      About a year ago I doubled down on Keto / OMAD ... I lost 115 lbs (Going from 350 to 235). My AIC went from 12.7 to 5.8 ... the doctors at Kaiser were stunned that I had thrown away the insulin and metformin which I knew were killing me. I did as much cardio as I could and I've built up my lower body strength enough to jog... but shin splints kept me from keeping up that intensity. I try to do 30 minutes of medium intensity elliptical every day. Now that I've hit 235 , it's like slamming into a wall, and my AIC is creeping up (now 6.9). Even keeping my sugars under 20g a day, and meals to once a day... I've started to gain weight and I still get ridiculously high blood sugar (like 190). I'm not consuming that many calories in that single meal... usually 1500 or less. I'm kind of stumped as to how to push that needle down and get to healthy weight.

    OMAD and Keto has been the magic bullet I've been looking for all my life. Such a shame I've been trashing my body all these S.A.D. years (I'm 64) and yo-yo dieting with no success. Can't see needing more than one meal a day anymore. At 6'-5" and 305 lbs. I began Keto last Sept. I'm now down to 235 and off B.P. meds after nearly 30 years. Want to drop another 10 lbs. or so and know I can stay there for the rest of my life now. Keep up the good fight Sten.

    I have been on OMAD for over two years. It has helped me with not only weight control but my blood work has been in the safe zones. ( triglycerides in particular)
    One thing I strongly want to emphasize is the minimalistic aspects of managing the quality of intake.
    When managing three meals a day it is very difficult to get a good balance in all three of these meals. With one meal I can use the extra time and money to make that meal really count.
    Also with the mindset of realizing that we do not need any where near the frequency of eating or the amount we consume.
    I find it laughable now looking back at all of the conditioning we receive about what is important in our everyday life style.
    All of the lies we bought into, the fear, the paranoia, is just incredible.
    When I explain to people my life style they just shake their head and look at me like I am going to die. Then they tell me how they could never do this because their body type would not allow it. Then when I laugh and tell them I have maintained this for over two years with only favorable outcome they have no argument except this isn’t breakfast the most important meal of the day. I eat in the evening. Then they say that’s the worst thing you could do is go to bed on a full stomach.
    First I say yes breakfast for me is the most important meal ,I break my fast at night and it is my only meal. Then I point out that all animals rest after eating and doing excretion while your body should be in repair cannot happen & having all your blood volume preoccupied with digestion takes the resources otherwise needed for excretion.
    Both these arguments are part of the food industry conspiracy.
    Getting back to management , shopping for three meals, doing dishes and cleaning up. Waisting water and energy (heating water ,heating food)
    Addition waist packaging etc. this in itself has taken great stress and energy out of my lifeI now I can use these resources for other problems.
    When I look around now and listen to people talk about diet & life style I just feel sorry for their misinterpretation of reality.

    I've been doing some form of intermittent fastingh since August 2019. I started with a 48-hour fast, and then when I went back to feeding, I started doing 1-hour OMAD. Frankly, this was by default because after one modest plate of food, I was stuffed! (My previous normal was TWO full helpings). Also, my palate began to change and as I moved into ketosis, my breath changed (one of the temporary signs of ketosis). Now I have settled in to 16:8 eating because it's less restrictive. I gave up artifical sweeteners and all I drink is water, water + ACV, or black coffee. I started this to reverse my metabolic syndrome (diabetes, high cholesterol, high blood pressure). Over the years, due to work stress and insomia, I had packed on 250 lbs onto my 5'6" frame (prior to that I had a muscular 170 lb frame). I went to the doctor a month ago and ALL of my tests were in the normal range (blood pressure, cholesterol numbers, blood sugar). Another measure that was reduced, but still in an abnormal range, was my HA1c because it's an average measure that takes months to normalize. I should point out that I could only be successful at this after leaving my pressure-cooker of a job this past spring. OMAD and intermittent fasting saved my life. Thank you Dr. Ekberg; your videos helped to shepherd me through the process, and I still rely on your expertise.

    I love OMAD...1) Eat once and done 2) only real fasting protocol in my opinion as you actually fast for awhile not eating 23 plus hours a day 3) cheap way to eat also...16/8 allows very little actual fasting time...I started with that...then The Warrior (20/4) and then OMAD...I think the misconception alot of people have on IF is your fasting window starts the second you set the fork down and swallow your last bite. It doesn't...The post-absorptive state lasts until 8 to 12 hours after your last meal, which is when you enter the fasted state. It typically takes 12 hours after your last meal to fully enter the fasted state. When you're in the fasted state your body can burn fat that has been inaccessible during the fed state. So if you do a 16/8 and you may only actually be in a fasted state 4 hours but on OMAD it is at least 12 hours and as much as 16...

    • @drekberg
      @drekberg  5 ปีที่แล้ว +46

      Very good points. That's why everyone should do what you've done. Start 16:8 and then go to 20:4 or OMAD if necessary or if you like to see what happens with longer fasting.

    I started LC and IF in 2013. I dropped from 305 to 275 and stalled. However with the lc my A1C dropped from 12.2 to 6.0+/- and weight stabilized. Still obese however. I tightened up and dropped down to 250+-. Bounced around that weight.
    I am now an over the road driver, home every other day. I decided to do strict keto and either water fast or OMAD while out, refeeding while home. This has worked astonishingly well. In two months I have dropped to 230.
    I have leveled off but am weight stable at 230. I still have 70 lbs to lose but this is not a race. I am extremely insulin resistant from 35 years of obesity.
    I now track blood ketones and glucose. The ketones tell me my fat burning level. Just another valuable tool for watching my body adjust to a healthy way of eating.
    I have done many 36 hour water fasts, the average time for a round trip 1000 mile drive. I often will eat a very specific 300 calorie keto meal I designed at the 24 hour mark (OMAD). I have done a half dozen 48 hour water fasts and two 72 hour water fasts in these last two months. All of this is what drives a pretty serious and steady weight loss.
    OMAD and keto really rocks for finally losing weight. And with a keto diet, the fasting is easy (er).

    • @drekberg
      @drekberg  5 ปีที่แล้ว +8

      Thank you John Colby for sharing your story. That is so awesome. I am so happy you have noticed a positive change. Seems like a great idea to fast on the road, so you don't have to eat a bunch of diner food. Try measuring a fasting insulin and calculate your HOMA-IR. That would tell you more about how the IR is changing. Keep it up!

    • @jwcolby54
      @jwcolby54 5 ปีที่แล้ว +6

      @@drekberg I follow Dr Boz. She uses a ratio of glucose to ketones with the concept that anything under 80 is "fat burning" and under 40 is probably in autophagy. It seems a reasonable concept. It is essentially a proxy for insulin. Low insulin allows high ketones.
      And yes, avoiding truck stop food can only be a positive. But equally critical is the lack of "social eating" making fasting much easier.

    • @drekberg
      @drekberg  5 ปีที่แล้ว +5

      The glucose to ketones sounds interesting. I think its a useful tool, but I've seen people with 100 glucose and 2.5 ketones and fasting insulin of 20 who are not losing a single pound. I'll have to study that some more though.

    • @jwcolby54
      @jwcolby54 5 ปีที่แล้ว +3

      @@drekberg well as we know in the end calories do matter. That is really the point of the fasting side. If sugar is high and ketones low I will not lose weight because insulin will be high trying to drive the sugar down. IF sugar is low and ketones high then that is a good sign that insulin is low which is required to lose weight. However if I am eating a bunch of fat, at a calorie surplus then I may very well still gain weight. If I get into ketosis and I am fasting then I am almost certainly losing weight. Even that is not certain. If I am highly stressed with high cortisol then it might be possible (short term) to not lose weight. That said it is really hard for the body to not lose weight when fasting. The BMR cannot drop to zero this side of death. No calories in with calories out means weight loss. 😁
      The ratio is nothing more than a pair of the more important bio markers, which happen to be opposites of each other. They do tell us a lot about where energy is coming from.
      More importantly they are markers that I can check without labs.

    This is to share my weight loss journey to all mediums that inspired me and hopefully can inspire someone else as well.
    I lost 13 pounds in last 11 months , it sounds slow but I am so glad that I have achieved this through intermittent fasting, 24-48hrs water fasting, LCHF diet and mild exercise. it’s been an amazing journey from 6 meals (3 big and 3 snacks low fat diet) to OMAD (one meal a day). The biggest surprise was that I am losing fat from all parts like arms, belly and hips, but not from my face and bust line which was completely opposite what I experienced 10 years back when I was in UK with LCLF (low carbs low fat) diet and rigorous exercise almost 5-6 hours a week. I enjoyed this transformation as well as but once I was back in India , I couldn’t continue gym and gained back what I lost. Though my lifestyle changed from sugar laden meals to no sugar at all and occasional rice meals as we Indians love eating rice. I was always overweight but i started my weight loss journey in my 30s and now in 40s I was determined to find a sustainable way of losing and maintaining weight. I am glad that my brother introduced me to bulletproof coffee and Keto diet talks on TH-cam. So big thanks to my bro and TH-cam giving us an opportunities to understand the vast and new information on Keto and sustainable weight management options.
    My Tricks that worked on me are -
    0. I plan and cook my food from whole food with saturated fats like mustard oil, coconut oil, clarified butter , butter etc.
    0. I make sure that I move around a bit but depends on the day schedule
    0. If I have socialising events coming up - I fast the previous day and the following day to hit a balance and I just love it.

    Thank you muchly for this important information. I'm 20/4 Keto, but sometimes I am OMAD, it depends what I've eaten the night before for Dinner, certain foods fill me up so much, that I don't even think about food till hubby says, "What's for Dinner,?" The next night. It happens a couple off times a week. My Hubbies been OMAD for 6 months, & he's lost 45 pounds. We're both convinced that giving up sugar was the best start to changing our lifestyle for the better. Have a Happy Weekend, & take care. ❤🙂🐶

    Excellent presentation!
    OMAD keto is a marvelous tool and the Gold standard in so many ways. Your body will LOVE you for it. The only negatives are; 1) the long physical adjustment period getting into a keto state without 'wobbles' and being comfortable, 2) the social pressure to EAT; family gatherings, lunch invites etc. Initially, I had to become a recluse just to stay on track. LOL It really is incredible how much emphasis is placed on 'consuming'. And 3) going away on holiday requires planning. I load up a cooler with 'good' oils, macadamia nuts, soft cheeses, avocados and sugar free 100% cocoa bars to ensure most of my calorie consumption is fat. The bright side; if flying for travel, you get to skip all those nasty airport/airline meals (and not miss them). Just be strong, the rewards will blow you away.

      Nicely put Phil, The distractions and social pressure is epic when trying to keep to the game plan. I do Keto for diabetic management of which my Doc is fully supportive - but it is despairingly easy to fail to execute in the face of all the distractions. Overall, the benefits have emerged but I could do a lot better.

    This is one of the very, very best videos I've come across on this subject. The explanation of how this works and why is very lucid - if you're only going to watch one video about why those suffering from obesity are fat and how to do something about it, watch this one.
    My Story: In July of 2018 my weight was around 27½ stone (385 lbs / 175 kg). I'd been fat for around 20 years. I'd not long turned 55 years old and just by chance I went for a ride on a push bike and found that my knees / ankles didn't hurt - but boy, was I out of condition / puff. I managed half a mile before I had to stop (sweating profusely). Over the next month to 6 weeks or so, I went out and rode around 2 miles (~3km) initially once every 4 days, then once every 3 days. I managed to make my trips ~3 miles (~4.5km). I had lost about half a stone (~7 lbs / 3kg). On August 19 2018, I decided to go out for a ride on a bike EVERY NIGHT (evening). Other than a few nights lost to the weather for more than 8 months, I was out every night. 3 miles a night became 5 miles a night and on the run into Xmas of 2018, my weight had come down to ~24 stone (336 lbs / ~150 Kg) - but even though I had been cycling further and faster and successfully climbing local hills on the bike the weight loss had stalled. WHY?
    Well, I'd been spoken to like a 5 year old by the staff at local diabetes clinic for years about not eating cakes and biscuits and instead eating "healthy stuff" like cereals and fruit and drinking milk and low fat (which I had actually been practicing, I'll add) and taking exercise... Now here I was putting an hour a night into going out EVERY NIGHT too - and little more coming off. So I went on google. I came across an ordinary American bloke's video called "Butter makes your pants fall off." Enlightening!
    My 2019 New Year resolution was to put the low carb diet into operation. I bought a diary for the year and began writing down my weight on waking (having been to the loo) & blood pressure. Then How many miles I might ride in the evening and finally my blood sugars during the evening. My weight on 31 December 2018 was 23 stone 8 (=330 lbs ~119.6Kg). I lost over 3 stone (~45 lbs / 20 Kg) in 6 weeks and by 16 July my weight had dropped to this year's low of 16 stone 4 lbs (224 lbs / ~101kg). Currently my weight this morning (at 1 September 2019) was 16/13 (9 lbs / ~4kg heavier) - but I have been enjoying the summer and eating ice cream and not quite going out for a ride every day through doing other things!!!
    The long and short of why I've added my story is to underline that THIS WORKS! Sten puts into words what I have found out for myself with a little seeding over Christmas and then lots of empirical experience. No one likes being fat and overweight - but now *I know why* and what would be remiss would be not to do anything about it. And as for OMAD / feeling hungry, this man's comments about this are absolutely true too. Although my body periodically suggests I might want to eat, I really don't get hunger pangs like I used to (where it feels like your belly is about to cut your throat). Eating OMAD is something you just get used to.
    I had previously watched a motivational video on You Tube featuring the late John Rohn in which he'd said "What if you should walk around the block for your good health - and you don't or WON'T?!" along with the punch line *"Come on man - you gotta be smarter than that"* - well thanks to John Rohn and other contributors like Sten on You Tube, I have the way out of the obesity mire.
    So - if you're fat, do it for yourself. There is a way out, it isn't easy but if you want it enough give it a try and persevere. You are smarter than a couch potato - now you know, now you know!

    I have been following your advice as well as a couple of others to get healthy by lowering insulin resistance. It started with fear. I have a fatty liver and suffered with hepatitis (cured) and was obese for many years. My doctor read me the riot act but could not lose weight. I started low carb, moderate protein, fat. After one year now I have just started OMAD because after losing 59 pounds I have reached a plateau. I am and have been exorcising how you recommend. I feel great. I am not hungry at all. I am now only overweight and no longer obese. This not a diet. This is a life change. I grew up on homemade pasta. I no longer have any cravings while those around me eat sweets and carbs galore at the dinner table. My blood pressure has come down as a bonus. I still have a way to go but it is a piece of cake. Pun intended. Thanks.

    Dr Sten,
    I found you just a few weeks ago, and I find your explanations so easy to follow and understand. Thanks for that. By the way, the old erroneous concepts about eating 3-4 meals a day, and not skipping breakfast is SO ingrained in most people's minds that is really hard to "show" them this should be the right way to eat. Even people in the medicine field seem to find it hard to believe. Thanks for such a great work!
