This gives calories in and calories out a whole new smelly meaning. 😉 In all seriousness, these ideas show just how complicated human metabolism is, and why it is almost impossible to be exact when assessing calories in and calories out. There are so many variables that can determine how many calories are absorbed, and I surmise, based on the variability of one person's gut to another person's gut, that the amount of calories one person absorbs or excretes verses another person can vary quite a bit as well.
Some TMI but I suppose this is the right video for it, I'm carnivore for a few months now and I eliminate once or twice a week and in very small amounts. It's not constipation either, I feel great, no bloat or difficulty in that regard, I just generate very little waste now. What I've heard from other carnivores is that this happens because the small intestine are very efficient at breaking down and absorbing meat.
Right. My bowel movements were reduced when I did carnivore, and I've heard the same from others as well. That seems counter to losing calories in stool. I wonder if it's true for all ketogenic diets, or if certain variables need to be involved (like having fiber). It's also contrary to one of the supposed benefits of carnivore, little to no malabsorption. Kind of ironic that ketogenic diets may work so well specifically because they increase malabsorption.
@@matthewtrout1440 high insulin blocks the intestines from absorbing cholesterol and it ends up getting eliminated as waste. So having very little waste is probably a good sign that you are maintaining a very healthy uninflammatory vascular environment with an ideal average serum glucose level and no insulin spikes. I used to be very diabetic so any sign that I'm returning to insulin sensitivity is really encouraging in my case
There's almost no waste from fat, lean meat, fatty meat, and meaty fat. Once it is reduced to a liquid slurry, the absorption is extremely efficient. Plant material not so much, though the plant advocates who want their plants to resemble some form of red/bloody meat substitute often go out of their way to say undigested meat sits rotting in the colon for weeks. I wonder how it immediately got a get out of jail card and bypassed the small intestine to go right to the colon.

Nice job Nick. This is complicated data and I think most people would walk away from this with at least a partial understanding of how the gut microbiome may be changing in states of nutritional ketosis. Well done!
Might want to look at studies on what happens on a high fiber diet (as with a diet of whole plant foods). I mean, talk about calories consumed leaving the body (via the fiber which is pooped out, bringing with it some other foods consumed). A very effective weight loss protocol, among other things. Miche did one or two very good videos on this.
Perhaps not a good idea to eat large amounts of fibrous food that your body is not able to get nutrients from just to lose weight? Maybe eat less fibrous high nutrient food like red meat!
@@AuntyJack123 On the contrary - eating fiber is not just about weight loss by any means. WE don't break down fiber well, but our gut bacteria surely do, in a major way. This promotes a healthy gut biome, and hence healthy gut walls, such that every 10 grams of fiber consumed results in a 7% lower risk for colorectal cancer. Kinda the opposite of red meat. :)
@@tspicks4360 ok if you choose to believe it. Our bodies don't need fibre to survive and our gut microbiome adjusts to whatever our diet is. All you're doing is adding bulk to your poop. We gain no nutrients from fibre. Your studies have been bought and paid for by big food.
Makes in a pita to gain weight. I lost a lot of weight from UC and cannot gain it back. Went carnivore because it's the only thing that doesn't bother my UC riddled gut. Helps to supplement with bovine gelatin in light coffee a few times a day (or add gelatin to beef stock or broth).
Very nice. Would be helpful if all intake studies at population level and individual patient level(n=1) included energy out data. (Ie outcomes matter).
Makes me think of Stephen King's movie "Thinner." The protagonist was losing weight due to a curse put on him, so much so that he was going to die. Here's a sci-fi scenario: a scientific discovery for weight loss that accidentally becomes irreversible and contagious.
Contagious... hmmmm... have you seen the new South Park on Obesity? ... I'm not usually a fan of SP, but I had to watch it and it 'resonated' ... at least to some degree.
My n=1 shows I can eat far in excess of my BMR calories on carnivore and lose weight while this is not possible on a 'normal' diet. It is so obvious which is better for my metabolic health. I would be interested in a study that explored the impact on BMR and metabolic health.
try weightlifting with 2500 calories from oatmeal vs 2500 calories from meat and vegtables. yeah thats right, you cant do anything with carbs except generate fat and energy.
Nick, I've suspected this for decades, honest. Science has finally broken through the calories in calories out, and that one must eat many servings of foods produced by industries that know how to lobby...
Any tips if increased bile acid from high protein & fat meals leads to bile malabsorption + diarrhea? Maybe binders like fiber before eating my steak & eggs?
Is anyone else confused about the use of calorie? Calorie is a measure of photons. What is the substance that’s not being absorbed or further process that is being excreted?
Given that the body effectively never sees the unabsorbed fats (and they therefore don't participate in satiety regulation), isn't this effectively equivalent to eating less with extra steps? Oooh, my diet caused malabsorption, how great is that?
If the mice study was calory restricted, then wouldn't appetite just increase according to their weight loss? And what effect would that have if they were free feeding? Also, in the human study with the 11.6 lbs weight loss, was it "free eating" or calory restricted? I mean, if you "poop out" 500 kcal, then by how much does your appetite increase to make up for that? It's also known that any diet where you suddenly restrict food groups will lead to initial weight loss, not just keto. That could be another explanation of the human weight loss.
I guess one of the reasons is that fat digestion is capped by your bile juice production capability by the liver. In fact, since human digestion is largely enzymatic, every nutrient absorption is capped by the stored enzyme targeting it, except for simple ones like glucose, fructose, salt, etc. Never mind about gut bacteria, they are overrated
I’m wondering if this connection plays out with when you switch to a ketogenic diet hardcore like Carnivore, that the bad gut microbe Candida that loves feeding off sugar & starches, freaks out and starts starving & dying off and you eventually start losing most of your sweet cravings.
its a yeast. you loose your cravings because your insuline resistance falls, and the cell gets energy in a far mor effective way. so you cease to have cravings.

I remember as a kid i would eat 1-2 bags of nuts or peanuts and then see what looked like pieces of them in my poop. Now i saw that there is studies that say unprocessed nuts have like 30% less calories than what it says on the label because they are not digested fully and the fats are excreted in human poop. Is people eating less processed/absorbable calories also a reason why people are leaner on keto? Do you guys roast your nuts?
People don't chew enough these days because they are used to comfort food. Nuts are a big victim of this. I speculate this 30% would drop significantly by long chewing.

Well yes since I started the carnivore diet 41 years ago I have eaten up to 7 pounds of meat and a dozen eggs a day and never gained weight after age 25. The amount or poop I produce is small although daily. Interestingly decades ago there was a statement that fat people had different bacteria in poop than lean people. I know my poop has calories because along with pet waste and yard clippings it produces the gas I use for cooking!
7 pounds of meat a day is over 9000 kcal(literally) a day. That wouldn't be keto, perhaps it is the opposite of keto. You'd be very insulin insensitive or having a large amount of carbs together with it to suppress the satiety
Is the "advantage" in "metabolic advantage" a concept that has only made sense for the past 100 years of human existence? Perhaps the top 1% in previous centuries could afford to eat so much they worried about being fat, but didn't most people worry about having enough energy to get their work done? Wouldn't a "metabolic advantage" to them be to get the most amount of work done given the food they could afford?
You know, I always thought that the term "metabolic advantage" was a funny one. What we're doing is calling energy wastage an advantage, which is certainly not always an advantage. I think the terms "metabolic inefficiency" and "metabolic efficiency" are much better.
I think its more complicated than that. Even in that case getting fat is only advatageous up to a point. You want to stay fit, even when there is plenty of food and having an extra 10-20 kilo of weight on you is not helping.
they had a working homöostasis system. they didnt overeat. they only ate what they needed, and ramped up energy if they had to much. carbs threw a spanner in that system. people think hunger is the same as craving. they believe that sateity is when your stomach is full, regardless if you ate sawdust or potatoes. the whole concept is misleading from top to bottom. calories dont mean anything. they measue calories by burning some matter, and then maesure the ash and soot. but burning is not at all what happens in the body. its sience for toddlers.
Thanks for this video! But how meaningful are mouse studies for ketogenic diets? Don't know much about mice but I would guess that when feeding on grains, seeds, fruits etc. they are never phsyiologly in ketosis and never had adaptation to it. So aren't they studying more of a arbitrary state, that has no adaptive meaning.
Did you note the human data I shared in the video? Ketogenic diets have similar effects on functional aspects of human intestinal microbiota with similar changes in bile acids. Suggests to me conservation of mechanism.
What about supplementing with TUDCA? I did it for a few months and I couldn't separate out any effects from a Placebo effect and I was already on a high protein restricted calorie diet.
@@nicknorwitzPhD I've been doing a high protein, low Carb, Low fat diet and someone I follow was doing it for better "Energy" during workouts and as a digestive aid for fat burning. I did notice enhanced "Regularity" as I was still sorting out my Stomach Biome. I thought I noticed some increased energy and some better bathroom sessions but I couldn't tell the difference between that and the Placebo Effect. So after about six months, I stopped and really haven't missed it.
I don't know. I tried to reduce carbs intake only, but no avail. I didn't gain weight but didn't lose either. Only when I started additionally count in/out I started to lose weight. Probably it's my individual peculiarity.

Doctor Nick, could you do a video about constipation and keto diet? All the times I quitted goin' keto was because of this. Thanks, doc. Cheers from Brazil!
Thinking evolutionarily, doesn’t this suggest that meat and fat are too high calorie for us, so we excrete it. And that we are starving for glucose which is why glucose sensitivity improves.
Evolutionarily, there were no plates on a table to be finished. We wouldn't overeat, and would save the extra foods for later, so we wouldn't worried about hunting every day. Also, evolutionarily, glucose dense foods are extremely rare in the wild, and glucose sensitivity should be always high.
@@onlyonecai Very true; archaeo-medicine has established that before settled farming began (c.10,000 BCE), mTOR easily processed the relatively few carbs available to us. Once we "turned grasses into grains", insulin became dominant, to cope with the huge influx of dietary carbs. Archaeologists introduced a new word for the bulky human body shape shown in art work of c.7,000 BCE: "steatopygous". I think, today, we'd call that 'metabolic syndrome'.
The reason peoole were skinnier in the past was because they ate more meat, less sugar, and more saturated fat that feeds gut bacteria that consumes and holds more energy than bacteria fed by sugars.
Interesting hypothesis. Care to elaborate? PMIDs appreciated. And - no - just because your profile picture is Gandalf does not mean I can automatically trust your wisdom ;)
I’ve watched many videos from highly qualified doctors agreeing with Matthew and even more videos of anecdotal evidence from average people who have cured or lessened a vast assortment of ailments . I personally agree that the carnivore diet is pretty incredible for regaining health.
@@nicknorwitzPhD its different kinds of fermentation. they have different results, yeast + sugar = alcohol and waste and gas. yeast, salt and cellulose = short fatty chain acids no gas. aka thats what fast food people lack. so they have hard times to swith from carb burning to fat burning. normal people can use these short fatty acids for energy and more.

I just don’t think we should use calories it’s that simple. Just know how much of what you eat if you measure and adjust that amount of food for your specific goals
I have been enjoying sh!ts and giggles now for 2 years, thanks to carnivore lifestyle. I used to experience horrific sh!ts and tears (angry hemorrhoids, anal fissures, bleeding and excruciating pain). Weight loss and T2D reversal were an added bonus.
Kelly Hogan talks about how she was able to eat double the calories on carnivore than she can on the SAD (where she was gaining weight on 1200 calories). The fat fell off on double the calories. Dr. Anthony Chaffee also talks about this happening to him.
Is there similar mechanisms to why very low carb high fat and very low fat high carb diets (assuming whole foods) share a similar outcome of weight loss?
I'm not sure they do... I can guess what trials you may be referring to... but please drop specific references... I think there may be "quirks" worth highlighting.

I understand that industrial western culture has a diet and lifestyle problem but it really is a misuse of the term “advantage” when the effect is that a lower percentage of calories in the diet are successfully metabolized. This is, in fact less efficient 😂
@@nicknorwitzPhD I do get your context. As an ultra endurance athlete who enjoys healthy metabolism of all of the macronutrients it just struck me with horror to get less out of my fuel. The science IS FASCINATING! Thank you for putting this out, I’d probably never see it otherwise
I don’t see how this detracts from the concept of CICO/ energy balance. You don’t make this claim in the video (or if you did I missed it ) , but it seems to be a common take away in the comment section. At any rate I remain unconvinced of any special advantage keto may have over other diets. But this was an interesting video , I also enjoyed your interview with Simon Hill 👍
CICO always applies, its just physics. But this shows again that its not just about how many calories you eat but which foods you eat. Because the other things in foods change the metabolism and appearently also the absorbtion of calories long-term. What most people mean by CICO is tracking dietary calories / eating less calories and expending more calories with exercise. But why even track calories if a (dietary) calorie isnt a calorie?
@@wtsuser i think different groups don’t use the same definitions of CICO when they’re promoting its importance / dismissing it entirely. I think those who dismiss it most commonly simply disagree with certain conclusions drawn from it rather than the principle itself (although those people exist too). But the same can be said about looking at mechanisms such as what’s discussed in this video and the conclusions some people may draw from it. Does this mean people should all go to keto? Why? We know people have success all sorts of diets. I agree that it always applies , but there seems to be a growing amount of people who deny even this. They tend to be in the carnivore crowd mostly , but there are some in other groups - particularly low carb groups. I dont think CICO means that you MUST count calories , although some people certainly do find success that way. I think the only thing that it says is that in order to lose fat over a given time period , you must be in a caloric deficit over that time period. How people choose to do that is up to them , as long as it’s something they can adhere to long term. Learning more about the nuances of the human metabolism will probably help in most cases when people are choosing how to go about this - but perhaps there is risk of ‘paralysis via analysis’ here too. 🤷♂️
@@nicknorwitzPhD I’ve seen the video, thanks for the reminder. I agree that it is possible for the laws of thermodynamics to hold without calories being directly causal for weight loss. However, I simply disagree with the idea that calories are not causal. Specifically, I believe that an energy deficit is indeed causal when it comes to fat loss. In other words. Sure that could be the case , I’m just not convinced it is.
although i think keto diet is more forgiving than standard diets, you can certainly get fat with any diet if you over consume calories. ive been on a keto diet for 8 years, always maintained very lean healthy weight and still do; but where i moved has terrible air quality and i dont ride my bike every day and only weight lift 4 days a week because of my work schedule; so yeah ive been most certainly getting softer in the last year. Not overweight by any means but noticeably different
Air pollution could definitely be a factor which is seen to affect insulin sensitivity. Exercises don't really have an effect on insulin sensitivity, and your body will compensate the appetite accordingly which means you will just get a bit hungrier after exercises. I only do about 20 mins of weight lifting per week, no other exercise besides walking to my work. I easily maintain 10% body fat all time.

Plant material not so much, though the plant advocates who want their plants to resemble some form of red/bloody meat substitute often go out of their way to say undigested meat sits rotting in the colon for weeks. I wonder how it immediately got a get out of jail card and bypassed the small intestine to go right to the colon.

Welcome :)
Couldn't this potentially lead to LPS producing bacteria being better fed leading to a negative effect? Also maybe taking bile acid salts could help?!
still a bit confused.
i missed exactly WHAT in a ketogenic diet is causing this.
is it......fiber?
is it inulins that promote good bacteria?
Also how is nutritient malabsorbtion a good thing 🤨
Interesting that topiramate is used as an anti-epileptic treatment -- like the medical ketogenic diets.

True “preaching “ is persuasive, not “forcing.”

How might this affect those who have had their gall bladders removed?
That’s not true. Having fat helps you live longer without food, in starvation periods.
Advantage does require content. I think you know what context I'm operating in.
they had a working homöostasis system. they didnt overeat. they only ate what they needed, and ramped up energy if they had to much. carbs threw a spanner in that system. people think hunger is the same as craving. they believe that sateity is when your stomach is full, regardless if you ate sawdust or potatoes. the whole concept is misleading from top to bottom. calories dont mean anything. they measue calories by burning some matter, and then maesure the ash and soot. but burning is not at all what happens in the body. its sience for toddlers.
Nick, what about peeing out ketones? Maybe that is more significant?
What about supplementing with TUDCA? I did it for a few months and I couldn't separate out any effects from a Placebo effect and I was already on a high protein restricted calorie diet.
Bile works on fat not protein.
What effects did you notice? And why did you start supplementing in the first place?
@@nicknorwitzPhD I've been doing a high protein, low Carb, Low fat diet and someone I follow was doing it for better "Energy" during workouts and as a digestive aid for fat burning. I did notice enhanced "Regularity" as I was still sorting out my Stomach Biome. I thought I noticed some increased energy and some better bathroom sessions but I couldn't tell the difference between that and the Placebo Effect. So after about six months, I stopped and really haven't missed it.
Would this apply for carnivore diet?
I don't know. I tried to reduce carbs intake only, but no avail. I didn't gain weight but didn't lose either. Only when I started additionally count in/out I started to lose weight. Probably it's my individual peculiarity.

Does anyone know how people fare on ketogenic diets for seizure control after many decades?

The reason peoole were skinnier in the past was because they ate more meat, less sugar, and more saturated fat that feeds gut bacteria that consumes and holds more energy than bacteria fed by sugars.

although i think keto diet is more forgiving than standard diets, you can certainly get fat with any diet if you over consume calories. ive been on a keto diet for 8 years, always maintained very lean healthy weight and still do; but where i moved has terrible air quality and i dont ride my bike every day and only weight lift 4 days a week because of my work schedule; so yeah ive been most certainly getting softer in the last year.
