  • Hello Friends! This video is all about Imaging - and what imaging is appropriate for accurate diagnosis in TMJ Disorders. I see a lot of questions on Facebook support groups about this from people who are suffering from headaches, jaw pain, neck pain, ear pain, popping or clicking in the joints, and/or a locked jaw! These comments and questions inspired me to make this video as a way to get more information out there. I hope you enjoy it!

    Have watched most of your videos and been helped tremendously.
    Question: my TMD issue is primarily pain in upper teeth to ear when I chew soft food. Any suggestions?
    Additional info: 49 year old male developed pain free jaw clicking about a year ago til this pain manifested. Dentist ruled out tooth issue and sinusitis after a week of antibiotics. Thanks

      Yes please watch my video “stuffy ears, vertigo and ear pain explained”. There is a muscle called the lateral pterygoid muscle that can refer pain to specific teeth and into the ear. We calm this muscle down with orthotics and myofascial release.

    Hi doctor since the last 13 years I sometimes get twitching on the bottom of my lip and sometimes the bottom chin. And one time I used a new pillow a memory foam and it's set off that muscle spasms in the bottom of the chin and lip for a day or two. Do you think this can be related to the TMJ? What do you think this might be trigeminal nerve?

    Hello!
    I got three questions if you don’t mind:
    1. Would it be able to diagnose if my “sharpened” joints are related to ICR or TMD or normal wear and tear?
    2. Which of the two would be better in diagnosing ICR?
    3. If both are needed for ICR and only one could be afforded because it would paid out of pocket, which is generally the cheaper one?

    Hi Dr. , i got my cbct done and the report says:
    Right tmj:
    There is flattening of the superior surface of the right condylar head. In the closed position, right condylar head is located relatively well centrally in the glenoid fossa and translates to a point inferior to the summit of the articular disc.
    Left tmj:
    There is flattening of the left condlyar head. In the closed poition, the left condylar head is located relatively well centrally in the glenoid fossa and translates for point posterior to the summit of the articular emminence at maximal opening.
    I have jaw pain and crepitus. I know it might be hard for you to say anything but does it look too bad and can this be managed with a stabilization splint at all? 😢

    How likely is it that osteoarthritic changes were not detectable or foreseeable from a CBCT ~9 months prior to detected arthritic changes, if the patient presented with limited range of motion and pain eating some food?

    I'm to get a cbct next week, how do I see if they look for this as y'all do? Can I send you a copy and you see if you can help me? Is there a specific MRI machine or kind that helps? Like fiesta MRI is best for TN. With contrast or can it be without is ok?

    Hi doctor! I recently went to an ENT specialist because of all the dizzines, pain behind the ear, tinnitus, etc. that I've been experiencing and he ordered an MRI of the brain, since everything appeared normal in my ear. I told him my dentist thought it was a TMJ problem and he thought I was crazy. Anyway, my question is, will an MRI of the brain with and without contrast show if there is a problem in the TMJ?

    Hi doc, follow up question. Do you still use the traditional CT scan in Dentistry?
    One OMFS asked me for a facial bone ct scan instead of a cbct scan which made me think is one better than the other?
    In what instance is traditional CT still used/preferred?

    Hi, when would instances in getting both be helpful? I will be seeing an oral surgeon soon for chronic tmj/tmd issues, really hope they can do the imaging needed to aid in diagnosis n treatment.

      a CBCT is always recommended. An MRI can be helpful if we are trying to determine if the disc inside the joint is displaced.

    Hi Priya.
    I have spoken to you before about my severe TMJ, brain fog, vertigo, dizziness, vision and hearing issues, ringing ears, jaw pain, neck pain.
    I have spent the past 21days in neurological ward in hospital as I'm struggling to walk due to dizziness so bad and all other symptoms the drs wont believe me when I say it's from my TMJ.......I know that's where it's from as talking, eating, swallowing and brushing teeth an effort.
    They are trying to say I have functional neurological disorder because all tests are clear.
    I still cant find anyone to do splint so is there another way of treating it without splint....I cant find dentist do splint in OZ.....what splint would help me so I can ask dentist try make up.
    Do you think it's still TMJ or this functional neurological disorder I don't think it is.

    Hi mam, good video.. I have recently done a CT scan for PNS, would that help in determining TMJ disorder?

      The CT scans will show the bony components of the joints, the nasopharyngeal airway, upper cervical vertebrae, and more. It does NOT show the articular disk or the muscles. The CT scan is just one piece of the puzzle in diagnosing TMJD.

    What treatment do you recommend the most when it comes to MRI diagnosed as DDWR (Displaced Disc with Reduction)?

      We treat this with orthotics, myofascial release and osteopathic manual jaw manipulation. We do weekly and biweekly sessions with this protocol and typically see recapture of the disc within 4-6 months.

I’ve been doing TMJ physio and Myofacial release for over 2 years with no disc improvement just more clicking and pain on the right side of my head but less jaw pain. As well as joint noises/ear popping, really not sure what other treatment can help my symptoms

      @@rachelsanelli3482 we make day and night orthotics with the plan to wean off the daytime within 12-16 weeks. These orthotics along with myofascial release and osteopathic manual jaw manipulation done weekly over 4-6 months is what recaptures the discs.

    Can cephalometric analysis for corrective jaw surgeries be performed using MRI?
    I have read on some articles in medical journals that claim so (I'm not a medical student). Please guide.

      I believe CBCT or lateral ceph are best for analysis.

    Normall MRI which is done for brain scans usually Takes images of facial area too, can detect TMJ dysfunction as well ???

      No it has to be an MRI of the Jaw joint in open mouth and closed mouth position :)

    My endodentist say no disc displaced by seeing OPG x ray
    Is he done right ?
    Will in OPG ,u can see disc is it k or not ?

    But when i got T scan for bite the left bite force was weaker than right side .
    Can this cause the popping and tinnitus and ear fullness.

    I am scheduled for cd scan for tmj but i am very very much afraid of the radiation so i am looking for a doctor who will give me mri. My ct appointment is tomorrow but i want to cancel that. What do u think? Can i see the same results on mri instead d scan?

      No they show different things. A CBCT is less than 1% of the radiation of a regular medical CT scan

    How long do you have to keep your mouth open all the way for that portion of the MRI? Worried about it causing too much strain or pain. It took a year for me to get out of pain after a tonsillectomy.

      I think the entire MRI procedure is an hour long. But they do imaging with your mouth closed AND open.

      8 minutes for opening. You have to bite something to keep it opened. Since I have clenching I thtought im going to break the syringe. If you have phobia for. Enclosed area it will add up to your clenching. Last is open jaw I thought im not gonna finish it but thankfully I was able to do it. I have a diagnosis now. It’s worth it plus it has no radiation involved.

    You say MRI as needed, but aren't both necessary for an accurate diagnosis? The only way you're going to see disc displacement is via MRI. Correct?

      THe only way to see the disc is with an MRI, yes. But you can often figure out what’s happening based off of symptoms, muscle and joint palpation, joint noises, and range of motion.

      iCat the same type of imaging but CBCT focuses on what dentists will want to see and evaluate.

    I just went to a TMJ specialist and they only did CBCT, I am confused should they be doing MRI then?

      I only do a CBCT for over 95% of my patients.

      Nothing images dysfunctional muscles unfortunately. The MRI can capture images of the articular disc and the CBCT shows the condition of the bone.

      Costs vary depending on where you get these done. I cannot give you costs for those. I also do not discuss my costs online.

      CBCt and sometimes MrI as needed. But neither of those can image dysfunctional muscles.

      Yes, it needs to be done with the mouth open and with the mouth closed.

      @@theTMJdoc I called a few places in my country that do MRIs, and I asked about the temporomandibular MRI, and they all said they do it with the mouth closed.
      I don't think I'll be able to convince anyone to do mouth open and mouth closed, unless maybe if I do 2 separate MRIs - one with mouth closed and one with mouth opened.
      So unfortunate I live where I live :( :p

They need to do it open and close to see if there an internal derangement, displacement in both sides, if theres an effusion I guess but it needs to see what was going open when your mouth is open. 😞 In which country do you live I guess if you can go out of country and have it check it definitely worth it than suffering

      @@candlelovervlog2388 I'll be going out of country about my TMD problem. Still, whatever happens, it seems like I'm damaged for life now. I don't see any real solutions to this to be honest. Yeah occlusion, yeah orthotic, yeah MRI, yeah supplements, yeah natural adaptation, but, whatever you do you're just rolling the dice.
      Healthcare is so f****ng primitive I can't even believe it. With all the technology humanity has, and there isn't a single health problem that can be fixed for real. Ugh...

    Hi Priya, Im struggling here with the right treatment. My TMD doctor did a CBCT scan and found I had a deviated septum on right nose. They said my body is compensating for this and thats why I got TMD. My bite has started shifting as I keep biting my left cheek when I eat. But I also have clicking in my neck right side. Thats getting worst. I saw a cervical spine doctor and waiting on results. How do I go about knowing what caused all these symptoms, so I can pick the best treatment? How can I answer the question, did the neck cause TMD or the TMD caused neck pain?

    Hi. Is 3 CBCT okay within two months period?

      The radiation involved with CBCT imaging is the same as a two hour flight. If you’re OK with three flights in a two month period, then you should be OK within a three CBCTs month period. 🤍😊
