The "Special" reward is given to the player, if they go to particular places on the route. You will hear "Good Job!" If done correctly. First case, go through the arch sign at the 180 Turn area. You should see another ambulance go into the water in the same area. Second Case, go VERY Wide on your turn where the Statue is at. You should go near the far wall. The Gang Member's Van will be on your Left or should be. Third Case, go Through the Gas Station that is on fire. Go between the Pumps and the outer pillar. The turn is HARD after that. Good luck. Fourth Case, After the large bridge jump, go through the left area that's on fire. 7:05 does show this Special. Other rewards: Stable. Have the patient status as "Condition Stable" when you arrive. Note that any damage done, will cause the status to change. The status will return to "Condition Stable" over time if no further damage is taken over a period of time. No Damage. Take NO Damage. Brave. Probably a particular route or driving style? Secret. Unknown. Another comment mentions arriving with the timer at Zero. The Cars are NOT RNG Based. They will show up and do the exact same thing, every time, if you do the same thing, every time. Manual Transmission is required for Case 4. It's nearly impossible otherwise. And Good Luck getting through Case 3 without Damage. I hate that one. Failing any Case, will show you a progress Map of how far you got and the Case's layout. Beating Case 4, will change the Name Entry Screen.

When I walked from bowling club I couldn't control myself, looked at the sky and screamed like Miguel Caballero Rojo in Tekken 6 intro as if holding that Police officer in my hands.
I'm surprised at how good it looks despite being many years old, this game certainly deserves a remastering (with a few more missions) current graphics. At the end of the day, when the message appears that thanks you for playing, it is a detail that does not happen in today's video games.
