I though a baby's gender is determined at the moment of conception when the sperm fertilizes the egg and it is identified during pregnancy through an ultrasound, when the baby's genitalia are visible enough to determine sex. Now it is assigned after birth. I am confused can someone explain. Maybe i am wrong and it is not the case anymore.

If I were biologically male (all else being equal), I'd be a man. It has nothing to do with some separate "gender identity" that just so happens to match my biological sex. In fact, I wouldn't say I even have a separate gender identity, any more than I have a "height identity" or "age identity" as distinct from my physical height or chronological age. With respect to all these characteristics, my body just is whatever it is; I don't need to "identify" as anything. Nobody calls me "cis-height" or "cis-age" or or cis-anything else, and likewise I see no reason to be called cis-gender. Of course I won't throw a tantrum if someone does call me that. But it is rather hypocritical of some people, who get grievously offended when others don't use their preferred terms to describe them, to insist on describing me as cis-gender whether I like it or not. One wonders why some feel the need to push so hard for the use of this term. I suppose it's a way of perpetuating their theories on gender (in this case, the idea that basically everyone experiences a gender identity as distinct from their biological sex, not just trans people), but in an implicit way so as to avoid pushback. It's also done less subtly with terms like "assigned male at birth", which tries to assert (incorrectly) that "maleness" is just a title that people get given, in contrast with the fact that they were male from well beforehand and would have continued to be male either way.
