    Thank you Dr. Jamie for starting this series. I'm 55 years old, so those demographics really spoke to me. I've been on the fence about this ever since I heard of it a few years ago, but there hasn't been much information. I did consult with a surgeon, but I had to lose weight to get my BMI down. Anyway, I'm eagerly awaiting part 2.
    LOVE your channel, thanks for doing these videos.

      You're welcome! I am happy you find this information helpful. The study outcomes are very similar despite being separated by a number of years. As a researcher and a clinician this is great because it shows stability and consistency of results over time.

    8:18 I had a radical cysecprostatectomy with a urostomy (infiltrating bladder cancer). The urologist also recommended the vulvoplasty as any complication with a vaginoplasty would be too hard to fix.
    There is a specific aspect for trans women having a history of radical prostatectomy or radical cysecprostatectomy. One of the most common side effects of this surgery is a definitive erectile dysfunction. Most of us, if not all, chose the vulvoplasty to fix this side effect.
    Having a definitive erectile dysfunction is really a game changer both on physiology and on psychology.

    One tip that I can give from my own experience is that I did laser hair removal first, which took about a year going once a month. A local salon was very reasonably priced and did a great job! Fortunately, I have Kaiser which covers electrolysis and only costs me $5/session. At first the sessions were 60 mins. After 3 sessions, I was down to 30 min sessions and again after 3 sessions, I was going every 2-3 weeks. I think I did about 10 sessions total and was cleared by my PA. Overall, it took about 14 months but I was able to complete it with minimal pain.I thought that watching TH-cam or listening to my favorite music would help, but actually talking to the Electrologist really helped cope with the pain. In my experience, Ibuprofen/Naproxen and Lidocaine tropical cream didn't make a difference. My first few sessions were insanely painful but after noticing the pain and sensitivity in that area after 3-4 weeks, I knew the intensity was set too high. Once they turned it down, the pain became much more tolerable and I didn't feel any pain after 10-15 mins.

      Thanks for these pro tips. Kaiser is great. I am with you on the lidocaine, ibuprofen/tylenol. It’s basically useless. I also like talking to my electrologist during sessions. Happy you’re done with your hair removal. Such a major transition accomplishment on the path to bottom surgery. 🩵🤍🩷🤍🩵☺️

    Incredible content again thanks. As much as I want full fepth a zero depth maybe more viable just because of my size/BMI. Thanks for this info. It is helping.

    I will add my thanks to the list, I'm 66 in long term committed relationship, and we are no longer active. So I feel a full depth is not a sensible option. My present genitalia is something, I need gone.
    After listening to your once again stellar presentation, I know what I will asking for when I see my Dr this month.
    As well I know my BMI requires some work, thankyuo again for helping understand what I need to do.

    Thank you for this content! I'm 80, I don't want full depth, nor do I feel I would want that much drama. I would be happy, I think, to just be rid of what's there, i.e. no more T and clothes would fit. I'm anxious for 2&3. It's as if you could hear this subject rattling around in my brain! ❤😂

      I'm 74, and not in a relationship. At this point, I'm not sure full depth would be worth the added recovery time.

    Two friends who are in the last stage before surgery have decided full or shallow. Both have valid sound reasons for going forward. I went to both of their appointments and did I get an education on the surgical procedures and processes.

      @@DrJamieTalks that is the plan right now. Another third friend has not indicated what she will do for her surgery but I have my guesses. lol
      If I were ever able to take hormones myself and have surgery I would have the full surgery yet present as androgynous like I feel.
      Cancer treatments and intersex development made living as a male unacceptable for me. From my earliest experience I found loving as a woman easier than a male. My spouse of 50 years has always loved me this way. Well maybe I should ask her first before I assume this. lol
      If you find away of making things work don’t fight it just exploit your love. ❤️
      I did pass on information about your you tube channel to friends and hopefully they will be dropping in for a look see very soon.

    Thank you for the information that confirms my choice of Vulvoplasty surgery, my age, my health, no dilation, and no interest in vaginall sex. Because so few surgeris are performed, there is little to zero information on Post Op results compared to Vaginoplasty. Can you help? ❤

      The documents available on GRS are detailed in the procedure and complications. Learning from a patients first-hand experience is invaluable and would help others prepare mentally and physically for their surgery.

    I find it I suppose you could call it in an odd way comforting (or maybe reassuring is better) that of the cases of the surgeon recommending vulvoplasty, 10 were due to surgical risk and one due to, probably, questions about post-op care. Not one raised any question about the validity of the patient's preference. Considering how recently it was hard to find a surgeon who even took the whole idea seriously, I choose to take that as a sign of social progress.

      Most surgeons I know who work in this space are pretty open to their embodiment goals. That is definitely a sign of progress. I meet surgeons at WPATH conferences who just love to work with trans people. It really warms my heart. 🥰

      @@Chloedawnknauer Gender affirming surgeons see the joy gender transformation has on their patients. Not all surgical procedures leads to this level of joy in patients.

    I notice that according to the table the recommendation for the patient with schizoaffective disorder to undergo vulvoplasty was made by the surgeon. I sincerely hope that this was a decision made in consultation with the treating psychiatrist, the patient and possibly even their support system. Unfortunately as people with severe mental health issues have dramatically worse health outcomes it's hard to be confident of this.

      @@anneharrison1849 Yes, it is impossible to know. Typically surgeons will consult with the mental health team if there are any MH concerns.

    ❤❤❤❤ may think on this- is interesting, if nothing else @ 70.Am overwhelming happy to have my Stage 1 GRS scheduled & happening before Xmas. Small steps..get through this first.Do wish to have FFS- but that's to repair broken nose, from car accident- 7 years ago.Major dysphoria, that matters not..too the people, who really love me- before & after transition xxxReikki Kyra

      @@DrJamieTalks to get to here, electrolysis & lazer..over two years- huge hole in Age Pension( about 65%/ fortnight) @ thousands of bottles/ cans collected from put out 'red & yellow bins on street- over 5.5 years for redemption @ 10c a pop.The laser was $3500AUS, & the electrolysis approximately $30,000 AUS currency .By time, have paid for the basic Stage 1 GRS ( lose two troublesome hitchikers) will have spent $46,000 Australian currency- none of which- is covered by medical insurance. Only $500..is covered by Medicare.Can add another $2000-$3000 to that for GP, Pathology & specialists ( Phycologist, Endocrinologist & assessment Phychirist) over past 3 years.Transition- ain't cheap..in Australia.

      @@DrJamieTalks are no figures available for vulvaplasty in Australia, which more than likely means- it is not performed here.Vaginaplastly is- done here..are only 3 surgeons,& I got the best of all three..loves a challenge. Though, this time around- it's not. Am just testing ' the water'.Cost for vaginaplasty is minimum of $30,000, more if revision is required.Do have some other good news.Saw my endoncronologist- 30 minutes ago.My kidney/ liver function test- perfect result. Testosterone- is barely measurable & estrogen- back up to 450( was 70..6 months ago & my blood pressure, kidney & liver function ' all over the shop'.Joy of extreme adrenal insensitive syndrome/ being born intersex)..E- level now back to where was after first year of HRT. So happy 😊- do know how this sort of happened. T- blocker dosage was halved ( to protect my liver/ kidneys..obviously- that did not work: did get genuine "sorry" @ previous appointment) & brand of dermal estrogen patches, was using- became unavailable for 5 months. Just a simple thing, turned transition- to nightmare.Did figure this out- for myself.For 4 months, both endoncronologist & my GP- gave up/ stopped listening. Never knowingly, had an intersex patient before- in my age range( have highest rate of suicide of any group , in Australia. Wonder why??Is no brainer..nothing is textbook)Even, the young ones 15- 25 years struggle with access & support.Anyway, all good.Sun shines- on my day & new beginnings xxxReikki Kyra

    I'm wondering how much worse is the recovery from full depth? I'm considering both, but full depth is mostly on the off-chance that I might be interested in specific sexual activity later on

      The recovery is big either way. The risk of damaging structures like the colon or bladder are low with the zero depth procedure. Some complications such as fistula would also be very low if nonexistent.
      For me, I chose the full depth in part for the reason you describe. Being mostly asexual and aromantic my interest in relations has been low at best. As I’ve transitioned, I have experienced some ‘what if’ sparks of sexual interest that I decided not to ignore.

      It shouldn't matter whether you are able to retract your foreskin, but I recommend speaking with a surgeon about it. :-)
