  • The Rule of Thomas for Hemoglobin A1c Conversion
    I love medical eponyms! Is it a delusion of grandeur to think that I could have a medical eponym of my own? Perhaps, but I will try regardless - please take my self-aggrandizement cum grano salis. Without further ado, I present:
    "The Rule of Thomas" for quickly calculating average blood glucose levels from Hemoglobin A1C percentages. Use the following formula:
    Average Blood Glucose = 120 mg/dL + 30 (HgbA1C% - 6%)
    Plug in your variable, namely the Hemoglobin A1C Percentage, and solve the equation. You will be left with an average blood glucose.
    For example, your 30 year old patient with type 1 diabetes typically has excellent control of their blood sugar. Because your patient is young and healthy, you encourage them to maintain tight glucose control. You order a blood draw and obtain a hemoglobin A1C level of 6.5%. You would then have to use a table to find what their average blood glucose would be, but if you use the Rule of Thomas, you will be able to calculate the average blood glucose quickly.
    Average Blood Glucose = 120 mg/dL + 30 (6.5% - 6%)
    Average Blood Glucose = 120 mg/dL + 30 (0.5%)
    Average Blood Glucose = 120 mg/dL + 15 mg/dL
    Average Blood Glucose = 135 mg/dL
    As another example, you have a 58 year old male with type 2 diabetes that is newly diagnosed and poorly controlled. His hemoglobin A1C level prior to treatment is 12%. Using the formula:
    Average Blood Glucose = 120 mg/dL + 30 (12% - 6%)
    Average Blood Glucose = 120 mg/dL + 30 (6%)
    Average Blood Glucose = 120 mg/dL + 180 mg/dL
    Average Blood Glucose = 300 mg/dL
    As the medical community moves to patient-centered approaches to managing chronic illness, The Rule of Thomas is an easy way for clinicians and patients to understand and work with Hemoglobin A1c levels and their correlating average blood glucose levels. The Rule of Thomas will help patients and clinicians communicate more easily about hemoglobin A1c and glucose levels.

    Great…For Americans! but we watch your content all over the planet. In UK we are measured in mmol/L and instructed by doctors in those terms…your mg/dl thing means nothing to me!

    Blood glucose levels & A1C levels are just numbers to novice patients. Patients need to know the effects certain levels of blood glucose have on body functions!

    Your equation can be simplified to G = 30(A - 2) where G is blood glucose in mg/dL and A = a1c * 100.

      I know my Avg. I wanted to know my A1C. I rearranged the rule of Thomas - (Avg/30) + 2 = A1C

    28.7×A1C-46.7=much less cumbersome than The Rule Of Thomas. I don't get how RofT is any easier, or quicker...What did I miss? Seriously asking.

    I believe there are easier formulas to use for the conversions. The two ways of measuring the estimated average glucose are related by mg/dl=18Xmmol/l.
    So 108mg/dl=6mmol/l and 144mg/l=8mmol/l.
    The percentage of HbgA1c % is related to HbgA1c by the formula H%=HX0.6+2.
    So 10%=0.6X13.6+2. And your 58 year old with 12% A1c has 12=0.6H+2, i.e. H=16.7 which multiplied by 18 gives 300. Am I wrong?

    Your formula has nice round numbers, but it is not exactly the same. You can rewrite the ADA formula. -46.7+6*28.7=125.5, so the ADA formula can be written as Av. Glucose=125.5+28.7(HbA1C-6).

    I've got a A1C of 5.8 is this good? 55 years old work out every other day lifting weights for 1.50 hrs at a time.

      that's great I'm 6.9 that's not good I'm 222 and 5,6 I look real good ,I really don't look that fat...but still have controlled b/p and eat fairly healthy. I always say to my hubby lets party which means order kickers and garlic bread no pizza from dominos..and movie and we do this 1 time a week maybe 3 times a month ,and I share with my dog.so its not a lot..but still...and we walk 1/2 or more once a day... I eat very good dinner and breakfast no fatty food. Some but not to much and don't buy any sweets well a pieces of cheesecake once a week or more..lol

      Nooooooo!!!!! A1C Should be 5.11 or lower. Your blood sugar is about 120 (too high!) with a 5.8 A1C reading and should be 100 or lower.

      Mean and average are two words for the same thing - the only difference is one is “estimated”

    I was just diagnosed with diabetes at a 6.7% A1C. Is it possible to reverse this number ? And if reversed would that mean I am no longer diabetic ?

      I was pre diabetic as my A1c was between 6 and 6.6 but I started drinking Bitter Melon juice everyday morning about 4 ozs.of 1/2 bitter melon it went down to 5.9 in 3 months .I am going to continue another 6 months I want it to go down below 5.7.

Thank you for your reply I am currently using bitter melon that i buy at my local asian market and I blend it in my nutribullet without the middle stalking. It works so well it has made me feel like a new person. The difference is between night and day. Thank you so much for the update. I have read it may be bad to drink it over a long period of time because it can damage my kidneys but I am being cautious and taking it periodically and drinking lots of water. I have read it's like a natural insulin source and once I stop taking it my suger will go back to the original. So far so good. More people need to learn about this.

      eat low carbs veg.and meat,,eggs,fish,eggs.
      your target should be less than 5% A1C.

    Hello, nice video, one question in reference to the chart comparison are we comparing the fasting average blood glucose or the average after a meal or the overall average glucose?

      Marla - great question! What the Hemoglobin A1c gets at is a person's average plasma glucose levels over the last 90 days. That is, Glycated Hemoglobin is a reflection of how much sugar is in the person's blood over the 90 days prior to the test. Fasting glucose would be lower, and post prandial blood sugar levels would be higher - the Hemoglobin A1c or Glycated Hemoglobin does a good job of getting the average blood sugar level over that 90 day period.
