    ABOUT ME:
I'm Monica, an Internal Medicine attending at University of California, Los Angeles. I'm here to provide high-yield videos for medical students and residents. My favorite topics include how to succeed on clinical rotations, clinical skills, and some good old pathophysiology! I'm also obsessed with my cats and fitness, but I think there are enough cat and fitness videos out there...

Hey Monica, could you give an example of an admission note/H and P for students to get an idea of how it's written? Like a document?Thank you!

Great video. How many hours do physicians spend with paper work per day? Do you do this notes in the end of your day or while talk to pacients? Is faster to dictate? Do you spend more time with pacients or doing paperwork? Thanks

From my experience, you have missed something that Doctors learn. Leave a good space at the bottom of the note, so if you mess up, you can retrospectively add further information that covers up your mistake. In the UK, lying and covering up is positively tolerated and has become an acceptable practice. Probably the same with all Doctors, happy for others to update on this subject.
