My understanding is AFTER you worked row 3, and your on the wrong side of the work, which would make it row 4. One of the reasons markers are important, to remember which is the right side, in a reversible pattern. 😊

The only ways I know to prevent an ugly side are knitting in the round and double knitting, but they both make it twice as thick.

I would love to do this as a cowl. Is there a difference working in the round others than eliminating the two selvedge stitches?

Yeap, no problem at all. Before starting the project, knit a swatch first and see if you like it!
How to knit this in reverse direction?
