I made a Malkavian NPC who had the delusion that he was not created by another vampire but by God to combat "true" vampires. The scary thing is that the delusion granted him the True Faith merit. Yep, he could brandish a cross and repel other vampires.
Mine was an actor who played a doctor in a 1960's General Hospital type soap opera and post embrace his character took over his personality. He interacted with reality as if it were one long arc on his television show and gained actual medical abilities, fun times

I had a Sabbat Malkavian in 3rd edition who became 3 characters. He was himself and had megalomania in life and developed sanguinary animism after his first feeding. He saw the power his sire had and craved it so much that he diablerized his sire. The strength of his sire's blood was too much for him and that the sanguinary animism overwhelmed him to the point his personality split. He was now himself and his sire. During the campaign, he was in his sire persona and diablerized an Assamite assassin who was after him. Now he was himself, his sire, and this assassin, all in a cat and mouse game between all three. Shortly after that it became too much so I handed that character to the ST to control and rolled a new, less memorable character.
Currently playing as a malkavian named River, who is constantly attuned to the Cobweb, which is cranked to max volume for her. The only way she can silence it is by wearing a puppet on one of her hands at all times. Unfortunately for her, the puppets talk through her with a will of their own, giving voice to her hunger and telling her things *she does not want to know*
Their malkavians: I am a tortured soul, every moment is an experience. My Malkavian: "With all this chaos and destruction I caused, How did we get here? well It all started when the IRS tried to freeze my bank accounts due to all the Eroge game Purchases I made."
As a DM I would play along with it: You and the party encounter a man in a suit and he speak to you: "Sir We would like to confirm this information about some purchases. Im from the IRS"
I had a Malkavian NPC who was a classic doctor attacked by a patient and embraced. However, his bane manifested in the form of dementia. By the time the players interact with him, he is at the state that most mortals with the disease would be vegetative and near death. He didn't even remember being a vampire... as his body was trying to continue to breath and struggling with it since he didn't need air. One of the principal antagonists in the story was his friend and student who he wouldn't embrace as he didn't want to pass on the Malkavian madness to her. So, she tried to delve into the occult (and got deep into it) and eventually ran into and convinced a Tremere to embrace her. Through everything and onto the other side of the Pyramid, she never lost the one guiding light that she wanted to cure him of his Bane. On the flipside, Malkavian is the one clan I will NEVER play. Not because I hate them but I feel they defeat the point of view I come into VtM with: if you are the Emperor you should never be aware you are naked.
I don't play this game or really know much about it, but I have a friend who does. I've known this guy for over a decade and he tells me I'm a perfect fit for a Malkavian. Not sure how to take that.
My Malkavian was Duo, an industrial worker whose starting affliction was a compulsion to bisect things, or at least break things as cleanly as possible. During moments of tension in meetings, he'd pull a pencil from his pocket and snap it just to calm his nerves, would have to fight the urge to cut or break anything that was handed to him to the point he'd almost irreparably damaged a document we were supposed to carry, and once he found an industrial-strength, diamond-edged, hand held cut-off saw? Used it on a baron's prized sports car to bisect the thing when he had nothing else to do. Eventually it evolved into an obsession with duality in general, and he started finding match objects to bisect and rejoin into half-and-half amalgams. Considered doing it on people a few times, but luckily people are rarely properly similar in build to make a valid mirror.
I heard from a friend who played VTM that they once played with a Malkavian who thought they were still human. Yeah, you really have to work to make joke/gimmick characters work. I heard of a guy who had a character who was a Malkavian sired during the Vietnam war. Hilariously enough his specific derangement was Cotard's Delusion. (The belief that you're dead/don't exist).

My most prized tome? You shan't be allowed to read it, but I can tell you of it. It is a book I wrote myself, though the content is not my own. It is "The collected visions of the Fatebreaker". The stories and prophecies of Sorina, a Malkavian seer without equal. Apparently, she carried the voices of all those she had killed in the back of her mind. They spoke secrets and truths to her, words that she did not understand but which perfectly mapped out the shape of the world. She was her own Cassandra, speaking of the future with such clarity that everyone but her could see it. Such a blessing. Such a curse. She asked me to compile this tome for her own sanity's sake, slim though it was. If she could read her own words, see the truth of them in hindsight, maybe she could begin reconciling her own mind with the world. No such luck, sadly, as it brought her only tears for what she could not understand. Still I keep it, for some of the prophecies within have yet to be fulfilled. That is why you cannot read it, Ashspawn. Woe comes to those who know too much, as the Clan of the Moon can attest.
My Malks a Russian detective that thinks he just has memory issues when in fact he has a split personality that’s his sister which he already had before thanks to all the molestation that his father did to him. He went to his sire after being turned because he was looking for his sister who had already been ‘turned’.
Possibly, but his problems aren't with insanity. I happen to have a degree in psychology, and nothing he does makes sense from a point of view of mental illness. He's too competent, for starters.

Are all these Vampire clans in the first game? Or did they come from the battle royale?
They are from the Tabletop RPG they are all based on, though both bloodlines and bloodhunt have been very accurate to lore.
