      I think it's a miscommunication, swelling and "guarding" would prevent a decisive physical examination. I believe the source told her that and her interpretation is that the swelling is preventing the imaging which is wrong.

    I wish my doctor was like him when i tore my ACL and MCL. The first two doctors i saw said i would be fine in a few days and wouldn't even arrange me for an MRI. I had to wait a month before they would send me to a specialist (they wouldn't see me until my doctor referred) and did an MRI...it turned out i had 2 tears on my ligaments. I couldn't walk for a year. I could have been healed in a few months if i could see a specialist right away.

    Great channel just recently subbed! I got a question.
    Is there any way I can actually strengthen my acl? I know there are exercises to strengthen the muscles around the knee, but is it possible to actually strengthen the acl ligament? Regardless I would assume that strengthen the muscles around the knee is good prevention against any acl tears.

    A partial tear can be repaired by weaving the donor ligament around the torn ACL. This creates a bridge on which the injured ACL can mend itself. It takes a skilled surgeon with great success to do this procedure. Most orthopods will simply remove the whole damaged ACL even if partially torn. This causes a long recovery time and a greater risk of a retear.

    I was kind of thinking this when I originally saw the replay. I was think a "non-contact" acl tear just because it wasn't a blow to the lateral aspect of the knee but it still looked like the moi was similar. Hopefully it's nothing to serious and he can recover quickly.

    Brian, You did not talk about the possibility of Kawhi's *signature New Balance shoe* being a contributing factor to his injury. In the video when Kawhi plants his right foot and gets bumped by Joe Ingles it looks like Kawhi's shoe held to the ground because the shoe has a posterior flare guard which stopped his shoe from rolling onto the posterior side of the shoe. Rather in the clip you the shoe sticking to the floor and see Kawhi's foot rotates inside his shoe. Did the shoe not rotating contribute to the internal rotation of the tibia leading to possible ACL trauma. Please advise you opinion. Thanks, Nick

    Doc Brian, your videos are extremely interesting. I have a question since I tore my ACL 2 years ago and I had surgery a month ago: from what I know, actually the ACL doesn't heal on itself because there isn't vascular supply in that area, so the only way is the surgery, is that correct?

    Doc, without numbers on hands I feel the rate of torn acl and Achilles mainly are steadily increasing in NBA: many point the finger to the number of games but it actually did not increase compared to previous decades, so there is no correlation. Can you do a video on this topic? Love your channel, except when eyeballs are popping out of site ;)

      Just shooting from the hip but players seem leaner, faster, and just generally more athletic so I’m guessing they have greater impact forces when cutting, jumping etc. which may be a contributing factor.

      They're doing more work outside of games (more work + less recovery = injury) and they're also getting better at putting the maximum amount of force possible through their joints. The offseason nowadays just means they aren't playing in NBA games, their workout regimen is probably even more intense than during the season. Shit just didn't work like that in the past. Also kids are burning the rubber on their tires way more than in the past, so people are getting to the pros with more wear-and-tear.

    Fwiw, Ramona did later say on the Jump that there was an initial MRI performed, although there was initial confusion about whether or not it happened at all. It does seem like there was an mri done, so that’s probably good news if they didn’t see an obvious rupture right off the bat. You don’t see other players with clear injuries have to wait 3+ days for an mri due to swelling, it is kinda odd.

      Regardless, his ACL has likely been compromised (already further than it has been previously) and his knee will never be the same again. He will need further load management and have increased risk of ACL rupture...

    Can you go into MCL sprain implications? It seems like this its more likely to be of a worse grade on the mcl than the ACL. What are prognosis’s for grade 1 vs 2 mcl sprains? And if we see a grade 2 Partial tear, how likely is surgery?

    Any update ?? I hear now that they are saying a partial tear, what kind of recovery needed for a partial tear ? We now a full tear is 9 months to a year out. Thanks

    When I was watching that game live I didn't think much of that injury. It happened right at the end of the game and I figured he just tweaked it and felt discomfort but it wouldn't be anything of importance. Crazy how something that looked so normal could be so devastating. I'm a Jazz fan but prayers to Kawhi. Hate to see anyone get injured.

    I had that exact same injury while stepping on a layup. I didn't feel any pain or anything. I actually thought it wasn't anything and tried to play through just to realize I didn't have the same explosiveness and stability. It's been 2 or 3 years and I didn't have surgery, just rest, medicine and therapy. But one thing I can attest is I've lost my hop. I couldn't jump as high and hard prior to that. And till this day I still can feel minor discomfort when I'm stretching/extending my leg.

    ESPN or any other sport channel should hire this guy to do injury reports and explanations. To educate more people and to get people talking about sports medicine in media. I love this channel, I just think these topics need more exposure.

      Cut the regular season games maybe? Like 10 games less?? Cause playoffs is just damn to long as well every round you get a best of 7 series. Just my opinion.

And yes cut the season to like 64 games a season. And get rid of the dumb play in tournament. But I know that won't happen bc POS Adam silver only cares about money.

    Yeah he's most likely done this post season....Especially because ACL was mention....if it were a slight MCL sprain he might have been able to come back like embid but ACL he's going to need surgery.

    I remember you made a vid about Klay waiting 2 yrs before coming back to the court after his ACL injury. My question is after his recent injury, should he wait one more season before coming back. thanks brother, keep up the good work.

    A thought. Swelling is a possibility, but not always a given. The ligaments intracapsular, extra synovial and nearly all ligaments, are very low blood perfusion. My ACL had almost no swelling at all at time of rupture.
    Also research is being published in the bjm about surgery being undecided if better than rehab for ACL's.. As more premier league foot ballers are opting out of surgery in favour of conservative treatment.
