How come 500 mg and 1000mg over one hour?? If 1000 mg can be given over 1 hour , 500 mg should be over half hour!!

No , any strength , it should be given in 1 hour only . Basically it’s the rate of infusion which causes the side effect .

Thank you Sir is AST to be done prior starting Vancomycin.

ideally should be done ... but usually not many do that... the point is start slow and watch for any reaction ... in any case it should not be given in less than 60 min
