For vertical and pull-out test it is applicable but for lateral load test 1.5 or 2.5 load is not applicable. Also in IS this is not mentioned.

What is the size of Concrete block for jack support,
That block size should be sufficient to take the jack pressure reaction,
Practically the block weight should be Pile test load x 2.5 (to account the friction resistance of concrete and soil)
