Good interview bringing awareness to the impact of carbs on heartburn. One correction - You said at 8:30 that low carb reduces stomach acid, but your 2006 study was looking at acidity in the distal esophagus, not the stomach. So it actually showed that the acid was not getting out of the stomach into the esophagus not a reduction of acid in the stomach itself. And it makes sense that your body would want to produce more, not less acid with the increase in protein. We have attended some of the same meetings in the past and I wish I had discussed with you my theory (published in "Heartburn Cured" and Fast Tract Digestion Heartburn") that the problem is carbohydrate malabsorption leading to the overgrowth of gas-producing microbes in the gut. The pressure builds up in the intestines, but also in the stomach (higher intragastric pressure in GERD patients is documented in the literature). This pressure drives reflux pushing stomach contents into the esophagus. Based on this idea both cutting carbs but also addressing causes of malabsorption such as pancreatic or brush border enzyme deficiency and many other possible underlying causes are both expected to reduce heartburn symptoms. 🙂
@@lunaticatiga Brush border enzymes are located at the tips of the microvilli and include lactase, sucrase, maltase, sucrase/isomaltase, and others. They break down disaccharides but also complete the breakdown of starches. Testing involves endoscopic biopsies and specialized laboratory analysis.
Did you stop taking them on the advice of your dr or cold Turkey? Were you able to stop taking them immediately after going on a keto diet or did you wait a bit? I’ve been on omeprozole for years and tried to wean myself off of them and it was horrible (I was not on keto -and not on keto just yet).
You can buy chewable omepeazole tablets and slowly reduce the dose. By slowly I mean shave off a little bit over a period of a month, depending on original dose.
My problem was acid running up into my throat at night and it would wake me up choking. It would burn my esophagus and hurt. I started sleeping on two pillows...almost sitting up. When I started Keto, this stopped. I found that if I ate sweets (off the diet), I would have a spell of it. Nothing bothered me but eating sweets.
My GERD went away on keto. Previously, I took antiacids seemingly every night--sometimes being woken up at night by the reflux, but once I reduced total carbohydrate to ~20 g per day GERD ceased.
I have heard you say on a couple of occasions that there are probably some people who are doing Keto who really don’t need to. I’m curious to know who should not do keto. Thanks!

I’m keto and have lost 133 pounds so far. I am hoping that this is true for me when I get to my goal weight because it is the only thing that has not happened. I have had gerd for 22 years. I was told I might have to have an operation then, sounds like I still may need to. Ugh. I am on prescription strength Prilosec.
Was always given the advice to not eat red meat, chocolate, tomatoes, citrus, coffee, tea, fats to help with my GERD. Was taking 4-5 Omeprazole or other PPI for 15+ years. Guess what I ate on a keto diet? Red meat, chocolate, cheese, tomatoes, lemon, coffee, tea, butter. GERD was gone within a month or two, have not needed any medication for it since. Been keto for 16 months now, lost 40+ lbs, only have 10 more to go to be my ideal body weight. Not pre-diabetic anymore, am working on my hypertension and have never felt better. Back to college weight now. Oh, did I mention I am a doctor?
I used to get acid reflux, especially after a heavy (like Christmas) meal. I also thought it must be the fat in those meals. But after a while on LCHF (3½ years so far) I noticed no more acid reflux, unless giving in to a high carb meal on a special occasion. So in my case, the reflux is definitely due to too many carbs.
I’m on a carnivore diet and on OMAD (at noon) and have acid reflux at night, it wakes me up. Any ideas? I have problems sleeping because of it and it hurts my stomach and throat. Thank you
I am confused. I definitely know that keto does help with heartburn. My confusion stems from people who are not on keto. It is my experience that many people who have heartburn are greatly helped by taking apple cider vinegar tablets before a meal. This acidifies the stomach further and their heartburn goes away. The way it was explained to me was that in order to close the sphincter muscle at the top of the stomach, the stomach acid has to be high. When too many carbs are eaten, the acid has to increase greatly in order to close the sphincter muscle. My sister is a good example of the benefits of apple cider vinegar capsules. She has suffered from almost daily heartburn since she was a child. She is now 71 and has always eaten a “healthy” food pyramid based diet with lots of carbs. Just last year I finally convinced her to take two apple cider vinegar tablets before a meal and see if it would help her heartburn. It is now a rare occurrence that she has heartburn. She is not the only person that I have asked to do this experiment. It has helped a lot of people. I would like to put forth a theory. The reason that a ketogenic diet works so well in eliminating heartburn is because it illuminates excessive carbohydrates. Even though the stomach acid goes down with the keto diet, it is still more than sufficient to deal with the limited amount of carbs that are found in this way of eating. For people who are on a standard American diet, I would encourage you to change to a keto diet. If you are unwilling to make the change, I would definitely experiment with apple cider vinegar capsules.
Hi keto your right acid in your stomach has to be very high this is to kill of bad bacteria that comes into your system in food I used to have terrible bloating and heart. Burn even to the point where I had to make myself sick to release the pressure in my stomach I'll explain what caused this my stomach acid was very low because as we age we produce less stomach acid which makes it very easy for bad bacteria to grow in the stomach so as you probably know the food bad bacteria eat are carbs this causes fermentation in the stomach which then causes pressure in the stomach pushing acid into the wind pipe causing burning so the only always is to increase the stomach acid to kill the bad bacteria apple cider vinegar helps but even that might not be strong enough to raise the acidity level enough to kill the bad bacteria off so I would recommend using BETAINE HCL this is far stronger and would work far better at killing all the bad bugs I've gone from sever pain to no symptoms at all but I do stay on this all the time and use digestive enzymes with every meal I'm a keto man forever now I still have apple cider vinegar also because it's very good for you hope this tells you exactly what reflux is all about and you can help more people with this info all the best martin
