    While we all have heard people talk about the highs of mania or hypomania, and the lows of bipolar depression, a mixed episode happens when both of these symptoms occur at the same time. Meaning that we feel impulsive, having flights of ideas, not wanting to sleep, but are also irritable, physically exhausted, and hopeless about our future. It’s diagnosed as having met the criteria for a manic or hypomanic episode while also having at least 3 depressive symptoms. I know it’s hard to comprehend how someone could feel both depressed and manic at the same time, since we often think of people swinging from one extreme to the other, but mixed episodes are common (about 40% of those with bipolar disorder will have one) and are actually very uncomfortable. Many of my patients in the hospital and in my private practice have described it as having so much energy but feeling terrible about it. One patient said she wished she could just tear her skin off, it was so unbearable. Because the manic symptoms are so impulsive and erratic, but we also feel depressed or hopeless, this state can be very dangerous. When we feel that low and have the energy to do something about it, suicide attempts go up, which is why any mental health professional will be watching us closely if they think we are in the midst of a mixed episode. It can also be dangerous to our bank account and our relationships because we can impulsively spend, have casual sexual encounters, and even look to argue or fight with those we love. Which is why it’s important that we let our treatment team know what we are experiencing so that they can help us manage the symptoms before anything bad happens. On average these episodes last for at least a week, but sometimes 2 weeks or longer. So the sooner we speak up and reach out the better.

    I've been diagnosed and in treatment for a decade and this video did a significantly better job of explaining the causes and effects of Bipolar than any therapist, psychologist, psychiatrist, counselor, doctor, or social worker I've ever had. It’s such a concise answer, it's phenomenal. Thank you for this. It’s already helping myself and loved ones understand better what's physically happening to me, rather than only behavioral/emotional struggles.❤ Also, zoloft sent me to the hospital twice in three months with hypomania, that stuff is no joke if you're Bipolar.

      I believe that, I was prescribed Zoloft once for just depression and I couldn't keep control of my emotions or even think straight.

      @@CheleBadoo yeah, I'm on welbutrin and Lamictal right now. I've been stable for about 4 years, but for a solid year of that I was a zombie on abilify.

    I was diagnosed with Bipolar almost 20 years ago and have never heard of mixed episodes. This was incredibly interesting and describes a lot of my experience perfectly when my meds aren't working and I have breakthrough symptoms.

    Whoa! I guess I’ve had plenty of mixed episodes. It’s awful. Want to cry, but I can’t. Great, fantastical, amazing ideas. But I can’t implement them with depression. It’s not fair.
    When you said a woman’s mixed episodes were so painful, it made her want to rip her skin off, I was surprised that someone else had my exact same experience.

    I'm BP1. I have been having mixed episodes for the last five years. They truly suck. I'm glad my psychiatrist realized I was having these types of episodes. She put me on Latuda which has made my symptoms almost completely diminish. Thank you for all your videos.

    My mixed episodes when I was younger made it take a looong time to be diagnosed with bipolar. They said I didn't "swing" far enough and I swing too quickly.
    When I finally got diagnosed and got on the right meds, it was a life changer. No more being awake for 4 days. No more forgetting to eat for 2 days. No more being so full of energy, but not being able to do anything about it that I'd pace in the same spot for 4 hours.

    Thank you for this, I struggle with knowing if I'm in a mixed episode and could point out some times I might have been with this video. I think something interesting is when I'm in one I often think the depressive thoughts are more true because I also feel "up" in other ways if that makes sense. Whereas knowing I'm depressed makes me realize the irrational thoughts are 100% not true.

    I was in a mixed episode when I visited my psychiatrist for the first time to get diagnosed. I didn't know what it was at the time I was super confused and sort of scared.

    Before I understood bipolar better, mostly thanks to this channel, I used to wish I was bipolar, just because only being depressed sucks. Now I see how out of control a person can be while experiencing mania, I feel for those of you that are bipolar and hope you can find help.

      Yeah one huge myth about mania is that it’s always euphoric and expansive. My mania first manifested as relentless panic attacks and racing thoughts that felt _very_ dysphoric and distressing.

    I have autism, bipolar disorder, and anxiety, and I happen to have mixed episodes. I would describe it as almost feeling like an agitated or angry depression. I start getting very pissed off by things I ideally shouldn't be getting angry about. It sucks when I feel that way.

    In my experience brain speed/energy operates completely independently from emotional extremes (euphoria/dysphoria) so I differentiate ‘mixed’ as either ‘dysphoric mania’ or ‘euphoric depression’.
    There’s a huge difference between the two, and I’ve never heard a professional talk about euphoric depression so I’m guessing nothing’s being taught about it. Also here in the UK I’ve never met a nurse or psychiatrist who could successfully identify mixed states, they’re so invested in euphoric mania/dysphoric depression. I even went to a workshop on bipolar where the facilitator skipped dysphoric mania completely. I asked why, she said ‘I thought it’s not a big problem, is it?’ Half the room instantly shouted “YES It IS!!”
    I’d also like to take issue with the arbitrary ‘at least a week’ duration. If I get manic I know instantly, it’s nothing like happiness or sadness or any other kind of normal. Serious loss of insight only comes later, once I’m completely sleep deprived. I can wreck my life in a weekend, but can’t get anyone to help because it’s not been a week yet🤬

    I'm currently pursuing a bipolar 2 diagnosis with my doctors, learning about hypomania and starting to track my moods to better understand the mania symptoms I feel, which are miniscule in comparison to the depression I'm experiencing. I mentioned you and this video to my therapist today, and after watching this after my session it's definitely giving me some more clarity on what I've been experiencing. I still don't know for sure, but it's seeming like a likely diagnosis for sure.

      You know, my mom is 62. She was diagnosed. I think maybe six years ago
      I'm certain she's almost had problems before then but she won't take her medicine
      She says the medicine that they have. It causes damage to your organs when you're older
      Is this true? Do you have side effects because you take the medicine
      I don't know because I have CPTSD
      I had a great medication 5 years and it's never caused any problems to my body
      Answer I wish I could have taken it in my twenties and thirties.
      Suffered a lot because I didn't have access to medical care. Until I went to taiwan.
      But I too was told the same thing that medication destroys your organs
      From what I understand though the percentage is very low. And i'm bothered to tell me that.
      I'm back in California now. And i'm going to try to get medication here through medical
      I just wish my mom would take her medicine
      Let me talk I swear it's almost like she's always having mixed episodes

    I'm experiencing this right now. An SSRI and Anxiolytic sent me into a mixed episode. It took a while to figure it out because the activation feeling felt like anxiety until it became full blown mania. It is truly miserable. Thanks for sharing!

    I have ALL the mixed episodes. It’s the worst! Thank you for this video. It was wonderful, as always. (This is Jori C by the way - I just changed my account name, not my real name.) I didn’t realize the gray matter was decreased if bipolar is left untreated. That has certainly been my experience though - I can think much more clearly when I’m treated. And I don’t think it’s simply the control of the illness. I feel … smarter. But I also know from school that the longer you let your brain remain sick, the more it wants to stay that way. So I never had problems with wanting to stop my medication.

    Hi Kati ! I'm struggling right know with a mixt episode. I was rushed in the psychatric hospital in may by my family after expressing suicidal thoughts. Since then I'm hype or really down. It is the most severe episode I had since I was diagnosed.... My thoughts are racing, I spent almost 500$ in 3 days and hate myself for this and putting my loved ones through all of it.... My psychiatrist changed my treatment, fingers crossed it will get better soon. Thanks for educating people on mental health. Take care ! ❤️

    I had a mixed episode shortly after my first manic one/diagnosis. It was the scariest, most turbulent time in my life. A simple explanation like this would’ve helped so much in figuring out what the hell was going on in my head. I’m on lithium and a handful of other things. It has saved my life.

      Stop pushing cbd . It hasnt been well studied and isnt controlled in terms of quality. There was a study that came out a month or so ago that tested melatonin cbd gummies for multiple brands found absurd levels of all ingredients . You better believe that affects your mood

    Research shows that those who present mostly with mixed symptoms are much less likely to be lithium responders. It appears lithium is more effective for those with "classic" bipolar.

      I wanted to say that but worry I would sound superficial.. ! Her hair looks beautiful in this video, so shiny and healthy and more blonde than usual. 👱💕
      I think she doesn't always straighten it because the irons damage the hair , that's why it always looks so lovely and sleek when she does straighten it!

    Welcome to my world! I have bipolar 1 and mixed episodes are common for me. Usually, I just avoid people.I stay home and write poems while chugging coffee. Occasionally I see a shadow out of the corner of my eye.
    How do I cope? I call my doctor and they put in a prescription for me. Otherwise, I ride it out. Suicidal ideation happens often out of the blue for no reason. Since I was diagnosed in my 20's and I am now in my 50's, I am he to this.

    I absolutely cannot take SSRI’s or any other antidepressant, other than Trazodone, or I experience the worst manic or mixed episodes ever. The worst and most dangerous episodes I’ve ever had were when I was treated for depression without a mood stabilizer or antipsychotic. But now I’m on an antipsychotic, Zyprexa and have been able to get off of my mood stabilizer and use Trazodone. It’s been a life saver. My psych said it’s one of the only antidepressants known not to induce mania because it’s also a sedative (and other reasons of how specifically it works on the brain). I’ve had great luck with it. No episodes, good sleep, and helps with anxiety. Just wanted to share because I’ve literally tried everything on the market because my depression was so bad but I couldn’t take anything for it because it sent me way off in the other direction and finally found something that didn’t.

    I'm brazilian and diagnosticaded as bipolar for over 10 years now but there's not much content in portuguese about it (and the few that we have i already consumided it) so i'm here (even i'm not fluent in english) to learn more about this disease. Thanks for sharing with us.

    Thank you! I have been medicated for depression for many years. I was also married for 37 years. Recently, my MD changed my diagnosis to a more severe form of depression and my license to drive a school bus was 're-classified' and I needed annual medicals. During the pandemic, I could not get a medical evaluation and I am now a car driver only! That is not a problem. My problem has more to do with "Narcissisms". My EX! I have entered a program called NARP, Narcisstic Abuse Recovery Program. I am feeling much better about myself, in fact starting to believe my "Depression" had more to do with my life situation. While I have continued my medication, is it time to have this reviewed again?

    I was diagnosed with Bipolar 1 Mixed, with rapid cycling. That was back in the year 1995. I was given Lithium, Prozac and Ativan and over time, I was taken off Prozac because it seemed to exacerbate my manic episodes. Now, at age 66, I am not taking Lithium due to the fact I developed elevations in creatinine. I am now taking Hydroxyzine HCL 25 mg at bedtime but this has not reduced my instances of mania or depression over the past few years. I have been through Cognitive Behavioral Therapy and once was prescribed Seroquel but it had negative side-effects. Due to Covid-19 I began to socially isolate since 2019 and seldom go anywhere. I have basically become a recluse. It was suggested by my primary care doctor that I should see a therapist but I am reluctant because I have deep fears of the possible side-effects of any anti-psychotic medicines. I have developed a social phobia. I feel anxiety every time I go out in the world. I need advice.

Hopefully I can be the first to comment next time, but going into the topic of this video bipolar disorder and mixed episodes sounds like a horrifying experience for anybody who suffers from it. I don't suffer from this disorder myself but just thinking about what those that have it go through, my heart pours out for them.

      Yes it sucks. Had my first experience in college and had to be hospitalized (awake for 7 days). Since that time I've been medicated and haven't had another extreme episode like that.

    Last time I had a mixed episode I knew I wanted to jump out a plane, I just didn’t know if I wanted to use a parachute or not. I can handle the up and the down but mixed episodes are the absolute worst. It’s an experience that is really impossible to understand from the outside, even living with bipolar didn’t prepare me for my first mixed episode and unmedicated bipolar is crazy enough.

    I am in a mixed state right now. I was released from a psychiatric hospital 3 days ago. I was admitted voluntarily, but on the strong advice of my psychiatrist. I am sleeping enough, but on a highly irregular schedule. I am definitely drinking enough, simply because I am pretty thirsty. I am eating enough, on a pretty regular schedule, but ofc based upon when I sleep/wake up.
    I am bipolar type 2 and are thus in a hypomanic state, but my depression is quite severe.
    I am in the process of increasing the dose of my mood stabilizer and also get some more professional help. I do not know what, but likely therapy.
    Any advice on how to handle my mixed state are fine. But I could use some further advice on how to handle this/get out of this mixed state. Words of encouragement are also highly appreciated!

    My mania last night was insane and I was all over the place. I figured i would grad my water jugs and do arms curl until i was wore out and it worked cause i was tired and ready for bed about 20 mins later.

    I am currently in a mixed episode and it's pure hell. I have bipolar 2 and I have borderline as a comorbid as well, so the paranoia is rampant, and I haven't been sleeping, I feel run down and wired , I want to spend what little money I have on nothing useful, impulse spending is my goto now that I am soberish (I have had a mini relapse during this spell) then feel ashamed for even window shopping. I am paused at 1:30 and shaking with both tears of validation and overwhelming anxiety. I can't stand the constant screaming thoughts, like there's 8 of me all fighting in my head. Beside where I am typing this comment is the recommended video for Maladaptive Daydreaming, I have cPTSD and I have been finding myself just staring at the wall for sometimes hours, to avoid that I have been figuring out tik tok because it's like a constant new laugh every minute, but then the dopamine crash hits (reminds me of my Ecstasy abusing days) and it's back to staring at the wall. I know that this is primarily because I let some people who weren't healthy for my mental health into my life and I stopped taking all my medications on schedule then ended getting drunk after 3 years sobriety a few days ago. I haven't even pressed play again on the video, but I know once I start to get back into my routine, doing my healthy habits, and taking me medications on time I will start to level off again. OH and also throw a touch of PMDD in the mix, cause the theme in my life is "when it rains, it doesn't pour. It's a goddamned hurricane"

    Hi Katie,
    I am finding a plethora of information about how to help loved ones with a mental illness. Can you provide some tips on how we, as the family member or partner without the illness, can cope without losing our minds ? I mean that literally. I can't imagine how much the ill person must be suffering and caring about someone who is ill and actively symptomatic can be exhausting especially over time. It starts to really wear you down and your body starts to register this person as unpredictable/unsafe to be around. It starts to wear on the mental and physical health of those around them I am hoping you have some nuggets of wisdom to share. Hugs and many thanks.

    I just went through one, the first one I've been able to recognize since being diagnosed. It was freaking awful. If there wasn't stuff under my bed, I would have been hiding under it for 3 days and have my wife slip me some food. It was a few days leading up to it and about a week to fully move past it. Cant wait until the next one 🙄

    Can Bipolar Disorder cause rapid cycling daily and even several times a day, where one switches back and forth? Or is Bipolar cycling only every now and then rather than daily?
