  • Dr. Ron Israeli of New York Breast Reconstruction and Aesthetic Plastic Surgery (NYBRA.com) and Dr. George DeNoto, Director of General Surgery and Hernia Institute, St. Francis, Hospital, East Hills New York, give a presentation on the technique they have developed together for abdominal wall reconstruction.
    They have worked together to improve this technique for breast cancer patients who have had breast reconstruction using the abdominal tissue and have presented with hernias after the surgery.
    Their findings are published in the Atlas of Abdominal Wall Reconstruction.

    I am 20months post DIEP flap surgery and have been back to my plastic surgeon and a hernia specialist and neither of them have accepted that my bulge is a hernia - as you mentioned, my CT scan was done lying down. This video is so educational and I do hope it reaches surgeons here in Australia, as it should. Many thanks

    Wow! Talk about being fortuitous! I had a bilateral masectomy with DIEP reconstruction and December 2021, amd this is exactly what I am experiencing. My 2nd surgery will be next week, to repair or reconstruct this area. May I send you photos?

    So my plastic surgeon pointed out my Abdominal bulge on a recent visit, I asked him what can be done about this? He said, if you lose weight it might go down. I felt unsatisfied with that answer. I am seeing him again in January and will share this information with him and revisit the possibility of repair. Thanks for the video. My diep flap surgery was done in Sept. 2019

    Excellent video. I wish it hadn’t taken breast cancer to determine that I could benefit from repair of abdominal muscle separation that occurred with pregnancies 26 and 31 years ago and worsened over time. I have periodic back pain that I hope will improve with DR repair. It’s not just about the looks for me.

      It shouldn't be "just about the looks, Cynthia. As the saying goes in plastic surgery, the end goal is form and function. So in your case, hopefully that function, back issues, improves and I certainly hope so. Best wishes to you as you proceed with your decision.

    As always a fantastic educational video! Thank you so much for keeping your followers informed on all things DIEP related. This one really hit home because I’m concerned this is my situation. I see my ps in a few days about it. Is there a way to find out who the other surgeons are they have trained in their procedure?
    Also do you know how this compares to the Bars procedure in success, healing and long term outcome?

      Hi Julie. Not sure who else is doing this to be honest. I’m not familiar with the “BARS” procedure you mentioned.

      I e talked with a woman who had BARS procedure done after other failed mesh attempts to fix bulge. And it was successful. It ankers to the iliac crest and maybe pubic bone so sutures can’t rip out. I’d love your thought on it if you ever look it up.

    I am in UK and was wondering if it would be possible to have a copy of the slides please?
    I have been told my bulge can’t be repaired so I would really like to share this information with my surgeon.

    I had DIEP in 2015. I developed a bulge on the left that grew over time and was very uncomfortable without wearing a tight undergarment all the time. I had a mesh repair done by my plastic surgeon about 9 months later. The mesh helped with the discomfort but the bulge didn’t change much. I just saw this video and wonder if I should go for another surgery to correct this? Do I find an abdominal surgeon this time? Does the surgeon remove the mesh that I had sewn in in my 2016 surgery?

    I just had a muscle sparing tram . The dr said he took the lateral muscles , I am in pain and can see a small bulge below my incision. I am afraid I won’t be able to do alot of things, my surgery was oct 27 . I still need to remove my ovaries due to the cancer

    Thank you for this video! I had my DIEP in 2019 and I have a bulge that my surgeon says he can't do anything about it. I live in Montreal, Canada, do you know if some surgeon do the method Dr Denoto does? And is the procedure that they do is through laparoscopy or they have to open all the abdomen once again?

      You are most welcome Véronique. I’m afraid I don’t know of any surgeons in your area performing this surgery. I would use a Google search like “Hernia repair surgery in Montreal or even Canada.

      Surgeons are still performing them, however, I would seek a microsurgeon who performs DIEP flap breast reconstruction. This type of surgery spares the "sit up" muscle and has overall optimal outcomes for patients.       Monica, please discuss the safety of the mesh with your surgical team. In regards to the pain you are experiencing and bulge below your incision, again, I would consult with a surgeon over this type of condition.
