Remember when the transformation from the serum was impactful and took effort? The main thing I didn't like about Falcon and Winter Solder was how it was essentially instant and cause no physical changes.

His reaction to Steve's distress shows a lot about the Doctor's character. Even though this project could be key to winning the war against the Nazis, as soon as he realizes it's hurting Steve, he immediately decides to scrub the experiment and shut it down, as winning the war is not worth the cost of a young man's wellbing.
At least this was just the easy and comfortable process of becoming Captain America. Can you imagine how it would be if it was the process of becoming an Astartes?
This scene is referenced in Endgame, when Hulk has trouble snapping they try to get the gauntlet off him, but Steve stops them, remembering when he pushed through pain.
