has a human face then sprints like a cheetah only to open up its cage and put you in a human blender.

i honestly thought Fortissax would be the scariest. like big dragon that dropps deathblight on everything, the kicker is we as regular humans do not have any resistance to deathblight so contact with it would instantly kill anyone. so like he flies over a city and you see fog clouds and once someone touches said cloud black spikes come out of the ground to just impale them. terrifying

The one that you know is chasing you, but stays out of sight easily because he is faster than you, smarter than you, more agile than you. And haven't ended you because he is still enjoying the hunt.

I actually think mesmar would be much higher because of the snake and the story of the garden and eden. And also it would be easy for people to say he his the devil. Plus because he does look human it would be very unsettling. Of course he can put the snakes away. But the fact that he can control how scary he wants to be could the most scary if u think about it because most people couldn't imagine that. At least to me he is the most scary well top 3 for sure.

i honestly thought Fortissax would be the scariest. like big dragon that dropps deathblight on everything, the kicker is we as regular humans do not have any resistance to deathblight so contact with it would instantly kill anyone. so like he flies over a city and you see fog clouds and once someone touches said cloud black spikes come out of the ground to just impale them. terrifying

I think Maliketh has potential as "an unseen stalker in the woods"-like horror villain. You know, like the Predator. The one that you know is chasing you, but stays out of sight easily because he is faster than you, smarter than you, more agile than you. And haven't ended you because he is still enjoying the hunt.
I think rykard would be the scariest imo, seeing a giant half man-snake monstrosity surrounded by lava slowly slithering through a city would be terrifying😰

Godrick - He's one of the more realistic Bosses in which someone COULD do this... I think someone in history did something On a MUCH smaller scale though if I'm not mistaken. More of a "To see if it's possible" type of thing.
Morgot & Mohg - As like with Godrick. More realistic and possible. There have been accounts of people who have been born with Horn-Like protrusion. Just not as severe as the Omen Twins
Divine Dancing Lion - ONLY if said people puppeting the Lion were huge Buff Men going on a rampage while being synchronized in their attacks
Malenia - Basically If Resident Evil actually happened. Which it could. Also giant STD Mommy? Some might like it...

Hell even the miranda flower looks terrifying too with its contrasting design element. These came into my mind instantly. If there's anything left i want other people to stress about it

Goddrick should be higher because most of the bosses will only try to kill you in the most strait forward way possible. On the other hand if your body is "interesting enough" for him , not only you will die but before that he will tear your limbs apart + becoming a revenant.
Intellectually, Midra is definitely the scariest and follow up is probably Astel. Most fear inducing on an instinct level is probably Bayle, fire giant or Messmer
