    Sometime ago, I was a pedestrian hit by a car. I had spondylolisthesis of L3 on L4. Instead of referring me to a neurosurgeon, she prescribed me Tylenol #3 for almost a year. I had sciatica, foot drop on the affective side, and was simply labeled drug-seeking. Finally, a resident was with her & teste my reflexes. On my left leg, I had no reflexes & was finally referred to a neurosurgeon who immediately did a PLIF. I had been in agony for almost a year & could not stand upright. The surgery was, of course, very painful, and at the time physicians treated pain, unlike today. I took narcotics for a long time & pretty much quit when the pain subsided. Today, I can walk upright, have a bit of lower back pain & no healthcare provider will treat my pain. it's a shame the medical community refuses to treat pain, but I have no control about that. I've never been addicted & since I had work to do, I used narcotics that didn't affect my cognition. Docs today seem to think all patients in pain are drug addicts. I'm really tired of physicians telling me about the "opioid crisis." That does not concern me. I have spoken with a lot of patients & other healthcare providers and they, too are agonizing all the time due to their prejudices. Yes, I'm angry. That's what pain patients are being punished for, the drug addicts.

      That is true.everone is an addict one dtr screeched at ne when I said from what I c my client isn't an addict as he's very careful about painkillers.ive been sober and clean for 30 years at the time. After 5 years with him I might know if he were an addict. And what if he is.. pain needs treated somehow.not just. Oh, well you r an addict. Pfft

      I agree. Your 100% correct, I also suffer and because other people did wrong " we who really are in need " are treated as if drug addict.. I'm have multiple herniate disk low back. Dr said at first meeting well meds are just a patch.. I had a condition which caused me to be in such pain I threw up--rare disorder.. I was 80 pounds-- I had meds back then as I got better I came off the meds. Not everyone has an addictive personality.. animals are not allowed to suffer. My PCP knows I'm not addict--seen them over 20 yrs.. Pain needs treated somehow.. Prayers to all who suffer..

      yup. some people need medication. that’s what medication is for. the same dynamic is present for anxiety patients. sometimes we need benzodiazepines but these days nobody wants to prescribe them bc the same excess of addicts and drug seeking behavior. but some of us really suffer and need those medications that actually work.

      Yup. At 76 and numerous back issues...stenosis, spondylolethesis, sciatica and osteoarthritis. Have had shots and ablation, which helps. They will never operate because I'm old. This is American medicine.

    I have SPONDYLOLISTHESIS and disc degnerationn from L2 S1 . Im at the point where i can barely walk . 6/20 i am having minimal invasive surgery . I am 68 years old . I only want to get my life back . Bob M .

    Dra. thank you for your valuable information. What happens when I have bot things, spondylolisthesis and facet disease? I cant do forward fold or arch my back, not event twists. Its terrible, sometimes I can't sleep, the pain wouldn't allow me. Can you make a video for these 2 conditions? And or recommend me some new treatments to review? I saw somethings about disc gel and platelet injections.

    I got hit on my back 35 yrs ago. Got L4 spondy. Didnt bother me all the years till now. Driving me crazy. Tried massage, acupuncture, bodywork, chiro, exercises. Ill live with it as it caused a bone spur causing stenosis. Ouch. Im handling the pain had pt, Aleve, cbd. Its not fun but no surgery will not do that.

    Dr Andrea thank you for your videos, I have spd., L5-S1, for years, (12) I lifted a heavy box with a person, we climb 6 stairs and I felt the pain in my lower back, felt all kind of sensations, in my legs, I went to the doctor after a week and explain about my problem, I was in therapy almost 2 years, I felt good and continue with my job routines, lift, drive pull, push materials, I work in building maintenance, and this year I have the pain in my legs again, lower back, special when I stand up, I went for RX, and told me my vertebrae L5 moves, S1, but compare with exams while ago, looks the same, I feel better when I saw your video and explain, about the pain can managed with theraphy, I am 65 years, one doctor told me yes for operation, and another said no, I believe in God also make miracles, thank you for your time, help us a lot 🙏🌹

    I am a personal injury lawyer. The typical injury is an auto accident with a spinal injury. In workers compensation cases, we see heavy lifting injuries. This series of videos would have come in handy 44 years ago for my clients. Now I use the videos for my own orthopedic issues. All of my cases are mass torts in medical device, pharmaceutical, and toxic torts, such as "forever chemicals."
    So I am the patient these days. One workers compensation case involved a lower back njury which occurred in 1981. With an award of future medical care and treatment that goes through a utilization committee, I follow all physical therapy and operations for over 40 years in one client!
    Then I have my own lower back and I am 73 years old. I used to go to the UCLA biomedical library in the 60s to try to get this information. Now here it all is in a state-of-the art and up-to-date format.

    Many years ago, my surgeon said if I can live with the discomfort, to not have the surgery. I have a L5-S1 grade 4. No Pars break. Diagnosed in my 30s. I’m 62 now. I used to get awful spasm attacks. E excises helped eliminate them. I do feel like I have UTIs often but it’s due to compression from the spondy. Certain exercises take care of that.

      This is very positive news. I was diagnosed a year and half ago with grade 2/3 L5-S1. I've had to change the way I relieve stress i.e. no more ultra running let alone running. I've picked up swimming, cycling, pilates. So far I have been able to avoid surgery. I'm glad I did my research and sough multiple expert advice! I was so close to getting surgery until I was recommended to see another neurosurgeon that gave similar advice as yours! He didn't think I need surgery until 10-15 years - if earlier I will know and be banging on the surgeons door!

    Hello doctor. I have both spondylolisthesis and spinal stenosis. My spondy. is at a low grade, 2, but has caused me to lose multiple jobs. The pain is very bad at times. Have had multiple epidurals etc. Nothing helps. I have worked in healthcare as a medical assistant for 30 years. I will turn 52 in November. 4 different neurosurgeons have recommended fusion L3-5 I'm terrible scared. I can no longer workout. I lost over 100 lbs diet and exercise the past few years. This they madey conditions worse. Now I can barely walk. I'm so depressed.

    Thank you for your clear explanations on back pain. I tried the exercise for sciatica, the one that you lie on the floor with your legs on a chair. Unfortunately, it has caused very bad pain down both legs and feet. A clinic doctor ordered lyrica over the phone. Have only been on it 3 days but I wondered what type of doctor that I should see for follow up?

      Please talk to a doctor that knows pain problems. It can be a physiatrist like me. Or a family doctor, a orthopaedic surgeon or a rheumatologist.

      Thank you for replying. In Ontario right now you wait quite a long time to see specialist and I wasn't sure what my next step should be. My family doctor has retired and because of covid it has only been clinic doctors by phone. At 64 I realize that it needs to be addressed.

    I have S.lodthesis gr.1.doagnosed accidentally ,10 yrs ago.Doctors adviced againnst surgery.and only conservative management.I have loss of balance and stiffness in the leg.below the knee.Knee and feet feel so tight and I need support while walking.I haven't got severe or cosistsnt pain in the back or legs.At times if iam careless there may be slight catch esp left hip and plain inthe lower back while sitting straight.Thisbis relieved after hot phomentation and rest
    I don't use painkillers unless it is absolutely necessary. Iam 82 yrs old.havr hypertension.What can u suggest...

    This was very educational, and I am mva patient, with spinal injury 13yrs ago, bulging disk from L4, L5, to S1, I had very bad experience with pain for many years, although in canada to find an appointment of neurologist is very difficult, I appreciate your tips I will look into your exercises, I still have weakness on my legs especially on the right side, I can't lift anything bending is worst position for me, thank you for your info doc 😊

    What is the difference between spondylolisthesis and Posterolisthesis ? Also you mentioned Grade 2 - 5, how much does the vertebrate has to move in Grade 2 to be consider. If someone has 1mm is that Grade 2 ?

    I had surgery for this after years of ignoring the pain. Still have nerve damage 8years later from ignoring the compression for so long-no insurance.
    Much less pain now.
    I wonder if you could talk about occipital neuropathy.

    I had a serious backwards fall 58 years ago in which I did a reverse dislocation of my elbow when I reached back to catch myself. I hurt my back also but it was never investigated because of concern for saving my arm. Any bending now can bring on serious muscle spasms in upper back and I have had surgery for achalasia relief which has not been reliably effective. I have found that chiropractic adjustments relieves the achalasia symptoms suggesting a spinal column connection . I suspect I have a thoracic spondylolysis. Sadly things are degenerating and I am in physiotherapy twice a week along with chiropractic . What is the best way for me to proceed to try to relieve the pain and regain some of the mobility that I am losing? My doctor takes suggestions but doesn't actively investigate so I would like to have some procedures to request without overtaxing the healthcare system chasing down rabbit holes. Can I request a referral with you? ( I live near Toronto)

    Thank you, doctor, for your informative explanation of the different major lumbar problems. I had major back (lumbar) surgery 7 years ago involving a three-level fusion including placement of six large screws and other stabilizing hardware; partial disk removals; and clearing of some stenosis along a nerve route. Now, I have the same problems (this time one level lower) and most likely will need major surgery again. (sigh) I do have pain radiating to my left hip and groin area when I walk. I've had an X-Ray and MRI and I've had two Epidurals in the last six weeks that have not helped the sciatica-type pain. The surgeon suggested the possibility of therapy but I will now ask him exactly which "spondy" do I have and if therapy will really work, or is surgery my only real option to allow me to stand straight and to walk without pain, again... Again, thank you for clarifying the differences that I may have a better understanding about more informed questions I need to ask my doctor.

    Excellent madan. Here we uses to give modalities like diathermt bacj and teach all core exs. Also gives l.s bellt spinal flexion with pelvictration also...problem is education after seen by yours ...exercise and therapy. To all.it has to bring notice to higher officials so complex of treatment is good ...great dr keep up the good work to whole world

      @@DrAndreaFurlan yes madam you are absolutely right ...problem here the routine protocol we are following 1) intermittent pelvic traction + swd + spine flexion exs 2) if its grade 3 or 4 lumbosacral belt 3) grade 5 spondyloptosis for spine stabilization surgery ... but no cognitive therapies ..so i think we need to indulge in biopsychosocial model to educate them from fear and anxiety free of patients rather explaining anatomically ..with scans because that causes 100 percent depression lack of physical activity fear of doing movement ..leads to disability ...needs education in positive way ..am i right ??

    This is very interesting. I first thought I had carpal tunnel but doing a test they found out I have no damaged nerves, I have been in 10 sessions of hand therapy, 10 sessions of physical therapy and still have pain they sent me to have an xray and saw some problems with possibly the c3 c4 and c5 of my spine now I have to get an MRI to see if that is what is indeed what is causing the pain in my fingers. I had scoliosis as a child and I know it was never fully corrected.

    I envy those who have no symptoms. I have DDD with mild to severe faramial stenosis and several levels of mild central canal stenosis in both my cervical and lumbar spine with bone spurs at every level of my cervical spine. I'm in constant pain from multiple areas. I'm going to be 53 in December and I've been dealing with the lumbar for about 14 years and the cervical spine for around 8 years

      I feel ya with the symptoms! I have isthmic spondolisthesis, severe foraminal stenosis, mild to moderate degenerative disc disease, and bilateral pars defect all in my lumbar spine!

    My Pain is in my Lower Back, Hip and down my Left Leg. I have a hard time Walking and Standing. The Pain feels like an Accordian...Compression of the Discs which increasing the Pain.I was Diagnosed with Spondylosis but my MRI showed 3 Buldging Discs due to Degenerative Discs.     Thank you doctor. I have grade 1 isthmic spondylolisthesis which presents as neurogenic claudication; PT and an epidural steroid injection has lead me to being able to walk 2 miles (3.2 km) reasonably comfortably. Your stenosis video was also very informative in that progression of the amount I can walk is a very, very slow progression, on the order of 200-400 meters per month.

    I've been to a PT for 6 weeks basically for L3-L4 and L4-L5 disc bulge and anterolisthesis of L4 on L5. I mainly want to thank you for making this video and others. If possible, may I ask what to to do and/or to avoid for my condition with BOTH disc bulge and anterolisthesis problems? thanks

    Great videos, thank you very much! I have OA in my hip and knee, but I also have L4 vertebra slippage - it looks like Spondylolisthesis but I never actually got diagnosed with it. I have lower back pain with walking (after 10-15 min of walking, I feel like I carry something very heavy) and pain at the back of my leg (just above the knee radiating down on my leg, especially in the morning when I put weight on it).
    I am doing Physiotherapy - IMS and Bioflex Laser Therapy. Could you please talk a bit about the efficiency of these two therapeutic techniques? Thank you!

      Hi Monica. It is really important that you see a physiotherapist. The physiotherapist will examine your specific situation and tell you what you can and cannot do. These videos are for educational purposes only. Now that you are more knowledgeable about your condition and the types of exercises you could be doing, it will be much easier for you to discuss with your physiotherapist. You will understand what they are saying, and why they are recommending one exercise and not the other one. I think that patients who are more knowledgeable about their options are much better to adhere to the treatment recommendations. That is the ultimate goal of these videos, that you can discuss with your health care professional and be a partner in the decisions.

      Hi Reg, it depends on the size of the vertebra. For a small vertebra, 5mm might be a high grade. And for a large vertebra it might be a small grade.
