Hi Joseph. Thanks for reaching out to us. One of the possible cause is: there is a capital letter in your form, for example, if a column is named Name, it's wrong. It should be name (all letters small). That applies to all other columns. Another possible cause is: You have an empty worksheet with no column headers. Remove any additional worksheet without headers and questions/ choices if any. The last possible cause is using a capital letter in naming your worksheets, for example, you name your worksheet Survey instead of survey. The letter s needs to be small plus the subsequent letters. In a nutshell, ODK is case sensitive. In case you need additional assistance, please reach me on [e-mail]

Hello. Make sure you avoid spacing in the name column, survey worksheet. Instead, connect words using underscores. Make sure all the letters in your column headers are written in small letters. Finally, make sure you only use Version 1. of the ODKXLS Form Online
