From what I can tell, Alluka (his actual sister) uses feminine pronouns while Nanika (the Something she turns into to grant wishes) uses masculine pronouns. Hope this helps :)
Hxh fans won't admit this to you. But Alluka is horrible writing. An asspull, a deus ex machina. This arc is entertaining but alluka is a genuinely awful character. Imagine watching the whole show and then finding out killua has a wish genie sister that he can personally use with 0 consequences lmao. Actually a joke. That being said, it's still a fantastic story/show and togashi is a great writer, but fuck me the guy fumbled it BIG TIME here. The story writing was fantastic to this point but I guess even the best writers can fuck up sometimes. Luckily, the current arc in the manga picks up and has top tier writing
