Hi there, I am 31 years old, G3P1011 I am 18w6d and have been a type 1 diabetic since I was 7. I have had well controlled diabetes for over a year, A1c before conception was 5.3 and at my NOB appt in May it was 4.7. I use an omnipod and dexcom. I take about 21 units basal/day and between 3-8 units bolus depending on carbs and corrections needed on a given day. Since 8/12 I have had 3-4 instances where my insulin pump fails and doesn’t deliver my insulin properly, my blood sugars spike over 200 and stay there despite tons of blouses. All of a sudden a couple days ago my blood sugars have been high, insulin resistant, I’ve been needing twice the bolus a typical meal would require and have already increased my basal dose by 4 units/day. I don’t know what to do cause it was so well controlled and it feels like I just continue to increase my insulin but my blood sugars can’t seem to go below about 140 no matter what.

First, congratulations on the excellent glucose control! I don't know how your first pregnancy went, but what you're describing is not uncommon with type 1 diabetes in pregnancy. By the midtrimester, insulin resistance can be a big challenge. Your basal rate and your boluses may double or even triple before it's all over. Sounds like you need the help of your primary care provider or endocrinologist to talk about diet, activity, and pump management. Good luck!
