I don't think Acheron was fully tested by the community when making this statement of her E4 ability. Let's break it down and understand why this ability is more impactful than it's given credit for!

I think it's working as intended. The reason why it works when an enemy summons mobs vs when they spawn is in her talent. She only gains slashed dreams when an ability is used. The enemy uses an ability and spawns mobs and acheron passively applies a debuff therefore in that ability cycle she gains a stack. The start of the wave is not an ability. This can probably be tested if there's any other character that gives debuffs when an enemy spawns.
Either way e4 is better than immediatelly apparent but it's still not really that good.

It being "good" or "bad" is irrelevant. The knowledge and reaching a definitive understanding of the E4 logic is always useful.
As far as additional testing, one of Black Swan's Bonus Abilities inflicts Arcana when an enemy enters battle and that, under the logic of Acheron's Talent, should be counted as a unit using their ability to inflict a debuff yet that Bonus Ability isn't triggering the stacks either. I already checked since I have her.
"When [any] unit inflicts debuffs on an enemy target while using their ability" is where the conflict of logic resides.
The issue is in how the game defines "using an ability" because an ability is being used, just passively. Again, it could be balancing logic.

@@RockmanXelnia Passive doesn't count as ability usage?? Never did, besides you should be able to see an ability on the action bar.
Do you have an account where acheron isn't e4 that you could test this on. If you have e4 then even if black swan's debuff triggers it won't grant an additional stack as there's a limit of 1 stack per ability usage.
Also the good or bad was a side note.
Also again enemies entering battle probably never will give stacks, only an enemy summoning more enemies.
If you want to properly test you'll need an acheron that does not have e4 and black swan.
Take them to one of these enemies and have them spawn extras, see if it gives you slashed dream.
If not it's probably a bug and it'll get fixed.
If you're even more curious you can probably open a support ticket and have hoyo clarify for you.
