I see a lot of comments around on this issue that complain in demagogic phrases about "corporate greed" etc which just show some ignorance how it works. A great part of the shareholders who want returns on their investments consist of pension funds. And about all of the modern western world has a proportionately large amount of pensioners. And these oldies had fewer kids than their parents had. So there is somewhat less money going into the system while still older people, naturally, consume more healthcare. The problem Mangione quoted with having the most expensive healthcare in the world and at the same time lowered life expectancy is real for sure. But as ugly of a fact it is may depend on availability of doctors, maybe there are too few examined. Then there is a demand issue from a population where too many have poor lifestyle choices that unfortunately make them prone to have the chronic illness that some decades back were seen as health issues that struck old people. But now you have surgeons taking off limbs on people only in their 30s because they can't be arsed to try and manage their diabetes. That United Healthcare is a very large company have also a reason that they have made the effort to have among the worst fees/premium rates. I would guesstimate that is the insurance offered by employers with low salaried service jobs and such, which may come with other problems of course. But anyhow, the availability and increased demands for healthcare in general is likely not that much insurance companies realistically could be doing much about. In theory maybe they could but in reality way less (for the perceived individual right of not owing anyone health and hang on to crappy lifestyle choices). Edit: and I'm sad to say that I couldn't help myself from chuckle a bit when I saw that clip at CNN showing the crowd outside the court cheering for Mangione. 9 of 10 of them were obese or larger and pretty much part of the problem Mangione claim to be angered about. I could compare stats from 2017: in Sweden where I live we had 4.1 practicing physician per 1000 of population while USA had 2.6 per 1000. We still have demographic structures causing some problems and some degree of increased obesity rates around 15% but not as high as over 30% as in USA. And there is "slicing" to cut cost in our government subsided system.

But where USA stick out different is a couple of other areas. One is they have fewer medical residencies per capita. Another is the level of individuality and amount of people with poor lifestyle choices, as one example in the last three decades childhood obesity have quadrupled. Aside of massively increasing the risk for chronic illness in early years it is also so that obesity is associated with more forms of cancer than smoking. But plenty of the influencers who want to include obese people into "body positivity" (which initially was for people who could not do anything about their conditions and handicaps) tend to have an attitude they perceive as sassy with they "don't owe anyone health". Europe outscore USA in alcohol consumption though if we compare avoidable lifestyle impact.

I see a lot of comments around on this issue that complain in demagogic phrases about "corporate greed" etc which just show some ignorance how it works. A great part of the shareholders who want returns on their investments consist of pension funds. And about all of the modern western world has a proportionately large amount of pensioners. And these oldies had fewer kids than their parents had. So there is somewhat less money going into the system while still older people, naturally, consume more healthcare.
The problem Mangione quoted with having the most expensive healthcare in the world and at the same time lowered life expectancy is real for sure. But as ugly of a fact it is may depend on availability of doctors, maybe there are too few examined. Then there is a demand issue from a population where too many have poor lifestyle choices that unfortunately make them prone to have the chronic illness that some decades back were seen as health issues that struck old people. But now you have surgeons taking off limbs on people only in their 30s because they can't be arsed to try and manage their diabetes.
That United Healthcare is a very large company have also a reason that they have made the effort to have among the worst fees/premium rates. I would guesstimate that is the insurance offered by employers with low salaried service jobs and such, which may come with other problems of course.
But anyhow, the availability and increased demands for healthcare in general is likely not that much insurance companies realistically could be doing much about. In theory maybe they could but in reality way less (for the perceived individual right of not owing anyone health and hang on to crappy lifestyle choices).
Edit: and I'm sad to say that I couldn't help myself from chuckle a bit when I saw that clip at CNN showing the crowd outside the court cheering for Mangione. 9 of 10 of them were obese or larger and pretty much part of the problem Mangione claim to be angered about. Cognitive decline has gone down the drain.
Edit #2: for the availability factor. I could compare stats from 2017: in Sweden where I live we had 4.1 practicing physician per 1000 of population while USA had 2.6 per 1000. We still have demographic structures causing some problems and some degree of increased obesity rates around 15% but not as high as over 30% as in USA. And there is "slicing" to cut cost in our government subsided system.

Mangione was angry about the equation of USA having highest cost per capita for healthcare yet lowered life expectancy. One part of that is demographic, with a relatively high number of old people who naturally tend to need more healthcare which is also a problem for basically all modern western countries.
But where USA stick out different is a couple of other areas.
One is they have fewer medical residencies per capita.
Another is the level of individuality and amount of people with poor lifestyle choices, as one example in the last three decades childhood obesity have quadrupled. Aside of massively increasing the risk for chronic illness in early years it is also so that obesity is associated with more forms of cancer than smoking. But plenty of the influencers who want to include obese people into "body positivity" (which initially was for people who could not do anything about their conditions and handicaps) tend to have an attitude they perceive as sassy with they "don't owe anyone health".

What can we learn from this?
The CEO was not motivated by money because he came from working class? That's an odd conclusion since you would think not coming from money makes you more interested in it. The fundamental question is whether Brian Thompson was good or evil. Was he the CEO when the infamous policy went into effect. If not, did he do anything to rescind it when he came into power?
