      Just wonderful, I been tryin to find out about "mild fatty liver" for a while now, and I think this has helped. You ever tried - Bachalaswin Better Body Rule - (do a search on google ) ? Ive heard some interesting things about it and my mate got excellent success with it.

    I was diagnosed with cirrhosis based on Fibroscan requested by my gastroenterologist and on december 2021 my score was 15.7kpa, so stage 4 (cirrhosis) which was caused by fatty liver disease, I started the ketogenic diet with intermittent fasting , 1 year, 75 fewer pounds later, yesterday I went for another fibroscan, my score went from 15.7kpa to 4.5kpa, stage 1(mild/no fibrosis) so.. Cirrhosis IS reversible

      I have cirrhosis of the liver caused by a fatty liver (the worst kind of cirrhosis) but it took years before I got into trouble. Gallstone blockage on and off for 10 years was part of the problem, but I also abused alcohol for about 20 years before I stopped drinking and smoking. The rest was genetic. (I am 76 so I can't whine that life cheated me.) I am still at cirrhosis stage 1 and there are 4 stages. Stages 1 and 2 are considered capable of self-healing if you remove the culprit causing the pathology (i.e. stop drinking totally or get treated for other causes), but it's no guarantee, especially if you have a fatty-liver genesis. Stages 3 & 4 are more complicated but surgery, like a robotic surgery liver shunt (tube connection) from vein to vein can alleviate water buildup (ascites) and varicose veins in the stomach or esophagus--which can bleed profusely and cause death, unless you can get to the emergency room in time. Oh, and liver transplants have a cutoff date of age 70, or younger if you have other issues like high blood pressure and heart problems, etc.

    those changes in portal flow (the portosystemic shunt) trigger renal vasoconstriction because:
    1. portal hypertension makes the splenic ven vasodilate
    2. vasodilation leads to arterial underfilling
    3. sympathetic system gets activated (renin-angiotensin-aldosteron, endothelin activation)
    4. renal vasoconstriction

  • @marcuspersson7110
    @marcuspersson7110 7 ปีที่แล้ว +13

    I think you guys are doing a really great job, but I'm certainly missing some key aspects of the ascites formation. In my understanding the main source of transudate/ascites fluid is not from the liver itself but from the greatly increased pressure in the splanchnic circulation. This is also the reason why you can see some reduction in the kidney function since the splanchnic bed reacts to the greater pressure by dilating fully ((caput medusae) but not sufficiently to reduce the increase in pressure from the portal vein) and hence, the splanchnic bed holds a great amount of the blood volume. This leads to a relative hypovolemia which is recognized by the kidney as a blood loss. In order to secure it's function and maintain it's own blood supply, the kidney activates the RAAS which further exacerbates the ascites formation by increasing the amount of circulating volume.
    Any thoughts?
