"To" can actually go to the first "that" at the beginning of the sentence, such as "to that is the page we are referring." It sounds strange, but is correct and you don't need to change the second "that" to "which", only omit the 2nd "that."
OK, how about, "Where are you going to?" In that sentence the "to" would go with the verb "going to," but would be clearly wrong because it is needless. One should just say, "Where are you going?"
I guess I am kind of formal. I am very careful with the way I talk. I'm going to ask, "To whoom are you talking?" To me it just sounds better. I am more likely to say, "I'm throwing away the wood up of which I toar." Iwon't say, I am throwing away the wood I toar up." It just doesn't sound as good.
Even though this is the proper way, you will definitely sound weird for everyday conversations.. Nowadays, it's too common to put prepositions at the end of some sentences.
Don't forget that all languages evolve over time, that way of speaking may sound just weird, outright obsolete or even pretentious to an extent to the people around you.
