Cyberpsychosis isn't inevitable, but having a lot of implants is a risk factor. Removing implants when you start to have symptoms can help, but as you see, it can be hard to convince someone to give them up. The creator of Cyberpunk based it on roid rage, saying that not everyone who did steroids would fly into violent rages, but some would. He's also said that living in a healthy psychological environment is protective against it. Growing up with a caring and loving mom, and then almost immediately finding Lucy and the rest of the gang was at least partially the reason why David handles using his implant so well.

Literally meaning there would be less cyberpsychosis if the world weren't so messed up, leading to the world being even less messed up. So many could've beens, both in the Cyberpunk universe and our reality.

In universe, cybersychosis isn't well understood, but it's generally believed that it's caused by adding too many implants too quickly, and can be stopped by removing some implants to let the brain adjust to the modified body. Out of universe, a theory that I've heard is that "cyberpsychosis" is just a combined term for all kinds of mental illness, since almost nobody can afford psychologists and psychiatrists, but it does get exacerbated by overloading the brain with too many mechanical components to deal with.
21:28 it's not so much that David's "not a murderer". He already killed in a previous episode, and he's about ready for the average gangoon.
But there is one rule in this city almost every merc knows: you don't. Fuck. With Maxtac. Even Maine at his worst and most unhinged couldn't take them in a straight firefight, they're just that good.
So he's saying "you can't do it" because David is literally too inexperienced and not chromed up enough to take on the literal "psychosquad", as they're called. And they're called that because they're practically cyberpsychos themselves, just "under control" enough to function as a unit.
If you ever reach wanted level 5 in the game, and you hear that droning alarm go off, you'd either best be level 60 and chromed with the best cyberware and perks you can get, or RUN.
