While I injured my medial meniscus 6 years ago while snowboarding, I like the idea of being able to get back to the mountain in the years to come with ACL reconstructive surgery. I’m terrified to have surgery but I know with my current PT working to build leg strength over the next 6 months I’ll be ready for surgery this winter.

I had the hamstring autograft and it caused nerve damage below the knee. And my hamstring is still feeling weird 10 years past surgery. Would had been nice to avoid the nerve damage.

I regularly am lifting and am very active with snowboarding, surfing, hunting, and other sports. Which graft would you recommend? Have had friends and family who have done allograft as well as BTB. I’m 26 btw.
Most autografts do quite well. Of course, it's also dependent on surgeon skill and a structured rehab post. BTB or quad tendon autografts are typical choices for athletes. I’m not sure if you will see this comment but, what is your profession of using a cadaver quad tendon as the acl? Last question….why do they drill a hole diagonally? Why not drill a hole from the tip of the tibia to the center of the femur straight up? A vertical ACL . I feel like it would reduce tearing since the acl would no longer be at an angle.

If you’re young and active, it's generally better to avoid allografts (cadaver grafts) due to their higher re-rupture rates. The ACL’s orientation plays a crucial role in its stability; the diagonal alignment is essential for its function. I am currently 3 weeks post op. My surgeon gave me the option and recommendation to do a hamstring autograft PLUS some additional allograft to supplement and beef up the new ACL. I have yet to find a video or forum that details a single example of this type of graft. Anyone here heard of this?
What about quad autografts? I got that graft for my aclr 3 days ago and I’m looking to return to college basketball. Do you think that choice could fail me?
Hello Sami, The QT autografts are the least studied of the 3 main autograft options. There are no prospective studies comparing QT/BQT and BPTB autografts and no prospective or retrospective studies comparing QT/BQT and HT autografts. For this reason, we did not have sufficient evidence to make any substantive claims in this video. Nonetheless, the evidence that does exist is promising for QT grafts demonstrating no significant laxity, high return to sport rates, and less anterior knee pain!

Hi sir, I'm a football player. I had torn my right knee ACL during landing. What do you think on using Peroneous longus graft? Will it have any side effects on my ankle stability considering my sport? If peroneus longus graft is good to be taken, from which leg could it be taken (considering that I am right handed)?
There’s also Quad tendon reconstruction and a new procedure that is called the BEAR Implant acl surgery which I think are better options for women hamstring autograft is not good for women it has higher chances of retear

I had ACL reconstruction surgery on 24/7/23.. Peroneus Longus was used on me. I'm an active sports player... PL is advisable for sportspersons

My doctor used peroneous longus graft and its totally fine … also it heated very fast
why not augment the allograft with orthobiologics to help it regain some or most of its mechanical integrity?
