    This is the story of Tam Airlines flight 3054, on the 17th of july 2007 A tam airlines a320 was flying from port alegre to são paulo's congonhas airport in brazil. The plane departed port alegre at 5:19 pm local time. As the plane flew towards congonhas the weather was very bad, a sign of things to come. The a320 that they were flying was limited in a way, the plane did not have an operational thrust reverser on the number two engine or the right hand side. Thrust reversers are used to redirect air in the opposite direction of travel so as to help with braking.
    As they got closer to the they got reports from other pilots who had landed on runway 35L, the runway was wet and slippery, theyd have to be extra careful on this approach as they had an inactive reverser. The plane flew the approach perfectly, as the plane flared over the runway the pilots brought the throttle back, the plane touched down at 140 knots. But soon they realised that something wasnt right. The plane wasnt slowing down as they had expected and the spoilers hadnt deployed. Making matters worse the autobrake system hadnt engaged as well. The nose gear touched down and the plane showed no signs of slowing down, the plane ate through the limited amount of runway that congonhas had.
    Six seconds after touchdown the pilots realized that they had to do something and they engaged the brakes with the pedals. They were also fighting the plane, the plane wanted to veer right but the pilots fought it with rudder inputs. But it was too little too late, runway 35L was just too short for the plane to stop, the plane veered to the left and exited the airport premises. The airport was raised by a bit and so the momentum of the plane carried it over a major road near the airport. The plane impacted a building nearby. No one onboard made it and 12 people on the ground did not make it.
    Finding out what happened to flight 3054 would be hard, congonhas was one of brazil's busiest airports and if something was wrong they needed to know. Talking to pilots they found something strange. They were apprehensive about landing at congonhas, concerned in way. The airport was surrounded by buildings and was densely populated. Flying in and out of congonhas, especially under rainy conditions was very challenging for pilots.
    Why was this? In 2005 they found that the runways had low friction coefficients, that is the runways were putting up enough resistance to slow landing planes down, in addition to that water pooled on the runway making landings very slippery. So they set out to retexture the entire runway and they decided to remove rubber from the runway at increased intervals, all of this was done to increase the friction coefficient of the runway. But water logging problem had not been solved and so aquaplaning reports kept coming in when the runway was wet. At the beginning of 2007 the runways underwent some more modifications to fix the gradient and to work out surface irregularities. As part of this overhaul they decided to put grooves into the runway, the idea was that the grooves would channel the water away and prevent water from pooling on the runways. This would solve the aquaplaning problem that they had been facing.
    But the grooving would take time and so the airport management decided to put the runways into service without the groovings for the time being. Quote “In the end, the operational conditions of Congonhas airport brought the pilots a feeling of unease, according to what was learned from the interviews. The reason for the discomfort was precisely the lack of options in case of emergencies. According to the interviewees, the airport offered little or no margin for errors or failures. End quote.

    My wife was coming from Rio to CGH & her flight actually landed preceding this crash. She was still in the plane when this one crashed. She did not hear the explosion, but people on window seats on the other side saw the flash.She was also in an Airbus 320. The crew that day consisted of two full captains don't remember why, each with 1000's hours experience in this type of aircraft.. Ironically, the plane flew over one of the busiest streets in São Paulo at rush hour under a very strong storm, flooding had been reported in that area of town, it barely missed a busy gas station on the other side of the road, and plowed into the TAM airlines cargo building through the front desk & stopped short of the back wall. The building became an inferno with people coming in for the night shift. I remember the governor talking on TV and announcing that no survivors were expected since the temperature inside the building was reported by the FD was expected to be over 1000 degrees Centigrade.

    Damn, every time I remember this flight I get a goosebump. I'm from Brazil and can confirm that before this crash, there had been another incidents there...
    It's kinda sad that we need crashes to understand how to improve.

      I don't think we necessarily need crashes to understand how to improve. I think we understand that well enough with close calls and pilot complaints, easily enough. The problem is that we just DON'T SET PRIORITY to those improvements until there's a crash to bring public outcries, threats of boycott, and so forth as the ultimate incentive to BOTHER with the improvements... If no crashes happened, the improvements would still mosey along, just as "proprietary improvements" on later models as the airline companies (and governments eventually) continued ordering newer, nicer, easier, and safer planes...
      Other industries continue to improve steadily without endangering lives. It's just another marketing ploy. People get readily dazzled into "the new fancy version" that's 10X more expensive than last year's or last decade's model that was PERFECTLY good, and even still works fine for the job... The newer nicer stuff still sells...
      What's frustrating is that nobody sets a priority for improved readability or specifically mixing visual AND audible alarms or error reporting JUST because a few pilots complain about close calls or point out that they've also had to double or triple check things and complained about the added work-load or stress... ONLY AFTER a mass casualty event or two or even three, is there a new regulation instated or a mandatory upgrade published for those improvements... AND the complaints from the big companies are always the same, "It's expensive!" ;o)

    My father was in Porto Alegre working and would take this flight to meet the rest of the family in São Paulo so we could go on a vacation trip. His work dragged on more than expected and because of that he called and told us to go because he most likely wouldn't be able to catch the plane, and that he would catch up to us later.
    We went to travel, my grandmother used to live very close to the airport, so we passed on the road where the plane went off a couple hours before the accident. During the trip my dad called again to reassure us that he didn't catch the flight. The signal on the road was quite poor so we left it at that.
    When we arrived we saw on the tv what had happened, and thank god he called because just the thought that he was supposed to be in that flight made me sick. I can't imagine how it is like for people who lost their loved ones in tragedies like this.

    My wife landed at CGH that same night. This TAM plane was the next to land after my wife's flight landed. Her plane was still taxiing to the gate. She was sitting on the left side of the plane & didn't see anything. Passengers on the right side saw the plane landing & a flash coming from the end of the runway. The plane crashed into the cargo building of their company 2 buildings away from a Shell gas station. The plane crashed through the front door killing the people in the front counter & customers, and came to a stop before going out the warehouse back wall. The building caught fire & collapsed. Roads were flooded due to an unusually strong storm. Fire trucks took a while to get there because of stopped traffic & severe flooding around that area. It is one of the most dangerous airport in the world. I have landed there many times. There are 10 - 15 stories building within 1000 feet on both sides of the runway. You can actually see people inside the flats. I recall once seeing a topless woman suntanning on her balcony! The end of the runway has a huge checkerboard pattern red & white about 50 meters high extending the entire width oh the runway! The airport was built in the 1920's on top of a small hill, and grew with time. There is a 200+ feet drop from the end of the runway to the street below. It is one of the busiest streets in the city with 5 lane street going each way.very few traffic lights!
    This flight pilots that day were two captains who totaled almost 20,000 hours between the two of them. Bad judgement, or & much confidence on their skills probably played a part in the decision to land at CGH. CGH Is like La Guardia in NY. It's very close to downtown &near the business district. GRU, the international airport is about 35 miles away & has 12,000 feet runways. They should have diverted there, which many flights did that day. There is a direct bus service between the two airports every 20minutes!

    I lived 5 minutes from there,my brother had just driven past the building the plane crashed into,a friend was in one car and saw the plane crossing the avenue,and another friend witnessed from her bedroom window while i was talking to her(she started yelling about an explosion) i heard the sirens from my house,everyday on my way to school i used to walk by the hotel where the victims families were....It was extremely sad to see their pain....this accident affected me a lot, i always avoid flying to/from Congonhas ( even though its closer for me) because of those memories always come to surface when i have to go there,and when i drive past the memorial i always avoid looking at it, this kind of tragedy its hard to forget...

    I often did POA-GRU during that year but a week before this accident I flew POA-CGH instead. A friend was flying with me and as we approached Congonhas for landing, he told me about how short the runway was, the lack of grooves, and how risky it was when it was raining. So everybody knew about the risk at the time and did nothing.

    This is really random but does anyone remember when Google Maps would allow you to rate a runway? I remember reading the ones at my home airport (Lambert KSTL) while planespotting in 2017 and you actually got some good info about them from actual pilots.

      well, sadly, he didnt choose a braking method in the end . at touch down, he put one engine in reverse but forgot to /didnt move the other one to idle. we cant tell without listening to the CVR exactly how the pilots were preparing during those testing minutes, and without checking TAM training for opposite thrust setting = spoiler deactivation... and this video did not indicate if the pilot with headache was landing the plane... so many factors. sad, indeed...

      if he had done it right it was goin to stop earlier and ON the runway..not outside of it...sadly we all are human and we all make mistakes...i cant understand why the FO didnt check the throttles and brakes again...we all look onto the captain but the FO is there too...where was his mind at that time?

      @@rezzy851 : Good question. I think one factor is that it was was nighttime and it was dark in the cockpit, so the misplacement of the right throttle wasn't readily visible.

    10:00 This is literally the first thing that came to my mind when I learned the engines generated thrust in opposite directions. I can't imagine any non-disaster scenario where setting thrust in such way would be beneficial. I was surprised that there was no warning whatsoever. It's a real shame that this got overlooked in original design and only added later as a non-mandatory modification.

    The simulation of the approach has the most realistic lightning I've ever seen in a simulation. Amazing realism. This was one of the crashes from the television Air Disasters (or Mayday) series that really stuck with me. The pilot was so focused on the poor conditions of the runway that he forgot to fly the plane. Pure tragedy. The crash site was like being in Hell.

    I remember clearly this accident. Now I live about 2 kilometres from Congonhas Airport and every single plane that is going to land (we get a whole lot of them everyday) I feel apprehensive for those onboard wishing this tragedy never repeats.

      Why is there a building asking to get trampled by an approaching airplane standing right dead centre to the runway!!???.. makes no sense.... The airport should be closed and relocated or something must be done.. take advice from japanese technology.. machines under roads that sucks water into a tank during rains ... GENIUS!!!!!

If you search for Congonhas Airport in Google Earth or similar you will see what I mean. Thank you for your comment.

      @@anonymus4803 What I meant by that is sometimes or often, depending on how you look, money is more important than common sense, like avoiding construction around an airport. When Congonhas started this area was empty, you had to cross farms to get there but the city grew around it. Most people here follow the rules and abide by the law, maybe I am a little pessimistic regarding my country because I would like to see her better than it has been so far.     No matter how unlikely an event may be, remember: it may happen nonetheless. When training at Iberia, an instructor pilot told us trainees of two equal incidents, ony one month apart: telecompass failure. He then explained us what telecompasses are and their very simplicity.
    Telecompasses are not mechanical or electronic but simply electromagnetic. Each telecompass system involves a couple of electrically linked units, with no power system applied. Each unit has a 3-pointed star-shaped magnetic material (such as iron) with electric coils. One unit stays at a wingtip, the other next to the actual compass in the cockpit. Just so that the compass gets the Earth's magnetic field as far from interfering things in the plane as possible.
    Well, the odds of telecompass failure is thirteen million to one. So, when one failed, it was deemed just bad luck and substituted. But, next month's failure seemed too unlikely, and the manufacturer was called and an investigation was run. Result: again, just bad luck. Astronomical odds? Yes. But it happened.

    I met several relatives of the deceased and one of mine had actually scheduled to be a passenger in the cabin as pilot in training. Honestly, the company gave them psychological help for a couple of years then it was sold and they were forgotten. The impact on their lives was deep, one very young colleague from school lost his father. His mother took it so badly that she prohibited anyone from mentioning the death, so the kid couldn't speak with people and would start crying if anyone brought it up, which was a fairly normal conversation topic around town.

    I had two classmates on that flight. They were there because another classmate didn't agree to sign a petition to change the date of a final exam which the teacher agreed to. But it was necessary that each student signed it.

      @@NiHaoMike64 Actually, no, not really. we were med students then, (yup, I am a MD), she was just that much selfish. After graduation, I shared ER responsabilities with her a few times, her presence upsets my stomach to this day.

      What are the 5? I’m gonna try to guess:
      1. Tenerife
      2. JAL123 the blown off vertical stabilizer that still flew for 30 minutes which no simulator pilot could achieve
      3. The Gimli Glider
      4. The Netherlands crash in a forest / field where all survived.
      5. United XXX(I forget) the one out of NY that exploded due to hot AC units after 2h of stuck on the runway, then a low center fuel tank vapors being ignited due to old faulty wiring short circuiting causing such a violent explosion that the cockpit severed from the fuselage, killing everyone. Many missile conspiracy theories.

      @@MovieMakingMan honestly it shocked me too. where i live, the airport is far away from most of civilisation. the worst thing that could happen is that it could crash into airport buildings or other planes. i feel like airports should only be built in good locations with empty flat land

    Very nice video. Loved the graphics and explanation. Sounds like both of them were mostly heads up and looking out the window, but it was like the perfect storm. Seems like most crashes or mishaps happen after a few or several different minor things happen at the same time, stacking up on each other. 7 seconds isnt a long time to figure things out and fix it.

    Tl;dr - video very good, viewer much enjoy.
    I appreciate the factual emphasis i.e. stating fluidly and clearly what occured and why, without changing the focus to questions of which person/group is accountable for such a disturbing tragedy. It makes the viewer curious and compels them to read/research more on their own.
    Air crashes are the result of such complex, confounding and ,so often, a desperate chain of errors.
    Writing a clear, objective narrative of events from that pool of dense information; into a format that can be easily absorbed by even an adolescent... That is just admirable documentary making, man.
    The recreations are a great addition. Almost professional, one should say. Choosing a cockpit view is a smart move. Same with showing the throttle settings in real time.
    Anyways, this comment is now some more baby food for the algorithm.
    May this channel take off soon.

    As others have mentioned, the current graphics are very nice and much better than the previous generic pictures, but I think it could be beneficial to show some pictures/footage of the actual place. It just feels wrong and like a terrible idea to have an airport where Congonhas is. The actual runway ends in a small "cliff" so even without any obstacles, there is no place for the planes to go in case of running over the runway.

    Thanks for explaining why the old method even existed. I would have thought it was a shoe-in to always just put them both into reverse. One would think that it would be engineered so that the deactivated reverser simply wouldn't allow the engine to thrust up at all. Seems not to be the case.

      Probably because the difference is negligible. 180 feet on a 6200 foot runway. (55 meters on a 1900 meter runway.)
      Either method (pulling back both thrust levers all the way, or pulling back the "good" lever and leaving the defective one at idle) would've been fine. But instead the anxious pilot left the right one at full (or nearly full) power. As another commenter noted, it's ironic that the very fear that the pilot had about being able to stop in time ultimately caused him to fail to stop in time.

    Beyond pilot error (they failed to comply with the procedures for that defect) the plane was due to be fixed in São Paulo. However, corporate greed mandated that the airplane should not fly to SP empty (what a waste of money, right ?......).
    Many lives lost stupidly.
    Had the weather been different, the sum of all errors should not be this.
    Wonder how many times i (we) did fly under iminent danger.
    A stewardess, a person close to me, was the only whose body was never found amidst the wreckage.
    God keep their souls.

    Text, ahhhhhh yeah, that's like that lil block anybody can click to clarify what a narrative's voice may lack for a multitude of reasons. Nobody can be 100% at all times. It can be used if you are multitasking and don't wish to clutter an on going conversation in the same space/room your not directly involved in also. There are others that ail of adequate hearing as TH-cam itself has added text options to most all channels abilities. Yet, thinking about it this channel's narration hasn't failed yet. Your proactive response and acuity to clientele is if professional quality (Legacy TH-camr & Media would do well to take notice). Clear, well projected, not over amplified (BOOMING) multi tracks, etc. Great work. 👍

    A client of my accounting firm lost his wife in this accident, she was a passanger. Up to this day people from sao paulo associate congonhas with "the plane that didnt stop" and see that airport as "dangerous"

      Not on the MEL. Stopping performance during certification is done with no reverses, hence they aren't on the MEL. But on wet surfaces obviously they're the most beneficial as if the anti-skid is fluttering the main gear brakes and the performance is reduced, at least the reverses are doing something constructive during loss of friction moments.

      I would rather know if the plane I was a passenger on was compromised in its ability to stop. Redundancy is, after all, there for added safety and not to allow them to fly with known defects. If I took my car out with defective brake pads on just one wheel because there were three others with good pads I would be a twat that deserved to be crucified. 😂. Sounds like there’s an argument for the minimum equipment list to be reviewed each preflight in light of the destinations and conditions to be encountered. Asymmetric reverse thrust must rely heavily on tyre adhesion to counter the lateral forces and reverse thrust is most useful when tyre adhesion is most compromised. I am however not a pilot but just an engineer so I am imagining the effects of asymmetric reverse thrust.

    They knew they would need every possible meter of the runway, yet they didn't decide to go around after realizing it was all going wrong. It seems like going around would have saved everyone in many accidents

      @@jamesdean2185 Well diverting to another airport is a big deal. The airplane and passengers would not be where they are supposed to be, logistical nightmare.
      The pilots actions in that situation would be seriously questioned.
      It boils down to if the pilots operate the airplane as it should be operated in that situation, then this would have never happened.
      By leaving the right engine throttle above the idle detent, they caused two other systems to not operate.
      Auto spoilers did not move to the "Ground Spoiler" position, which in turn canceled out the auto braking system, delaying braking to when the pilots manually applied the brakes.

    On a dry runway you will get away with using the wheel brakes and flares to stop but on a water-logged runway the best thing to use is reverse thrusters to avoid aquaplaning. Who would have thought that an airline would operate their planes even though they were not mechanically in sound condition. Therefor lack of maintenance put aircrew and passengers in danger.

      By then it would be too late. It would take at least another 6 seconds to stow reversers and increase to TOGA and then get back up to take off speed. By which time you’d probably be near the end of the runway

      @@measl that’s even worse. If they think the reverses aren’t out then go to TOGA it would take longer than expected. The rule is once you are on the ground with reversers deployed you cannot try and go around because it would take too long to reset the aircraft in a safe condition to fly. That’s why when you do touch and go landings you don’t deploy reversers

    Being a computer scientist, I don't see why any airline should be charging extra for notifying you about speed differences between engines. The critical and safety parts of an airline such as this should always be included and airlines should be barred from making profits by providing them as optional features.
