  • We have had it drilled in our heads that Nitroglycerin is dangerous in Inferior MIs because it can cause unwanted hypotension.
    But does the science match up with what we've been told?

    Simple solution. 1. Follow your local protocols. or, 2. contact MCP for direction if any doubt. I've never been denied NTG for any MI w/ CP if the BP was stable.

      Yeah, the blood pressure is the key but then again I don’t get the hype about it cause lower bp is contraindicated anyway’s. But this is of course necessary to be aware of. Same with bradycardia and treated pt’s with pulmonary artery hypertension (on meds) has never been a contraindicated in our protocols which I think is odd, Inferior MI is no longer in our newer protocol, I think all three should be at least in precaution list so people is at least aware of it.

    I guess, the point here is to be cautious in giving Nitro to patients with inferior MIs because you don't know whether that inf MI is caused by a RCA occlusion or a LAD occlusion. Until such time that you have taken a right-sided ECG and confirm that there is no RV involvement.

    I think working in a hospital you would provide nitro but we weary of the consequences

    I understand that giving nitro in an inferior MI can be okay. But of those statistics how many of those inferior MIs had R sided involvement. Of those R sided MIs, what was the percentage of hypotension, and how many R sided received nitro and how did that affect blood pressure.

      Had an inferior wall MI with elevation on v4r earlier this year.. BP was 190/100. Very stable patient overall.

      new studies coming out are heading towards giving nitro in inferior wall stemis because patients who were with held nitro died.

    if there is elevation in v4r we withhold nitro correct? but if no elevation in v4r you can still give nitro if blood pressure is fine. correct?
