I did the math, there's about 1600 possible combinations between these 20 families. The assumed constrains being,
1. 20 families each, and each couple having 2 children.
2. No gay marriage.
3. maximum of 5 years of age gap.
4. Only peeps between the ages of 25-40 having kids.
5. Assuming equal gender distribution.
taking in these constrains, there would be about 40 males and 40 females in the pool each generation. Which would lead to 1600 possible combinations.
Given these estimations it would take 16 to 32 generations for inbreeding to be a problem. Which is 1000 years give or take, and that's assuming that no other genome enters the pool. which is quite unlikely! So I doubt it's a problem.

It’s 20 families total so it’s only 20 male and 20 female, asuming they can’t marry themselves or anyone they share a common ancestor with you get only 380 initial combinations.
Since the goal is to reduce the shared ancestors you also want the pairs of siblings to marry other pairs of siblings (i.e. brother A marries sister B, and sister A with brother B. This is better because siblings AB are now only restricted from marrying siblings BA as opposed of CA and BD as they would otherwise, let’s assume then that everyone does this.
This means that every time they marry they double the common ancestor of all their children.
1st gen you have family A marries B
2nd gen family AB marries CD
3rd gen family ABCD marries EFGH
4th gen Family ABCDEFGH marries IJKLMNOP
For gen 5 you need 32 ancestors you only have 20, so incest is inevitable to some degree.
And remember, this is playing optimally. If they have 3 children, or keep marrying only people who nobody else in their family has married they can cause an incest problem in as little as 3 generations.

It isn’t just those 20 families they marry. That’s kind of an exaggeration. But they do have to marry within the upper echelon of their society. A lot of them also have to marry for business reasons ie. when one company wants to expand into another industry and need a partner kind of thing. Funny enough l, even actresses are looked down upon when they marry into those families, no matter how popular they are. That’s why a lot of times, when an actress marries into one of those families, they usually have to give up their careers.
The best way to think about those families is to compare them to landed nobles of old. That’s the best analogy for them I think. It’s very interesting though but also quite insidious when you really think deeply on it, especially for foreigners that didn’t grow up with that system
