    If we get a trans patient who was born male but transitioned to female and is on hormones, or vice versa, then how do we treat those patients? In which category do we treat them because to what extent do hormones really affect the overall exercise?

      That's a great question. The ACSM consensus statement linked to below suggests that testosterone exposure increases athletic performance even if it has not always been present. So, a man transitioning to being a woman by blocking testosterone will remain athletically advantaged. Likewise, a woman transitioning to being a man will develop athletic advantages if they take testosterone boosters. In both cases, they would be advantaged over cisgendered women but disadvantaged vs cisgendered men.
