Revenue codes are 3 or 4 digit code in hospital billings (UB-92 claim from also known HCFA-1450 FORM) that explain the description of patient's treatment to the insurance companies.
where as CPT codes are 5 digit codes in professional billing or medical billing or physician billing (CMS 1500 Form) that explain the description of the treatment of the patient's treatment to the insurance companies, but are billed in physician billing.

If patient responsibility applied and no secondary insurance available or inactive. Then patient is need to pay him/her self

Hi Rashmi Thorat....
If claims denied correctly they will not pay you back. You will given a recouped reason why amount recouped. If no reason mention on recoupment, call & ask why recouped. If u denied the recoupment, call ins reps & probe the same.

Rep saying that due to special project called medicaid FHP cost recovery. It was not money retraction the claim adjustment for the overpaid claims that refunds were requested that never received. No money was retracted this was adjustment internally to reflect a refund the adjustment is does not effect to intial claims status..

@@Babumahesh149 Yes generally this things happens from state Medicaid payers (Secondary Payers) based state health guidelines & programs (QMB, SLMB, FHP, FPL...Etc) providers technically not get paid. Then we can adjust the claims ,in this case we can't bill patient because patient has Medicaid plan type.
