I've been taking 500-1000 mg of niacin everyday for at least a decade, along with ester C, fish oil, ubiquinol, D3/K2 and magnesium. Despite the fact that cancer, heart disease and diabetes run deep in my family (both parents died at 73 from cancer and heart disease), I am in excellent health with zero signs of any chronic illness. I think niacin has a lot to do with it. I also do vigorous exercise at least 3 days per week, fast on a regular basis and I keep sugar/carbs to a minimum. I believe all of these things done together is the key to living a long healthy life.
@@judycrisenbery2982 The flush is mostly what is GOOD about it. I do nothing other than I bought a very low dose, 50mg. Still get some flush. I can increase from there. If you flush too much with 50, maybe cut it in half or thirds. I would not buy the 500mg they sell- no way. I think 50mg for me is fine- easy does things. It can be hard on liver I think if you go too high. Like most things- easy gets it done more safely.
Hello Dr Brewer, I am 52 years old I had a heart attack about 15 years ago I have three stents I do not take aspirin I do not take any statin drugs, and my cholesterol was extremely high about 2 years ago, as I started watching your channel I start taking niacin I take 2,000 mg in the morning 2,000 mg in the evening and within one year my cholesterol was as good as a teenager, without any change in diet. Thanks to your videos about niacin, so I'm a firm believer niacin works., I'm also a chef and as all cooks say when there's too many cooks in the kitchen, things never come out good, too many different medication too many different drugs will not get you the right result or at least a good result, that's what I think

I have a problem with insulin resistance. I was doing pretty good with diet. My LDL was a little high so I started taking Niacin 500mg. LDL went way down. My glucose levels started going up. I didn't connect the dots until I saw a report that Niacin can cause insulin resistance to increase. I stopped Niacin and now glucose is very good again.
I love the flush!!! I was prescribed niacin for low HDL years ago......it worked!! A couple of years ago I quit because I went from High risk to low risk in regards to my cholesterol because.............I was consuming so many omega 3's...........as well as a healthy low carb diet with a few root beer floats intermittently! So mu,ch for the NIH, FDA, USDA, ADA, AHA, and all the rest of the establishment!!! I think I will resume taking niacin because I miss the flush!!! It made me feel alive and kicking!!!
If your doc said your Cholesterol marker was high risk, I think you need to change doctor.. What matters is your Triglyceride levels, you want these to be somewhat in the low end.
I also use niacin. I'm 71, take about a gram a day and it warms my skin. From what I've read and heard it help. A few years a go there was a doctor who was 95 at the time. I cant remember if he studied niacin or if he took it for a long time. Forgot his name. But that was when I started taking it on and off. I take no drugs and never have. I try to cure the cause instead of taking a drug to comer the cause. I believe exercise helps improve health.
I've been on 1500-2000 MG for at least 3 years. I was getting rashes whenever I went from 1500 to 2000 but I guess ive adjusted. My cardiologist recommended I not take it but it's inexpensive and doesn't appear to be harming me so I'll continue to take it.
Have you tested your homocysteine? According to articles and studies I read on NIH site, Niacin causes hyperhomocysteineaemia. I was taking 1g/day and the last time I tested my homocysteine it was high and it made me very disappointed.
@@jerseyjim9092 mine was 40 when I was taking 1g of Niacin per day. I reduced the dose to 500mg/day and it went down to 14. I was also taking B complex and methylfolate at times since it said it can help lower it quickly. I'm not sure if it did but reducing Niacin dose had a considerable effect on homocysteine.
I am like most people can't Tolerate niacin at the therapeutic doses. I can name many other things like Aged garlic extract that has reduced coronary plaque and heart attack and stroke. It also helps with cancer. Tocotrienols and Gilsodin also have impressive results. I do agree simply eat better is vital. The people living in Italy, and some other countries, have shown us how to eat, and how to live for longevity.
I use it for Triglycerides and LP (a) but after watching Dr. Brad Stanfields video on the proof how it increases NAD+ levels and research papers, I am convinced to not drop it.
Been taking atleast 500 mg niacin (full flush) and 500 vitamin C for more than 10 years. 66 years old at this time, credit niacin for ideal bloodwork, blood pressure, receding gumline reversed and other benefits. I avoid taking niacin together with other meds as a matter of caution.

@dereksdiatribe I went to my primary care physician in October 2022 because I have Hashimotos and hypothyroidism. She told me to find another doctor because I still refused to take statins. She also did not send an Rx for my thyroid. I do not have a primary care physician now.
Note the new mega interest over NMN, and or NR. Niacin increases NAD+, and they have yet to do a niacin vs NMN or NR (both metabolites of niacin...), likley because niacin is FAR less $. Much of the benefits from niacin likley its impact on NAD+.
Current NICE guidelines in the UK explicitly say NOT to offer Niacin B3 to patients with low HDL. Primary health care Doctors (GPs) are being instructed by the NHS “best practice” guidelines. What is more interesting no supporting evidence is provided with this apart from the standard “no good evidence” However those same practitioners remain keen to prescribe statins without testing lipid levels. The NICE guidelines instructions are to lower “X LDL” by 40%. No optimum numbers are provided or supporting references. Statin doses should be increased until frank signs of damage become apparent or that “40% reduction” has been achieved.
Niacin was one of the first treatments prescribed to lower LDL or “bad” cholesterol. However, eventually niacin showed to be less effective than other cholesterol-lowering drugs and was associated with other negative effects and higher mortality rates in previous research.

I just got it. Staring off at only 50 seems comfortable to me. I might increase over time. I can still feel some flush but it is not bothersome. If THEY tell you not to take it, it is often good for you.

I live on the road and A LOT of places do not even test for the advanced lipid panel with the particulate count. Very frustrating, I had to demand the GP give me a referral for the test and I’m still in the process of finding a place and scheduling it.
Harvard study was refined carbs, not whole grains and complex carbs. Please clarify that in your video. It was seen at 7:10 on your video "a diet high in refined carbohydrates (white bread, sugars, etc).....That is not the Harvard School of Public Health Healthy Eating food plate of 2011. Always avoid refined carbs, as in bagels, donut, cereals. Ezekiel bread and whole wheat pasta, bring it on.

I don’t take statins anymore, my HDL got up to 77 12 months ago. But for some reason my labs I just had my HDL fell to 55. In addition I thought Niacin was bad for your liver so I stopped taking it 2 years ago
@@boomerang0101 any supplement that is described as slow, sustained, extended, delayed or whatever other synonym are slowing the absorption of that nutrient which lengthens the time that the liver is exposed to such a high dose. That is what creates the potential for issues. Look up Dr. Andrew Saul for a good source of info for niacin supplementation.
Thanks doctor. I am 55, on a statin and BP medication (lisinopryl and amlodipine). Obese. My blood pressure is well under control. Just stated taking "real" niacin that causes flushing. I kinda like the feeling. However, I am wondering if I should lower the statin? I feel great, sleep great, but too many carbs ("American diet" as my physician calls it). Started lifting again, steam room, lots of water and sunshine. Trying to lose belly/butt fat.
Are we talking process carbs or any carbs? Because it’s my understanding that whole plant carbs are not harmful. I eat lots. Tons of energy, kept 65 pounds off for seven years now and exercise like a Tyrannosaurus rex.

If i hadn't done that I would have been conviced by the meta analysis you find when you google niacin research and not taken it. But I knew better and was shocked by the "research". I asked my doc about niacin when he was recording "the patient refused medical intervention for high cholesterol ". He said we no longer recommend it. No straight answer as to why.😊
My Doctor basically said the same thing. He told me that all the exercise and natural supplements would not do anything for my heart disease. High cholesterol, and other high lipid tests. He said he would not be monitoring me because I refused ”Western medicine”

Thank you. I been taking niacin over a decade. I take it for Lpa, cholesterol. I buy it in bulk because the prepackaged niacin has too much filler. Taking it for so long don't really notice flush anymore. It really works. But should monitor ast/alt and that can be done by going to Quest lab. Don't need a prescription to self monitor. 🙏🤗
@@marcrabbat yes. I noticed that after starting treatment my Quest Diagnostic test showed 18! Previously it was significantly lower. Now within past year it went lower to @ 12 or 14. My triglyceride level is too high 1133 😢 ordered vitamin b3 500mg, as usual as I used it last time I became red spots on my skin as I googled which normal, cinnamon helps me a lot.
I have been taking Niacin for 2 years and it works for my HDL/LDL/TRIG. My doctor put me on statins, that didn't work to lower my levels and I heard about Niacin, so I gave it a try and am so glad I did. I have a question. I love grapefruit and am wondering is it ok to eat them while taking Niacin?
depecheddurand I’ve done that before as well and still get an awful flush. Even 30mgs makes me feel like I’m about to set ablaze and have a heart attack.
Take an asprin or ibuprofen about 20 minutes prior to taking the niacin. The flush will be very reduced. Then you can lower the dose of the NSAID and slowly increase the niacin dose. I'm speaking from my own experiences. Check with doctor first.

I took 500mg niacin B3 and gave it to my husband. Oh LORD!! I thought we need to go to ER… It was like sunburn on steroids! It was even hard to breathe… Plus dizziness…After doing quick research and realizing what’s happening, we just waited for about 2 hours and it gradually went away. I’m afraid to take it again. What’s your suggestion? Take 100mg to start with and gradually increase or what? Thanks
Be careful with that, it is a different formulation. I was educated about that at the vitamin dept at a small local organic grocer. I totally trust those vitamin dudes.

Thanks. The standard dose of niacin is 2 grams. But when you use CV inflammation testing, you get a different picture. Also, until you fully understand your carb metabolism through OGTT, Kraft & CGM - you really don’t understand your risk factors. .

Tom dayspring on Peter attia’s podcast mentioned that sure niacin improves all those markers but at the end of the day it does not “work” because it actually worsens all cause mortality slightly. So his conclusion is that it must be doing something “bad” that we don’t understand. So it works for the markers but isn’t working….
Will or can Niacin raise your blood sugar? My not so scientific test “seems” like my fasting blood sugar is up slightly on my insulin resistance self when taking Niacin. I “was” pre-diabetic before a low carb diet and losing 30 lbs (20 more lbs to go) and my fasting blood sugar unfortunately still hovers around 95-100 (which I am working on with fasting/intermittent fasting currently) so when my last fasting sugar was solidly past 100 with the “only” known change in diet was Niacin made me ask the question? Yes. This effect tends to decrease over a few months. For those who take statins, they can as well. But with statins, it’s permanent. Pitavastatin is the exception.
I took 2 g per day for 5+ years and recently stopped due to causing LPB and insulin resistance. I don't know if I resume and stay at lower dosage, there would be any benefits or not. Not much research info since there is NO profitable market for it.

My doctor took me off niacin because of the AIm-High report. I would like to go back on niacin ,what should I say to my doctor or should I just take OTC niacin???

Please do a web search on this paper, "New Perspectives on the Use of Niacin in the Treatment of Lipid Disorders" published in 2014. In the paper, Figure 1 has the dosing info. Table 2 has the recommendations on how to reduce flushing.
The Aim High trial was a bust study. Niaspan (b3)was one of the best products on the market . Post AH trial, many cards and endos walked away from it. What an absolute sad story.

High dose niacin is hard on the liver. It also robs the body of methyl groups. I won't use amounts that are far out of line with the RDA I have very low Lp(a) without niacin.

Took niacin, don't smoke, don't drink, was on a low carb diet, went to the gym fairly regularly. One day, I went to the gym, pushed the sled across the floor about 6 times, had a heart attack. I know that my inflammation is high, but I can't get the doctors to treat that, they just want to push the statins. I had a bad reaction to statins, injuries weren't healing, aches pains, brain fog, depression... Now they have me on ezetimibe, I guess we'll see how this goes. I need to know what the protocol is. Is it a gram twice a day, or once, or can I split a gram between morning and night?
weird, do you take k2 to get the calcium out of your bloodvessels and into your bones? NMN or NR can be a better alternative to Niacin, though more expensive, they are much more effective

@@nextime3927 Yes, 1500mg with no issues. As I recall from another video, Dr Brewer indicated any increase in blood glucose is temporary.

@@marcrabbat I had it checked in Aug of 2019 and it was 9 which they said was optimal. But at that time I was only taking 500mg of Niacin.
@@jerseyjim9092 mine was 40 when I was taking 1g of Niacin per day. I reduced the dose to 500mg/day and it went down to 14. I was also taking B complex and methylfolate at times since it said it can help lower it quickly. I'm not sure if it did but reducing Niacin dose had a considerable effect on homocysteine.
I am like most people can't Tolerate niacin at the therapeutic doses. I can name many other things like Aged garlic extract that has reduced coronary plaque and heart attack and stroke. It also helps with cancer. Tocotrienols and Gilsodin also have impressive results. I do agree simply eat better is vital. The people living in Italy, and some other countries, have shown us how to eat, and how to live for longevity.
I use it for Triglycerides and LP (a) but after watching Dr. Brad Stanfields video on the proof how it increases NAD+ levels and research papers, I am convinced to not drop it.
Can I ask how much you use and the impact it had on lp (a) ?
On a later video, Dr. Stanfield recommends taking TMG to fix the methylation issues caused by niacin.

Been taking atleast 500 mg niacin (full flush) and 500 vitamin C for more than 10 years. 66 years old at this time, credit niacin for ideal bloodwork, blood pressure, receding gumline reversed and other benefits. I avoid taking niacin together with other meds as a matter of caution.

@dereksdiatribe I went to my primary care physician in October 2022 because I have Hashimotos and hypothyroidism. She told me to find another doctor because I still refused to take statins. She also did not send an Rx for my thyroid. I do not have a primary care physician now.

Note the new mega interest over NMN, and or NR. Niacin increases NAD+, and they have yet to do a niacin vs NMN or NR (both metabolites of niacin...), likley because niacin is FAR less $. Much of the benefits from niacin likley its impact on NAD+.

Current NICE guidelines in the UK explicitly say NOT to offer Niacin B3 to patients with low HDL. Primary health care Doctors (GPs) are being instructed by the NHS “best practice” guidelines. What is more interesting no supporting evidence is provided with this apart from the standard “no good evidence” However those same practitioners remain keen to prescribe statins without testing lipid levels. The NICE guidelines instructions are to lower “X LDL” by 40%. No optimum numbers are provided or supporting references. Statin doses should be increased until frank signs of damage become apparent or that “40% reduction” has been achieved.

Niacin was one of the first treatments prescribed to lower LDL or “bad” cholesterol. However, eventually niacin showed to be less effective than other cholesterol-lowering drugs and was associated with other negative effects and higher mortality rates in previous research.
Taking Niacin for skin conditions I look forward too skin flushing to transport cinnamon plant oils to into my skin low dose works great dose is 200 mg daily. It also keeps blood levels perfect.
I just got it. Staring off at only 50 seems comfortable to me. I might increase over time. I can still feel some flush but it is not bothersome. If THEY tell you not to take it, it is often good for you.

Ron Krauss MD states “ the perfect patient for niacin: a person with high lp (a) and high small ldl particle counts.”
Those are 2 very common indications for it.
I live on the road and A LOT of places do not even test for the advanced lipid panel with the particulate count. Very frustrating, I had to demand the GP give me a referral for the test and I’m still in the process of finding a place and scheduling it.

Harvard study was refined carbs, not whole grains and complex carbs. Please clarify that in your video. It was seen at 7:10 on your video "a diet high in refined carbohydrates (white bread, sugars, etc).....That is not the Harvard School of Public Health Healthy Eating food plate of 2011. Always avoid refined carbs, as in bagels, donut, cereals. Ezekiel bread and whole wheat pasta, bring it on.
Dr . Kahn, isn’t the issue any processed food in general? The standard American diet should be put on trial for murder! There ain’t nothing healthy about consuming dessert all day! You can’t separate refined carbs from whole grains and complex carbs in today’s world. It’s all adulterated and detrimental to our health . We are told to follow the food pyramid and we’ve blindly done it. Now heart disease, hypertension,diabetes & obesity plaque or nation. We blame it on salt. I don’t take statins anymore, my HDL got up to 77 12 months ago. But for some reason my labs I just had my HDL fell to 55. In addition I thought Niacin was bad for your liver so I stopped taking it 2 years ago
Only the "slow" or "sustained" release types are bad for the liver. Niacin in the form of nicotinic acid is just fine.

@@boomerang0101 any supplement that is described as slow, sustained, extended, delayed or whatever other synonym are slowing the absorption of that nutrient which lengthens the time that the liver is exposed to such a high dose. That is what creates the potential for issues. Look up Dr. Andrew Saul for a good source of info for niacin supplementation.

Thanks doctor. I am 55, on a statin and BP medication (lisinopryl and amlodipine). Obese. My blood pressure is well under control. Just stated taking "real" niacin that causes flushing. I kinda like the feeling. However, I am wondering if I should lower the statin? I feel great, sleep great, but too many carbs ("American diet" as my physician calls it). Started lifting again, steam room, lots of water and sunshine. Trying to lose belly/butt fat.

Are we talking process carbs or any carbs? Because it’s my understanding that whole plant carbs are not harmful. I eat lots. Tons of energy, kept 65 pounds off for seven years now and exercise like a Tyrannosaurus rex.

If i hadn't done that I would have been conviced by the meta analysis you find when you google niacin research and not taken it. But I knew better and was shocked by the "research". I asked my doc about niacin when he was recording "the patient refused medical intervention for high cholesterol ". He said we no longer recommend it. No straight answer as to why.😊
My Doctor basically said the same thing. He told me that all the exercise and natural supplements would not do anything for my heart disease. High cholesterol, and other high lipid tests. He said he would not be monitoring me because I refused ”Western medicine”

Thank you. I been taking niacin over a decade. I take it for Lpa, cholesterol. I buy it in bulk because the prepackaged niacin has too much filler. Taking it for so long don't really notice flush anymore. It really works. But should monitor ast/alt and that can be done by going to Quest lab. Don't need a prescription to self monitor. 🙏🤗

@@marcrabbat yes. I noticed that after starting treatment my Quest Diagnostic test showed 18! Previously it was significantly lower. Now within past year it went lower to @ 12 or 14. I would make appointment but my new car has not arrived. I been waiting for my car approximately 4months. Have not had problem with homocystine when taking niacin.

I have been taking Niacin for 2 years and it works for my HDL/LDL/TRIG. My doctor put me on statins, that didn't work to lower my levels and I heard about Niacin, so I gave it a try and am so glad I did. I have a question. I love grapefruit and am wondering is it ok to eat them while taking Niacin?

take after eating full diet not on empty stomach i went through hell and next day i ate it after lunch and i was fine.
depecheddurand I’ve done that before as well and still get an awful flush. Even 30mgs makes me feel like I’m about to set ablaze and have a heart attack.
Take an asprin or ibuprofen about 20 minutes prior to taking the niacin. The flush will be very reduced. Then you can lower the dose of the NSAID and slowly increase the niacin dose. I'm speaking from my own experiences. Check with doctor first.

I took 500mg niacin B3 and gave it to my husband. Oh LORD!! I thought we need to go to ER… It was like sunburn on steroids! It was even hard to breathe… Plus dizziness…After doing quick research and realizing what’s happening, we just waited for about 2 hours and it gradually went away.

Thanks. Take much lower doses as trials. Also take slow-down versions, such as Rugby or Enduracin. Most of our patients tolerate Enduracin better.
It's all about the money. I purchased no flush Niacin. Another example is hydroxychloriquine vs remdesiver. Cheap vs extremely expensive.
Be careful with that, it is a different formulation. I was educated about that at the vitamin dept at a small local organic grocer. I totally trust those vitamin dudes.

Thanks. The standard dose of niacin is 2 grams. But when you use CV inflammation testing, you get a different picture. Also, until you fully understand your carb metabolism through OGTT, Kraft & CGM - you really don’t understand your risk factors. .

Tom dayspring on Peter attia’s podcast mentioned that sure niacin improves all those markers but at the end of the day it does not “work” because it actually worsens all cause mortality slightly. So his conclusion is that it must be doing something “bad” that we don’t understand. So it works for the markers but isn’t working….

Will or can Niacin raise your blood sugar? My not so scientific test “seems” like my fasting blood sugar is up slightly on my insulin resistance self when taking Niacin. I “was” pre-diabetic before a low carb diet and losing 30 lbs (20 more lbs to go) and my fasting blood sugar unfortunately still hovers around 95-100 (which I am working on with fasting/intermittent fasting currently) so when my last fasting sugar was solidly past 100 with the “only” known change in diet was Niacin made me ask the question? It definitely can. It does raise mine.
Yes. This effect tends to decrease over a few months. For those who take statins, they can as well. But with statins, it’s permanent. Pitavastatin is the exception.
I took 2 g per day for 5+ years and recently stopped due to causing LPB and insulin resistance. I don't know if I resume and stay at lower dosage, there would be any benefits or not. Not much research info since there is NO profitable market for it.

No. Niacin only in the form of nicotinic acid can lower ldl, triglycerides.
Please do a web search on this paper, "New Perspectives on the Use of Niacin in the Treatment of Lipid Disorders" published in 2014.
In the paper, Figure 1 has the dosing info. Table 2 has the recommendations on how to reduce flushing.
All friends of mine who smocks a lot never had high cholesterol and who never smocked have high cholesterol.Maybe you should think more.

I eat a lot of vegetables so I'm not worried about Niacin. And getting one gram out of my half pound bag is not easy.
High dose niacin is hard on the liver. It also robs the body of methyl groups. I won't use amounts that are far out of line with the RDA I have very low Lp(a) without niacin.
LP(a) is primarily genetically driven...

Most docs that use niacin think it has to be 2 grams/day. But I have seen improvements with lower doses. The trick is measuring the impact.

Took niacin, don't smoke, don't drink, was on a low carb diet, went to the gym fairly regularly. One day, I went to the gym, pushed the sled across the floor about 6 times, had a heart attack. I know that my inflammation is high, but I can't get the doctors to treat that, they just want to push the statins. I had a bad reaction to statins, injuries weren't healing, aches pains, brain fog, depression... Now they have me on ezetimibe, I guess we'll see how this goes. I need to know what the protocol is. Is it a gram twice a day, or once, or can I split a gram between morning and night?
