Oi CJchi the people get knocked out because the stands (the ones fighting) are linked to the humans. The stand is stronger and has greater endurance but if it takes huge damage like getting impaled then the same amount of damage will be inflicted on the human using the stand.

Simply put, most Stands are extension's of the character's fighting spirit and soul. So when a Stand is wounded, that same wound is inflicted on the human too.

Only a stand can hurt another stand. When a stand is injured, the user takes damage.
the reason the sword knocked out the guy and the reason they get knocked out without fighting themselves as humans is because, the sword is a stand known as "anubis" which posseses anyone that draws the sword, the humanoid ones are also stands, all damage done to a stand will reflect the user (except for when it doesnt)
