Is precipitated withdrawal really all that bad?

  • Dr. Peter Coleman, who is Board-certified in Addiction Medicine and has been in recovery from opioid use himself since 1984, discusses what causes precipitated opioid withdrawal and just how bad it really is.
    -Safe, Comfortable Outpatient Detox
    The Coleman Institute for Addiction Medicine has been a leading outpatient addiction treatment center since 1998. Our innovative withdrawal management programs, often referred to as The Coleman Method, have helped thousands of people free themselves from addiction to Alcohol, Opioids, and Benzodiazepines. Originally founded in Richmond, Virginia, our network has expanded to cover 14 cities across the US.
    -3 - 8 Day Opioid Detoxification Process
    For short-acting opiates like oxycodone, most patients complete their detox and begin Naltrexone therapy within 5 days. Longer-acting opiates like Suboxone and Methadone typically take 8 days. Our standard opioid treatment package in Richmond, VA, Wellesley, MA (Boston area) and Willoughby, OH (Cleveland area) includes the detox and 6 months of case management and Naltrexone therapy. As a non-addictive opioid blocker, Naltrexone dramatically reduces cravings so that patients can focus on their recovery. We have a 98% success rate for completing detoxification and starting Naltrexone therapy. During the detox, we work with patients and their families to develop an appropriate aftercare plan. We also offer a convenient, 3-day outpatient alcohol detox and a Rapid Benzodiazepine Detox.
    -Caring, Empathetic Environment
    Our founder, Dr. Peter Coleman, understands first-hand the challenges our patients are facing due to his own personal struggle with the disease of addiction. He has been in long term recovery since 1984. Dr. Coleman has dedicated his career to treating addiction patients and creating a team and an environment conducive to long term recovery for his patients.

      @@getstraight234 totally agree. It without question causes ptsd. Ptsd is something I don't see talked a lot about when it comes to withdrawal/precipitated withdrawal, but it's an incredibly real thing and one of the main things that keeps people on opiates longer than they want/should be. The ptsd from the withdrawal will cause most people to do almost anything to avoid it. It's a very real thing and it needs to discussed and treated appropriately far more often than it is. Opiate addiction treatment/therapy still has a lot of ways to go in many areas and withdrawal ptsd is one of them.

      @@barefootiguana2760 a hundred percent agree. I've been in situations where people start talking about times or they've had precipitated withdrawals and I seriously have to tell them to stop talking about it because my heart heart rate I start going to the roof just thinking about it, I'll start breathing heavily and sometimes even get that feeling I had at the beginning of the precipitated withdrawals just from hearing people talk about it.

    I love when these doctors think they know it all because they have studied it. Unless you have actually been through precipitated withdrawal don’t try and say it’s “not that bad”. It’s the absolute worst experience/feeling/sickness I’ve ever felt in my life. Think about having the worst flu of your life and feeling like shit and multiply it by 1000x. I honestly don’t even do drugs to get high anymore I do them because I am deathly afraid of going through withdrawals again it was the absolute worst experience of my life.

      Dude I’m right there with you. I’m praying this Bernese method of micro dosing I’m doing right now will at least make my predisposed withdraw only last 4 hours or so instead of 16 like it was like time I got clean. So ready to be off these pressed m30s. I swear the devil made this shit in a lab himself. It stays in your system for 5 days. Notice he only said fent once he didn’t bring it up again. We use an approach like the Bernese method to help patients get into buprenorphine. It is a wonderful medication for many people. Thanks for engaging with our content. If you have other topics you’d like us to tackle, please let us know. Best wishes.

      I totally agree with you. It is the worst medical crisis I have ever experienced. After tapering down to 2mg of Suboxone, I was ready to transition to Naltrexone. I only took 1 50mg tablet as prescribed and within about 1/2 hr. I was on my way to the ER with precipitated withdrawal symptoms. This occurred as even after several weeks of stopping Suboxone it was still on the receptors. I spent the next 36 hrs. in the hospital for observation.

    Put it this way. Almost all the people I know that experienced precipitated withdrawal for the first time, went to the er because they legitimately thought they were dying, myself included.
    It’s like a barrage of g forces slamming into your brain, you can’t see straight, and if you close your eyes it’s like you’re spinning while falling down a dark hole.
    Your stomach REELS in pain. The shooting pain that takes your breathe away then folds over itself into a dehydrating cramping pain that will bring you to your knees. Your entire body covers in goosebumps and your teeth chatter so badly you cannot talk. The sweat pours out if your pores which only intensifies the freezing feeling.
    And just as you’re about to pass out from the spinning, every fragment of waste in your body comes flying up your esophagus and it’s a thick, suffocating burst of vomit which sends shivers up your spine, over and over throwing up. Exhausted from the painful vomitting you lay there, ears buzzing, stomach reeling, just to have what disn’t come out your mouth now bellowing in your stomach and you have two seconds to make it to the toilet to then blow the poison out your backside. All the while freezing cold, sweating, spinning, tasting and smelling chemicals.
    It’s so fking bad man.

I remember feeling like alarms were going off inside my body and then feeling like I swallowed a pitcher of ice cubes! Followed by projectiling out both ends, then rolling around in bed for several hours. The worst part? I had a 4 mg Dilaudid pill that fell on the ground next to my bed, that I didn't even notice was there until about 6 hours into the ordeal! Once I found that and put it in me, along with a little bit of crystal it gave me enough energy to make it to my plugs house even though I still felt a bit sick. Addiction is a crazy disease

      I really like your comment. I am finding not a lot of people know about PW or actually have experienced it. I actually did it by accident. I was feeling a bit sweaty... had to go to work.. thought oh take my husbands subox to help... I was so very wrong. I was at work had NO CLUE what was happening to me. There is not enough info about PW at all out there!!!! I ended up in the hospital! From about 9:30 am to 5;00 pm of just agony. Felt like I was being put in a microwave. ALSO.. my entire body felt everything... my clothing hurt even... so bad. Sweating non stop, cold and hot, nose running to the point I could not even wipe it, anxiety x 100, my body would not stop jerking around, I could not stay still but wanted to lay down. OMG the shitting almost killed me, as well as the barfing. I almost fainted on my boss.. when I went to say I had to go home. Work was trying to get me to a medical clinic or hospital... but I did not want them to know what happened... I though I had an allergic reaction to Subox... HAD ZERO clue PW even existed. I also remember my vision not working.. so I could not look up anything to help. I actually think I have a PTSD from it. I can not stop researching about it.... SCARY man. Finally at the hospital at about 4 30 pm they gave me some Advil and an Ativan. that little Ativan was what helped me calm down. Was discharged about 7 pm. What was SUPER scary... was the heart rate, it would NOT go down... the main reason I went to the emergency room really.... PW is no joke man... I will NEVER have that happen again... Those that have actually experienced it... no way would say.. its not that bad. It is that bad, no wait it is worse then that bad. worse then you can imagine!!!!!

    Early on during MAP treatment I was overtaking my subs. When I ran out, started to withdraw, so I took a pill called naltrexone. My girlfriend was taking that for alcohol dependence. Naltrexone sounds like naloxone. I thought her medicine would stop the withdrawal process. Nope. It sped it up immediately. 20 minutes after taking her pill I felt like I was in day 3 of opiate withdrawal. Not good 😬

    If he's talking about a 2-3 mg precipitated withdrawal that's not that bad.. anyone who's gone through 8-12mg+ precip will tell you it is truly hell on earth. Sheer brutal torture. Indescribable.

    I experienced this most recently kicking fentanyl, i was in day 3 of withdrawal from that, and thinking that i would feel better like so many times before using bupe to kick oxy i did. I think i took about 4 mg of sub and within 20 min had the worst imaginable pain throughtout my body and head ive ever experienced. I couldnt think straight and thought i was swaeting out icicles. My bloodpressure felt like it was comstsntly fluctuating and dropping.. Im pretty sure it would be the worst form of torture ever if it could be imduced on someone over and over again. .That would be the closest thing to hell imaginable.. Anyway after realizing the bupe was fighting off what fentanyl was still in my body the pain finally susided after I gradually took more and more bupe.. needless to say i researched and learned what microdosing was after that, and its effectiveness.

      Question for you. I too have had a similar experience and have read that more and more bupe will eventually bring you out on the other side of sickness. One doctor said he's seen 64mg in first day to bring someone out of PW. How much did you end up taking? 16mg didn't even touch my symptoms but I always read where 16mg is the limit first day dose.

    • @cljr16
      @cljr16 2 หลายเดือนก่อน

      @bradfordalan6924 I really don't think there's a method to taking bupe at a set time and amount to prevent /avoid precipated withdrawal successfully. Theres to many factors that come into play. I've learned this the hard way every time. Yes it subsides after a couple hrs with increased bupe intake but sometimes its dragged on longer. I don't ever plan on going near fentanyl again the risk and discomfort just isnt worth feeling like that. The only thing that worked for me coming off ff it comfortably is to take methadone for the first few days if you can get it.

  • @travisclymer2517
    @travisclymer2517 วันที่ผ่านมา

    I went through it last night. It was horrible! I took a 8mg Sub 24 hrs after my last dose of methadone and holy hell. Within 5 min I began to sweat and get cold/hot flashes. I got horrible diarrhea and the anxiety was brutal. It was horrible as I didn’t know how long it would last. The anxiety and hopelessness I think is the worst. Luckily, I took some clonidien, gabbalentin and some dyoxasomething and fell asleep 2.5 hrs into the PWD and woke up 10 hrs later feeling better. I feel like I got lucky as I only had a few hrs of it, but it was NOT cool. I couldn’t imagine having PWD for 6-12-24+ hrs. That would be pure torture. Luckily, it didn’t last super long. Maybe those meds helped too. 😦

    Participated withdrawal is HELL. Its difficult to put into words. I was using kratom, and took 50mg of naltroxone 1 hour after large kratom dose. Very stupid move. Next 12 hours I was in hell. Skin crawling, head spinning, could barrely walk, hot/cold flushes, whole body restlessness, twitching, Shitting every 10 minutes, Almost shit myself. Intense dyxphoria, obviously no sleep. And kratom is a partial agonist. I cant imagine what its like getting hit with narcar from H of fetty.

    Is he for real? He said precipitated withdrawal is not that bad! Well, doc, if YOU haven't had a bout of a full onslaught of that experience, then i suggest you do better research. I had not one but two bouts. It's much worse than a normal withdrawal. The 24 hours were hell. Talk to more addicts who went through this.

  • @neilwattoo

    Lies. I bet everyone would agree, it is the WORST thing imaginable. There are NO WORDS for how bad it is. It's the only thing I truly thought would kill me, and only thing I wanted to die from. It makes every opiate withdrawal feel like a cakewalk. I've done em all, a hundred times. I've gone through precipitated withdrawal 1x, and I never ever want to go thru that again. Shitting myself, feel 200 degrees, then 30 degrees, like razorblades are going thru your blood, and it's not just the kicks it is the entire body kicking. I remember laying on bed and my whole body flailing like the exorcist had me. How I didn't die or go to the E.R. and somehow toughed it out, I dunno. If I had an enemy and wanted them to suffer, this is what I'd want them to go through. It's that bad. 10 years clean now. Good lord, I'll never forget that fkn day. Some serious PTSD that I'll never be able to get over.

The doc finally hit me with valium because i wouldnt stop screaming and cursing them out. They also told me i was the first patient they ever witnesses escape the gurney straps by using my teeth 😂. At that point they had enough of my 💩 and sedated me lol

    You have to understand that from a doctors perspective compared to serious diseases like cancer, MRSA, etc. Precipitated withdrawal is relatively banal. When he says "not that bad" I think what he means is that it doesn't usually last more than a day and doesn't have serious long term effects. Go sit in a pediatric cancer ward for a few hours and then whine to me about your precip withdrawal. And yes I have been through it myself before so I know how uncomfortable it is.

      I've been abused in the past and I'm telling you now, if I was forced into either, I'd much rather be abused for a week straight than go through even one day of precipitated withdrawals again. My body is very sensitive to withdrawing and can be alot more intense and last alot longer than the average person. Doesn't matter if your comparing it to a children's pediatric cancer unit or not, it's still absolute hell on earth.

    • @user-rh4qv4gd8t
