Akiko Shikata is the composer of this song and the japanese symbols in the title are the name of the song. I can't read Japanese so unfortunately I can't translate it to you. :')

It's from a 2006 visual novel called Hanakisou (Funeral of Flowers) for PC, PS2 and PSP. It was never released in English. AFAIK, there isn't a fan translation either. The manga based on the VN was scanlated, though.
