After a bat bite on my left foot, I was given a rabies immunoglobulin shot on day 0 in my left foot very close to my bite wound. The rest of that immunoglobulin shot was given in my right arm. I also received 1 rabies vaccine shot in my right arm. Then, on day 3, I received a rabies vaccine in my right arm. On day 7, I received a rabies vaccine in my right arm. On day 14, I received my last rabies vaccine shot in my right arm. The brand of vaccine was "Bavarian Nordic Rabavert."

Rabies vaccine should not be given in gluteal region its efficacy will be compromised as chance of it becoming subcutaneous is high. If the cat is still healthy don't worry... You can check antibody titer and decide if further vaccination needed..

For healthy patients any prophylaxis definitely builds up memory that is why for re exposure no rig and only 2 dose given In immunocompromised we cannot guarantee adequate memory cells as it will depend on degree of immunosuppression.in them also preep is definitely recommended but for all re exposures ideally both rig and vaccine needs to be given... .different guidelines do give slightly different opinion on this...

Depends if it was late for 2 nd doses then you are at slightly high risk rember the after 3 rd dose vaccine delay is not a big problem but if the delay was long and before 2 or 3rd dose you should consult a doctor

Hi. My puppy(unvaccinated) bite my hand while playing and the next morning my arm got Aching. I had tetanus shot. After i went to doctor he said it's no need for rabies vaccine. After three days I got high fever/headache and pain in my arm. So I went and got anti rabies vaccine (3 days after bite) . It happened in 2016. The puppy grew, gave birth to puppies and it was alive and healthy for like a year. But I still get pain in the arm (where I got Bitten) along with fever and headache every month for days. It's been 7 now years now

But the HCP at the nearby clinic gave the shots on the guteal region. Should he restart the whole schedule and take it on deltoid? Our pet is fine and is not displaying any abnormal behaviour.
Taking rabies vaccine in gluteal region is considered invalid...but if it has already been 10 days since bite and pet healthy ..don't worry ...but would definitely advice everyone to take a pre exposure prophylaxis

Vaccine dose for primary and booster is same..only schedule is different .here you have repeated the whole schedule so don't worry you should have good antibody titers by now
Madam please update knowledge. Antibodies do works on nerve via CD4 T cell. Peripheral nerve and CNS has different barrier do not mix it . Abtibodies do cross blood nerve barrier . Untill virus reaches brain antobodies can clear the virus from system
If it was really so no vaccination failure would happen after day 10 of primary vaccination...as cd4 cells appear as early as day 3 and b cells by day 10 after rabies immunisation
We have to follow what scientist are saying by doing their test . Vaccination failure can have multiple reason . Every human has different immunity response . There are cases where vaccination given to few people on same day bitten by same dog but few people survived and few not so it is very difficult to judge rabies as we do not know where the virus actually lying in body. Hence CDC and WHO says itsnever too late to be vaccination untill virus reaches to brain so we should follow what science says . Here question is whether antibodies reaches to nerve or not and result shows yes it reaches . It is quite possible it will be reaching late till than virus must reached to brain

No think you got it wrong ...virus can remain at the site of inoculation for months to years without reaching neuromuscular junction that is why it is recommended to give vaccine and immunoglobulin depending on original class however late patient comes.

Yes here your first dose is invalid... But since you have completed the rest doses it should provide protection... WHO and CDC recommend only total four doses...so your schedule must be enough.. But Indian guidelines continue to recommend five doses so may take a repeat fifth dose after two weeks.

If you have previously received a pre or post exposure rabies vaccination you do not need rabies immunoglobulin. If you have previously not received vaccine before bite and your current wound is class 3 and it's already day 4 since 1st vaccine you still should get immunoglobulin. Immunoglobulin when indicated even though preferable to day on day zero can be given upto day 7 of starting vaccination
No rabies infected animals will never come near you and most importantly they most likely won't sneeze but the saliva may transfer rabies is went directly in wound but rabies dies immediately after it dries and is out of animal body usually takes 5 or 10 minutes to dry

If rabid dogs saliva falls on open wound it can cause disease... Touching or exposure at intact skin does not cause rabies. If you are living in a locality with high risk of exposure it would be better to take pre exposure prophylaxis

Post exposure prophylaxis is a highly effective method of preventing Rabies... If you have taken all precautions including timely wound care and vaccination.... It should help.

There are no guidelines recommending complete vaccination after a specified interval...as usually anamnestic response is good....22 year interval studies not there so can opt to complete series...or check antibody level and decide

Yes for immunocompromised check titers two weeks after last dose. If less than 0.5 take 1 dose and repeat titer... Or repeat whole schedule and recheck titer... If responding after one extra dose repeating whole schedule not needed. Cdc and who guidelines slightly different on this one recommends 1 dose and other repeat series before checking titers

The respected madam, while giving rabies vaccine on day 0,day 3 and day 7 intradermally in an immunocompromised patient , a small amount of rabies vaccine comes out along the surface of the needle while withdrawing from both sites on day 0 , day 3 and day 7 intradermally but there were distinguished blebs ( the size of blebs were 6-8 mm everytime )on both sites on day 0 , day 3 and day 7 intradermally in the immunocompromised patient .Does a small amount of rabies vaccine which come out along the surface of the needle while withdrawing on day 0,day 3 and day 7 intradermally from both sites with distinguished blebs ( the size of blebs were 6-8 mm everytime )on both sites on day 0,day 3 and day 7 intradermally affect overall immune response of rabies vaccination schedule in the immunocompromised patient or is it normal that while giving antirabies vaccine intradermally , a small amount of rabies vaccine oozes out along the surface of needle while withdrawing or is it true that small amount which comes out along the needle while withdrawing intradermally doesnot affect overall immune response in the rabies vaccination schedule in the immunocompromised patient ? The patient is left to take dose on day 28 and one extra dose on day 42 .The immunoglobulin was given on day 0. Depends if it was late for 2 nd doses then you are at slightly high risk rember the after 3 rd dose vaccine delay is not a big problem but if the delay was long and before 2 or 3rd dose you should consult a doctor

Hi mam i have ment with category 3 bite on my right hand plam it was bleeding for 1 hour i went to near by govt hospital were after 1 hour delay i washed my wond with soap and water then they given me tetanus shot and 1 st shot of ribes vaccine on both side of arms of hand. immunoglobulin was not available so after 24 hours i went to near by private hospital and took immunoglobulin on the site of wond on 3 rd day vaccination was given on gutial region remaining vaccine on 7 th ,14th and 28 th vaccine was given on right and left arm of hands i am so tensed after this bite its more then 40 days now i have also did on test named rabies RNA detection test also non as rt pcr test they collected my blood sample for this test i got report negative yet i am depressed about it when i should feel my self safe after how many days mam

How late can we take pep?. If a child or adult recalls history of class 3 exposure can we offer post exposure prophylaxis.? As you said virus can remain in the wound indefinitely and multiply.

According to WHO guidelines you are protected.... but Indian guidelines still mandate day 28 dose

Pre exposure before bite is 0,7,28 ...so you need to take one more dose atleast 2 weeks after last dose

Hi. My puppy(unvaccinated) bite my hand while playing and the next morning my arm got Aching. I had tetanus shot. After i went to doctor he said it's no need for rabies vaccine. After three days I got high fever/headache and pain in my arm. So I went and got anti rabies vaccine (3 days after bite) . It happened in 2016. The puppy grew, gave birth to puppies and it was alive and healthy for like a year. But I still get pain in the arm (where I got Bitten) along with fever and headache every month for days. It's been 7 now years now
Don't worry...it is not rabies related...a rabies virus positive dog is unlikely to survive beyond 14day after a bite

If vaccines taken in intradermal route amount of subcutaneous fat one has does not lead to increased risk.

Hello Mam! Excellent talk. My dad got bitten my our own pet dog which is vaccinated, but we still went ahead and got post exposure shots. But the HCP at the nearby clinic gave the shots on the guteal region. Should he restart the whole schedule and take it on deltoid? Our pet is fine and is not displaying any abnormal behaviour.
Taking rabies vaccine in gluteal region is considered invalid...but if it has already been 10 days since bite and pet healthy ..don't worry ...but would definitely advice everyone to take a pre exposure prophylaxis

Vaccine dose for primary and booster is same..only schedule is different .here you have repeated the whole schedule so don't worry you should have good antibody titers by now
Madam please update knowledge. Antibodies do works on nerve via CD4 T cell. Peripheral nerve and CNS has different barrier do not mix it . Abtibodies do cross blood nerve barrier . Untill virus reaches brain antobodies can clear the virus from system
If it was really so no vaccination failure would happen after day 10 of primary vaccination...as cd4 cells appear as early as day 3 and b cells by day 10 after rabies immunisation
We have to follow what scientist are saying by doing their test . Vaccination failure can have multiple reason . Every human has different immunity response . There are cases where vaccination given to few people on same day bitten by same dog but few people survived and few not so it is very difficult to judge rabies as we do not know where the virus actually lying in body. Hence CDC and WHO says itsnever too late to be vaccination untill virus reaches to brain so we should follow what science says . Here question is whether antibodies reaches to nerve or not and result shows yes it reaches . It is quite possible it will be reaching late till than virus must reached to brain
We all know antibodies are nothing but white cells which are body produced to kill virus which are dangerous for our body so how they can leave rabies virus in nerve . Nervous system also comes under our body only the only difference is that this defence system works differently on nerve . I will suggest you once read the yele University scientist report

No think you got it wrong ...virus can remain at the site of inoculation for months to years without reaching neuromuscular junction that is why it is recommended to give vaccine and immunoglobulin depending on original class however late patient comes.

Yes here your first dose is invalid... But since you have completed the rest doses it should provide protection... WHO and CDC recommend only total four doses...so your schedule must be enough..
But Indian guidelines continue to recommend five doses so may take a repeat fifth dose after two weeks.

If you have had a vaccine before the bite then you do not need immunoglobulins. But if you haven't had a vaccine before the bite, it is compulsory.
If you have previously received a pre or post exposure rabies vaccination you do not need rabies immunoglobulin.
If you have previously not received vaccine before bite and your current wound is class 3 and it's already day 4 since 1st vaccine you still should get immunoglobulin.

If rabid dogs saliva falls on open wound it can cause disease... Touching or exposure at intact skin does not cause rabies. If you are living in a locality with high risk of exposure it would be better to take pre exposure prophylaxis

There are no guidelines recommending complete vaccination after a specified interval...as usually anamnestic response is good....22 year interval studies not there so can opt to complete series...or check antibody level and decide

Yes for immunocompromised check titers two weeks after last dose. If less than 0.5 take 1 dose and repeat titer... Or repeat whole schedule and recheck titer... If responding after one extra dose repeating whole schedule not needed. Cdc and who guidelines slightly different on this one recommends 1 dose and other repeat series before checking titers
