Rosacea Skin Care - Dermatologist Review
Rosacea is the number one cause of sensitive skin. Skin care in rosacea patients has to be simple, yet precise as the threshold for skin irritation is low. Pimples and pustules can lead to compromised barrier function, further worsening skin sensitives. A few hints in regards to rosacea skin care include-
    1. Use low irritant topicals: everything from cleanser to moisturiser. Obagi and La Roche Posay make excellent cleansers and moisturisers, as does Cerave.
    2. Physical sunscreens with zinc and or titanium dioxide provides the best SPF protection with minimal irritation. Invisible Zinc is my favourite.
    3. Skin care actives should be carefully titrated. A sensible step would be addition of retinol once a week, increasing as tolerated. Formulations from Environ or Obagi are usually more cosmetically elegant compared to The Ordinary. If your skin can tolerate retinol, consider B3 - niacinamide. This topical can reduce skin inflammation. Good vitamin C formulations have a pH of 2-2.5, not a good idea if you have 'angry skin.'
    4. Rosacea specific topicals include metronidazole, ivermectin, sulphur, sodium sulphacetaminde, topical erythromycin and azealic acid. Discuss these option with your physician or your medical dermatologist.
In the context of skin rejuvenation and repair, I prefer lasers over topicals/chemical peels. Less side effects with the added benefit of targeting red skin.Vascular lasers in the wavelengths of 532, 585, 595, 755, and 1064 can target haemoglobin, in turn decreasing redness. I currently use the older V Beam, Alex 755 Syneron, VBEAM Prima 595, Scition 1064, Prima 1064, Lumenis 1064 in paint or traditional mode. Occasionally BBL or IPL with red filters are employed in the management of rosacea.

I have papular rosacea. Been very sensitive since I was a teenager and am now 32. I've tried almost everything, and have finally found a routine that is helpful. In the AM I use the La Roche Posay Toleraine dermo cleanser, followed by the LRP Toleriane Mist, then CosRx Snail Essence, LRP Rosaliac Serum and finally LRP Cicaplast Baume. If I am going outside, I will wear LRP Cicaplast Baume Spf 50. In the evening, its a much simpler routine. LRP Toleriane Dermo Cleanser, LRP Toleriane Mist, a pea size amount of Differin and then a good layer of LRP cicaplast baume.

I follow it with La Roche-posay Toleriane ultra light moisturiser, I also useTretinoin 0.05% every night. And always use a sunscreen. - la Roche-posay 50+ ultra light. I have rosacea and I have had it for years and I think it has come from face's exposure to sunlight even with so much sunscreen on. I have been on a skincare journey trying to find what works for it, and this video has helped tremendously because I know there are so many actives with benefits but I dont know how to build up a tolerance because the slightest bit of anything causes heat and irritation/inflammation. So thank you so much for your suggestions, I have started to use Niacinamide from the Ordinary and applying it in the morning and in the evening and I use Bioderma Sensibio miscellar water cleanser and hydriobio light cream moisturizer.

I’ve had rosacea since puberty 😕 my skin is super dry and sensitive to new products, it’s definitely a trial and error to see what your skin likes and reacts well to. Neutrogena toner, which doesnt have alcohol, is a must. Clinique lotion hydrates amazingly. I used to use the hydrating Cerave cleanser but never felt it was really cleaning my face, regardless if it actually was or not. I currently use one by Dr. Brandt which you can buy at sephora or ulta, it’s the yogurt ph balancing one, and for whatever reason my rosacea likes it and I feel clean afterwards. I definitely want to look into that La Rosche Posay line that he mentions, if anyone with dry rosacea skin has ever used it please let me know what you think of it.

I definitely agree with watching out for irritant topicals. I found out the hard way. I was covering up my rosacea with make up and it was only making it worse but when you are a woman in your twenties and self conscious about how you look these are the lengths you go to and for me it was better than walking about with a red face and pimples all over the place. I was almost heartbroken when my doctor and dermotologist both told me there was no cure and with my skin getting gradully worse I was caught in a vciious cycle and I felt like tearing my hair out. They were wrong! I kept trying various creams and ointments and struck it lucky with proskin rosacea by mama naturals. My rosacea skin started peeling off like an old mask in scraps/patches (I felt a bit like a snake shedding its old self) and one time in the shower a huge patch fell off and I burst into tears because my skin looked almost back to normal. I was a babbling wreck and it felt as if all of my pain and sorrow over the past twenty years came out of me in a storm flood. That night I felt emotionally drained but the next day I felt I was walking on air and my skin is looking amazing these days. I still use this as a preventative one day per week. I also use a fab milk cleanser by NYR that is soooo soothing I can't even begin to explian it and the combo is a winner for me. My combo may not work for you but there is a cure and don't believe the naysayers who say there isn't.

I’m 49 and had rosacea sine I was s teenager. I’ve only just in the last few years or so feel I have got some control. The main aim is to decrease the Demodex mite.
I know this will sound strange but I use baking soda to wash my face, you have to be careful I don’t dry skin out completely, so not every day use. Also washes and creams with tea tree or Manukua oil, this kills the mite but also need to use with care due to sensitive skin.
Also love niacinamide from simple range, if to strong mix it with something gentle like hyaluronic acid.
Also red and IR lights or IPL work wonders.
Although my skins not perfect and still prone to flare ups, I’m very happy with the control I have now.
Now it wrinkle time 😒. But I can now happily use retinoids with out a reaction.

''are you ok?'' and judgements like that really affects self esteem, every time i look in the mirror i just feel depressed, but i gotta say, i recently tried laroche posay AR and i bought niacinamide from the ordinary and i really hope it's gonna help decrease my redness and bumps, it's not as bad as in the video but still, i'm glad you talked about these products, gives my a little hope since it's not treatable

I’ve been having rosacea since I was born. I’ve never had acne but I do have little pumps (texture) and the broken capillaries 😭interestingly I’ve never had “sensitive skin”. I’ve tried many products & I’ve never had any reaction to anything. It’s just the rosacea is there permanently and doesn’t go away or increase, just the cheek flushing.

;o; Every dermatologist I've seen suggests Cetaphil and Cerave, but both make my rosacea way way worse. Cerave worked great for hydration, but I had bad flare ups every time I used it within maybe 15-30 minutes of application. Literally the only moisturizer that hasn't irritated my skin that I've tried is nadinola cocoa butter cream. I have no idea why it works or if it just works for me, but I've used it for 3 years and I love it. Every other one of the dozens of moisturizers I've tried the last few years has made my skin much worse. Argan oil also works well for me. Neither completely clear my skin, but they calm it down a lot to the point where it's a lot less irritated and noticeable.

I’m Indian (lighter to medium skin tone) 25 years old and I’ve always flushed easily however now I have permanent redness on the sides and tip of my nose as well as little pimply white heads that come up when I get super hot. Those bumps go away quickly and don’t stay like pimples would. I’ve always had a skincare routine and I’m vegetarian so I eat quite healthy. Never had acne growing up. That has improved my symptoms so much that I have slowly introduced some PC Retinol that I couldn’t use before now I can with a better skin barrier, also some glycolic - mixing with an oil or moisturizer. Thanks for discussing rosacea. It’s such a delicate balance and can be thrown into chaos by something like simply getting overheated.
I have the acne type, and I’ve been waiting on a pulsed-dye laser treatment. However, no one here does the procedure, so again I wait.
The best skincare combination for me is Dr. Sam Bunting’s Flawless Gel Cleanser and Flawless Moisturizer with Niacinamide. Still - a few times a week I use prescription Plexion Cleanser and Soolantra. I haven’t needed oral Minocycline in a while. I work hard at avoiding flare ups. All-mineral sunscreen really helps and doesn’t irritate my eyes or skin. I also had a cutaneous T-cell lymphoma spot (only one - go figure) and get checked at MD Anderson annually. I’m about to turn 60, and I would love to see continued improvement in my complexion.
Dr. Lim, I would love to know if pulsed-dye laser is what you use for your acne rosacea patients with aging skin with some visible broken capillaries.
    Thanks kindly.

Maria Genovese ~ Maria, I have problems with allergies which also affect my eyes. Sometimes I use prescription allergy eye drops. I had stopped wearing any eye makeup, including mascara. But I finally found one I can tolerate now.
My doctor was afraid rosacea would scar my face, but I got lucky. Most people wouldn’t know how horrible my rosacea got by looking at me now, but my complexion isn’t the same, for sure.
Someone I know told me she has ocular rosacea, but I wasn’t diagnosed with that. I did have an excess amount of demodex on my skin when tested. We all have them, but for some reason, some people get unlucky. The rosacea was serious enough to cause swelling around my eye area, too, and I also needed oral steroids to get the swelling under control. I took oral antibiotics for a long time.
At first, my doc told me to put nothing on my skin that she didn’t prescribe. She told me to use all mineral sunscreen she sold in her office. It’s by EltaMD. I also can get it at DermStore online. (By the way, I’m in Louisiana, USA.) She also said that someday I might be able to work in other products. It took me three years to get to this stage. Finally, I wear makeup on occasion. Mostly, I wear tinted mineral sunscreen. Everything that goes on my face must be fragrance-free, alcohol free (except for the non-drying type), and oil-free (including free of essential oils.) I have oily skin, but I finally can moisturize - but only in certain areas. I find Niacinamide helps with inflammation, but I also read some people have a sensitivity to it. I can even put it around my eyes without issue.
Other things I do to help my skin are ~ avoid alcohol, spicy foods, heat, hot showers, and direct sun. When I cook, I try not to get near the oven door until some heat escapes. A simple task like removing food from a hot oven caused a recent flare up. It’s pretty crazy, I know. So, I have minocycline on hand, as needed. I’ve been pretty good without it, but I still use prescription topicals sometimes - not every day.
I totally agree that minocycline causes dizziness and vertigo. Trust me, I’ve had vertigo prior to all this, so I completely get it. When the dizziness got bad, my dermatologist let me switch from minocycline to doxycycline (which made a noticeable difference in the dizziness,) but minocycline works better in the pores - or so I was told.
I notice lately that I have been getting an occasion deep blemish under my skin on my chin. I wonder if it’s related to the fact that I stopped taking oral ERT in July. I tapered off completely. I didn’t have recent issues on my chin prior to that.
I hope this helps. Sorry it’s so lengthy.

Tammy Woods hello. wow, thankyou so much for replying , so kind. All the information was great and so helpful. I too cannot wear any makeup which sometimes makes me a little crazy, cause I feel I need some color sometimes but I don't . No frangrance, no oils here neither. And the flare-ups are crazy and I'm trying to pay attention. No hot showers , spicy foods and the summer was dreadful, no sun. I just got all this in April of this year. So I'm trying to pay attention to what causes the flare-ups also. I can actually feel them . The vertigo has been bad so the Dr. reduced my dosage of the minocycline. And I'm still figuring out cleansers even though the Dr said no cleansers , no moisturizers...but I have been using sunscreen... I sware anyone that is in my circle does not understand ....I have passed up alot of invitations since April to do things but I'm like oh well , this is real. And I want to feel better. I'm glad you're feeling better and your skin is looking great. Who would think at this age ,we be going through this. I am from New Jersey and it was great talking with you, well texting with you. I hope you have a great holiday. Thank you again.🌻 I also had the dermadex and get few breakouts on my chin then they go away.

Maria Genovese ~ Maria, I truly feel your pain. During the first months of my diagnosis, the only time I left my house was to go to my doctor or the pharmacy. I had no idea how all this would affect me emotionally. I kept a photo diary of my progress. Slow progress; it was awful. Even now, there are times I just turn beet red and have no earthly idea why or what brought it on. During those times, my face gets very hot. This started with me just a few years ago. Prior to that, I used to get compliments on my complexion. I’m very fair. My dermie said nearly all of her rosacea patients are very fair, blue-eyed, light hair-type caucasians. I suppose there are many exceptions.
I wish they would give you some topicals. My prescription cleanser used to be AVAR. Now it’s Plexion. And my Soolantra cream is basically Ivermectin. It took about three years for me to be able to use some of my own products. As much as I’m not in favor of paying shipping fees from the U.K., I still use what I listed above ~ Dr. Sam Bunting’s Flawless Cleanser and her Flawless Moisturizer. She ships to the US. I have tried multiple substitutes. Nothing comes close for me. My new favorite sunscreen is by Glo Skin Beauty. It’s called Solar Shade SPF 50. (They also have a mineral powder foundation that is pretty nice.) I get mine at DermStore. At first there’s a white cast, then it goes away. I found out in August that my skin can tolerate Chantecaille Future Skin Gel Foundation. FINALLY! I don’t wear it every day, but I can actually wear products from that company - as pricey as they are. I must double cleanse to get everything off, including the sunscreen which is tricky to remove. For that I use DHC Deep Cleansing oil. It will completely emulsify with water, then I follow with cleanser.
Maria, I hope you soon get to that point where there is some balance. Unless someone is experiencing this, they really cannot grasp it. Please let me know if you have questions. I promise it will get better with the help of a good dermatologist. All my best to you, and Happy Thanksgiving.🙏🏻

After i got my diagnosis, I was able to get my condition under control and keep it there for the most part of the last 15 years. Cera ve and la roche posay were essential. I have ETR rosacea and after much trial and error I am now able to have a complete anti-aging skincare routine. I worked on improving my skin barrier and slowly incorporating actives into my skincare routine. I started seeing improvements as soon as I focused on hydration and sealing that hydration by applying moisturizer on damp face and using a hat always. I’m now able to use vitamin C, Tretinoin, p50 1970 exfoliating toner, and niacinamide. The rest of my skincare is very mild CeraVe moisturizing cream, hada Labo hyaluronic acid bubble cleanser, and hada labo hyaluronic acid lotion. I get very little irritation now. Definitely takes time and patience to find the right products. Unfortunately glycolic acid is a no go for me in any concentration.

Hiii! Are clay masks (like that Vichy clay mask) good for rosacea?

Thanks for this helpful discussion! I developed rosacea a couple of years ago (in my 50s) and got some help from a dermatologist. I wash and moisturize with cetaphyl products and have a prescription cream with a low concentration of azelaic acid. This combo keeps the rosacea under control. Could you explain why azelaic acid works? And is it true that when skin tests are done, rosacea sufferers have far more skin mites present than non-rosacea people? Thank you 🙏🏻

    Hello Dr Lim,
I’ve just found your site, I’m really enjoying your videos, so informative- so Thank you 🙏🏻.
My beautiful daughter is 14 and has been suffering with Rosacea since about 10... she doesn’t have the Acne type so we think she is the type 2. The butterfly look, we taken her to our GP and blood tests came back normal- I’m not sure if this was for Lupus though? She was 13 then...and had been told that she can’t be given anything due to her age ( we are in the UK 🇬🇧). She’s so embarrassed about it and has been wearing foundation since she started high school (aged 12) we have bought her the best money can buy and she has a strict cleansing regime... she is very good bless her. Since lockdown she’s not worn any make up and doesn’t want too, she wants to be like her best friends who don’t wear make up... she’s back to school 27th August and she’s starting to get anxious again about her face. I’m so upset for her Dr Lim 😔 I wish I could have it for her.
She is now Gluten free, doesn’t eat red meat, just fish & chicken, boiled veg etc ( I’m veggie boarding on vegan) so she eats extremely well.
We think she needs seen by a Dermatologist as this is affecting her daily life and mental health. She is an extremely pretty girl, shy and doing very well at school- she has loved doing her schooling at home - the best in the year group. She was pleased to be told at home as she’d of hated being on show at school. She doesn’t like anyone looking at her- they do because she is stunning to look at.
Do you have any advice for us as to what we can do now please and I thought about the creams in this video - just worried for her now as she doesn’t want to have to wear so much make up- but she wants something to hide the redness? I just running out of ideas and I’m so sorry for her 😔.
    Please please can you help?
    Best wishes
    Beckie Kirby

I have oily skin and Love the Simple range. The gentle cleanser actually cleanses without leaving a film. I double cleanse if I've had makeup on then moisturise with the light Moisturiser. (In Winter I switch to the richer one.)
