Sorry but this isn't on the hardest difficulty. If it was, you'd see the 'karate' guys showing up every 10 seconds or so. They're not showing up at all, especially on the first level.
The game has 4 difficulty levels: easy, normal, difficult & very difficult. You can see dip switches at the end of the video where it displays "very difficult". Perhaps you played another ROM version with increased difficulty. The karate guys are running every several seconds since the stage 5.
