This video changed my life, I HAVE AN SI ISSUE! Not a sciatic nerve issue! Finally! I have been told by doctors that i should work on losing weight and getting my back stronger. I did, i lost 100 lbs, by lifting and eating better and still had back pain. Found this video and two days later i went for a 1 mile run with my son and no pain in my back. Thanks, your diagnosis for SI joint pain, especially the breathing technique, is one of the most helpful I’ve seen on TH-cam-and trust me, I’ve watched a lot! For those dealing with SI joint pain, here are a couple of tips from my personal experience (I’m no expert, but these helped me): 1. Reason - Unequal distribution of body weight can be a cause of SI joint pain. This often happens due to weakness on one side of your lower body, from your hips to your legs (for me, it was the right side). When we do activities like standing or sitting, the weaker side doesn’t handle the load as well, causing irritation in the SI joint. You might notice yourself leaning toward your stronger side, shifting weight off the weaker side. The goal should be to strengthen the weaker side, so if possible, hit the gym and work on your lower body, focusing especially on any weaker areas. 2. For those who can’t hit the gym, there are a couple of things that helped me instantly. i) We Muslims sit in a specific way during prayer (Salah/namaz), u can check out the video how we offer salah and try sitting like that, but make sure to put more weight on the side that’s causing pain. This will help that side get stronger over time. ii) Lunges also help, and there’s a TH-camr named Athlean-X who has a video called “How to Fix Your Lower Back” that really helped me, so give it a watch. 3. Sleeping in correct position, maybe on one side sleep with cushion between your legs. And yes keep yourself hydrated and potassium intake through food (banana, cocomut water , etc) if you dont, suggested by local physiotherapist as it will help in avoiding cramps. But if the pain still persists consult a medical profession for right diagnosis and resolution.

I'm left with Arachnoiditis and a disc bulge, 2 level fusion and now it's 4 discs not just the two original and then throw in the hip and leg pain (Sciatic). 15 years worth, dealing with incompetance and insurance companies that don't really give a crap. That SI joint sounds about right for me. This started 2 days ago and was wondering what I did to get this. I lift weights so I thought it was too much of that. After really hearing this and that I need to strengthen it, it clicked. I've been working on my legs, but neglected my glutes, so I will definitely work on that tonight and give any updates...
Well he is right.. About the stretching.. As I keep trying and making it worse.. You just keep trying to get rid of it but pushing and crashing is my problem... It's a disc and sciatica..
I have pain almost 2 years in my lower back and i went to 6-7 doctors and did all kinds of treatments no one knows what it is and my pain is same like on day 1. I have pain while banding and picking up things and its killing my life. But i did this test and on 4:00 i have pain in my lower back. So on 3:30 i dont have pain only stretch in my hamstrings and calves but on 4:00 i have pain in my lower back. Does that means that my pain is caused by sciatic nerve being pinched somewhere? Let me tell you my story haha. I had burnout, insufficient levels of vitamin D, I was very tired, and anxious, my muscles were not recovering, and I started having joint pain. I was working out at the gym during that time, and at some point, I started feeling pain in my lower back that didn't go away for a few days. I waited a month to see how I felt and went to the orthopedist when I noticed it didn't improve. My symptoms seemed muscular because I felt stiffness in the lower back, but I had pain when standing for too long, walking, bending over, and trying to lift something. So, I had an MRI: minimal disc protrusion on the left side. I did physical therapy and now that I am not as sensitive, I have moved on to a program to become stronger and regain my mobility, just like you said, stretching exercises are temporary, and good for the initial phase, but then we need to build up (I learned these things from lowbackability program
Anyway, I adore you and thank you for this information; unfortunately, most doctors, at least those I've dealt with, are outdated and have scared me with all their recommendations. They have discouraged me, saying that I won't be able to move my back and that I should avoid specific exercises to "protect" my spine. As a result, I feel scared and guilty when I try to do certain exercises on my own without any support, as I've been told that I shouldn't do them. I'm really grateful to have found you.
Thanks for sharing your story! I know the medical system can be frustrating. The main thing to remember is that pain is always multifactorial. Sure, certain muscles or joints may be part of the cause of pain, but we need to zoom out and look at the bigger picture. Make the whole body strong, take care of stress/fear, eat healthy, and get good sleep. Zoom out. I break this approach down in great detail in the masterclass if you haven't seen it yet. Just a note from someone who’s been through perimenopause and menopause, and it’s been a massage therapist for 30 years- flexibility without proper support, without proper balanced strength, can leave you vulnerable to injury -so stretching alone, especially as women enter their 40s and 50s, (but then I guess men catch up eventually) can be a problem! Like what he’s teaching here, balanced strength IS a plus. We also lose tone (strength) faster each decade, and collagen gets looser- the whole shebang is usually much better off focusing on strength and not so much the stretching. (Woah- so many typos here- sorry! just rewrote)
Yep we strongly advocate for strength development which improves bone density and muscle mass, and we advocate against excessive stretching especially in people who are hypermobile. Controlled weightlifting is one of the single best things anyone can do in their lives, and this is especially important in women to reduce bone and muscle mass losses later in life.
What to do if I have more than one of these problems? Except that disc problem, I have si joint pain, sciatica mostly. Sometimes I get hip joint pain too. (I fell on my hips 2times during my early teanage years. My first sciatica problem occurred on my left leg when I was 15. It was because of spinal stenosis. With medication and exercises it cured. But now when I am 24, it occurred again, but this time on my right leg/right side of my lower back and hip. And after watching this video I can tell it's not only sciatica, it's si joint pain too.

I get sharp shots of pain like you explain from the potential discs issue, but it’s in certain postures as well as transitions. Most of the time I can move around really easily, but after sitting or laying, and then moving again, I’ll sometimes get a jolt of pain really low on my back and slightly to the left (SI area) and have had a few spasms that have put me on the floor from standing - then scrambling around for a position that alleviates the pain. I do get more minor jolts just from putting shoes on, or a slight turn, or even an unexpected drop step lower on uneven ground as well. My instinct, and after watching things like this video, suggests it’s the SI from bad posture (desk chair all day and sitting cross legged) causing posterior pelvic tilt. I’ve had some success from stretching (I have extremely tight hamstrings early in the day and some hip flexor movements) but as you say, it feels more like strengthening is needed but I don’t know what do next. Any advice would be greatly appreciated as it’s stopping me from playing with my young kids out of fear of pain or a spasm. Thank you.

@@samkafin2666 It’s actually lifted quite a bit for me recently due to a couple of changes. One key one was changing my desk chair to have proper lumbar support, plus a desk that I can switch from sitting to standing. Outside of that, I’ve changed my diet and lost a bit of weight. My assumption is the weight loss will always help, but some more nutritious whole foods can do wonders also.

Great video. Excellent tests. My SI joint pain feels like something catches or is stuck. Occasionally I feel it click pain free when I get up from pushups etc but if it catches I’m in pain. Not sure what’s the sensitivity is with that and how to strengthen. Subscribed!!!
I got numbness in the feet progressing up the legs whenever I walk or exercise or bend forward. Bending forward or sitting down in a comfy chair or in the car seat immediately feels like a thick warm blanket is covering my feet and legs. Only arching backward doesnt make the numbness worse. Even sleeping in a fetal position (which is my comfort zone) makes the numbness worse. The lower back pain is a annoying but insignificant, the numbness is what I find most worrying. It utterly weirds me out. I can deal with pain but what can I do about loss of sensitivity? The other day I walked a few miles and back home I noticed I had a bloody cut in a toe without noticing it. Tried rest, short walks, long walks, swimming, all kinds of exercises, everything I did except the McKenzie stretch seemed to make the numbness worse.

I have severe weakness in my right calf muscles and the MRI shows that my SI nerve is compressed at L5-S1 level. I also have numbness in my small toe and the outside of the right foot and heel. I am in pain for a month and nothing seems to work. Even my physiotherapist said I should go for surgery.

My lower back in the middle will go through bouts of major issues that last from 2 days to 2 weeks. I feel like it's my lower spine combined with muscle spasms. I use a tennis ball to push in on the area that hurts and it helps sometimes but It's hard to walk. I've had 3 hernias and I need exercises that will help build muscle without strain.
What If you lift your left leg and bend towards your chest , but when you let it back down it pops like bone to bone is clicking , but does not happen on the other side? I also had a stretch rubber band feeling when my doctor bent that leg , but it didn't hurt more just felt like a rubber band stretching?

My partner has dull aching pain in lower back through tailbone region, that radiates to both sides of the upper buttocks. Gets worse throughout the day. No numbness or acute pain down the leg. Sounds like it could be SI joint on both sides? She’s tried strengthening exercises but it seemed to irritate it more? Been struggling with this for close to two years. Anyone have recommendations? I'm 29 and have had all of these symptoms since I was 20 I can barely sleep at night I can't get comfortable anyway I sit or lay down and trust me I've tried I had insurance went through to the doc for about a year and they couldn't figure out shit so I said fuck that and now I'm here the only way I feel not in pain is standing or walking
What is it if it’s a punctual pain on the standing leg side when back kicking (ushiro geri) in the boney sacroiliac joint area? Did 2 corticosteroids in the SI joint to no avail, did no 0 sports for 3 months to no avail. When I come back to kicking it comes back immediately and gets the same worse in a few weeks. Also cannot squat to the floor heels down anymore cuz gives immediate back pain and gives sciatica leg block if I continue doing it ignoring the pain for few days.So it sucks, I can no longer back kick, squat below 90% and run, which btw is mildly painful to the lower back witch each leap and intensifies with each day I run. Been to 3 doctors to no avail… I have 36 yrs if it helps… basically what ai believe now there is no cure but just the body is broken down and that’s it end of story for training karate or athletics or weights… is bye bye. Basically I’m like a 70yr old guy at 36 due to my 5 days/week training since I was 20. That is my sad conclusion anyway….

I have pressure just above the butt crack, some pain close to the sides of butt crack, deep in the middle of the glutes, sometimes pain in the hips (big boney parts in back) and very tight hamstring… no mri, pt thinks disc. Hurts like pressure to sit. Any ideas?

In one of your videos, I heard you say you're not a fan of stretching. Or you don't believe in it. but from my personal experience growing up in life and people that I've seen. They stretch every morning and night twice a day and are extremely flexible. David Goggins runs ultra marathons and stretches 2 hours a day. My friend Rob does yoga consistently and can wrap his legs around his head. He's 48 years old. I've seen old women who are extremely flexible stretch multiple times a day well into their 90s. So why is it you don't recommend stretching?

For what it's worth stretching is the only thing that makes my back pain better, I'm sure drugs would make me care less but I don't have time for that. However, I pair mobility exercises with stability exercises. And my lower back pain is coming from my hips getting too stiff. I don't think I've actually got anything wrong with my back but by God, that muscle in the back getting tight had more than once, taken my breath away. I do know the more I move into movements that hurt, it hurts less. And I don't have any tingling or shooting pain. Edited to add: My SI joint is pissed. I'm on the right track with mobility and stability exercises.
On my right side my leg is short and was told that my pelvic was working hard over the past years it got worse it starts from my back to my thighs to my knee cap😢 my whole entire leg goes num when walking or standing for a long period of time. But I've to bend over to relieve pressure sometimes if im with my family when we out in public

Basically gradual progression back into normal strength exercise. Calm stuff down, build stuff back up! I likely won't make a video specifically on labral tear rehab since at this time my specialty and my focus online is back pain and sciatica.

Please help as I cannot pin point the exact cause of my pain (I have it around for 1 year). My symptom started when a kid sat on my back unexpectedly while I was doing push-ups. The pain used to be short-sharp pain that lasted less than a second and disappear. Can you explain to me why you say that core bracing is not a long term strategy? for example, if you are flexion intolerant and bending forward, or creating shear are your pain triggers, wouldn't promoting spinal flexibility be detrimental in the long term because you continue to push your pain triggers? i just cant see how anything but spinal stiffiness and core bracing is beneficial if you have an discogenic injury. thanks

That SI joint sounds about right for me. This started 2 days ago and was wondering what I did to get this. I lift weights so I thought it was too much of that. After really hearing this and that I need to strengthen it, it clicked. I've been working on my legs, but neglected my glutes, so I will definitely work on that tonight and give any updates...

I have pain almost 2 years in my lower back and i went to 6-7 doctors and did all kinds of treatments no one knows what it is and my pain is same like on day 1. I have pain while banding and picking up things and its killing my life.
But i did this test and on 4:00 i have pain in my lower back. So on 3:30 i dont have pain only stretch in my hamstrings and calves but on 4:00 i have pain in my lower back. Does that means that my pain is caused by sciatic nerve being pinched somewhere? Thank you

Let me tell you my story haha. I had burnout, insufficient levels of vitamin D, I was very tired, and anxious, my muscles were not recovering, and I started having joint pain. I was working out at the gym during that time, and at some point, I started feeling pain in my lower back that didn't go away for a few days. I waited a month to see how I felt and went to the orthopedist when I noticed it didn't improve. My symptoms seemed muscular because I felt stiffness in the lower back, but I had pain when standing for too long, walking, bending over, and trying to lift something. So, I had an MRI: minimal disc protrusion on the left side. I did physical therapy and now that I am not as sensitive, I have moved on to a program to become stronger and regain my mobility, just like you said, stretching exercises are temporary, and good for the initial phase, but then we need to build up (I learned these things from lowbackability program
Anyway, I adore you and thank you for this information; unfortunately, most doctors, at least those I've dealt with, are outdated and have scared me with all their recommendations. They have discouraged me, saying that I won't be able to move my back and that I should avoid specific exercises to "protect" my spine. As a result, I feel scared and guilty when I try to do certain exercises on my own without any support, as I've been told that I shouldn't do them. I'm really grateful to have found you.
Thanks for sharing your story! I know the medical system can be frustrating. The main thing to remember is that pain is always multifactorial. Sure, certain muscles or joints may be part of the cause of pain, but we need to zoom out and look at the bigger picture. Make the whole body strong, take care of stress/fear, eat healthy, and get good sleep. Zoom out. I break this approach down in great detail in the masterclass if you haven't seen it yet. Just a note from someone who’s been through perimenopause and menopause, and it’s been a massage therapist for 30 years- flexibility without proper support, without proper balanced strength, can leave you vulnerable to injury -so stretching alone, especially as women enter their 40s and 50s, (but then I guess men catch up eventually) can be a problem! Like what he’s teaching here, balanced strength IS a plus. We also lose tone (strength) faster each decade, and collagen gets looser- the whole shebang is usually much better off focusing on strength and not so much the stretching. (Woah- so many typos here- sorry! just rewrote)
Yep we strongly advocate for strength development which improves bone density and muscle mass, and we advocate against excessive stretching especially in people who are hypermobile. Controlled weightlifting is one of the single best things anyone can do in their lives, and this is especially important in women to reduce bone and muscle mass losses later in life.

Except that disc problem, I have si joint pain, sciatica mostly. Sometimes I get hip joint pain too.
(I fell on my hips 2times during my early teanage years. My first sciatica problem occurred on my left leg when I was 15. It was because of spinal stenosis. With medication and exercises it cured. But now when I am 24, it occurred again, but this time on my right leg/right side of my lower back and hip. And after watching this video I can tell it's not only sciatica, it's si joint pain too.

I get sharp shots of pain like you explain from the potential discs issue, but it’s in certain postures as well as transitions. Most of the time I can move around really easily, but after sitting or laying, and then moving again, I’ll sometimes get a jolt of pain really low on my back and slightly to the left (SI area) and have had a few spasms that have put me on the floor from standing - then scrambling around for a position that alleviates the pain. I do get more minor jolts just from putting shoes on, or a slight turn, or even an unexpected drop step lower on uneven ground as well.
My instinct, and after watching things like this video, suggests it’s the SI from bad posture (desk chair all day and sitting cross legged) causing posterior pelvic tilt. I’ve had some success from stretching (I have extremely tight hamstrings early in the day and some hip flexor movements) but as you say, it feels more like strengthening is needed but I don’t know what do next.
Any advice would be greatly appreciated as it’s stopping me from playing with my young kids out of fear of pain or a spasm.

@@samkafin2666 It’s actually lifted quite a bit for me recently due to a couple of changes. One key one was changing my desk chair to have proper lumbar support, plus a desk that I can switch from sitting to standing.
Outside of that, I’ve changed my diet and lost a bit of weight. My assumption is the weight loss will always help, but some more nutritious whole foods can do wonders also.

I got numbness in the feet progressing up the legs whenever I walk or exercise or bend forward. Bending forward or sitting down in a comfy chair or in the car seat immediately feels like a thick warm blanket is covering my feet and legs. Only arching backward doesnt make the numbness worse. Even sleeping in a fetal position (which is my comfort zone) makes the numbness worse. The lower back pain is a annoying but insignificant, the numbness is what I find most worrying. It utterly weirds me out. I can deal with pain but what can I do about loss of sensitivity? The other day I walked a few miles and back home I noticed I had a bloody cut in a toe without noticing it. Tried rest, short walks, long walks, swimming, all kinds of exercises, everything I did except the McKenzie stretch seemed to make the numbness worse.

I have a SI Nutation on the right axis
I have severe weakness in my right calf muscles and the MRI shows that my SI nerve is compressed at L5-S1 level. I also have numbness in my small toe and the outside of the right foot and heel. I am in pain for a month and nothing seems to work. Even my physiotherapist said I should go for surgery.

For me, the offending muscle was the gluteus medius. I needed to release the trigger points in the origin of that muscle.
My lower back in the middle will go through bouts of major issues that last from 2 days to 2 weeks. I feel like it's my lower spine combined with muscle spasms. I use a tennis ball to push in on the area that hurts and it helps sometimes but It's hard to walk. I've had 3 hernias and I need exercises that will help build muscle without strain.
What If you lift your left leg and bend towards your chest , but when you let it back down it pops like bone to bone is clicking , but does not happen on the other side? I also had a stretch rubber band feeling when my doctor bent that leg , but it didn't hurt more just felt like a rubber band stretching?

My partner has dull aching pain in lower back through tailbone region, that radiates to both sides of the upper buttocks. Gets worse throughout the day. No numbness or acute pain down the leg. Sounds like it could be SI joint on both sides? She’s tried strengthening exercises but it seemed to irritate it more? Been struggling with this for close to two years. Anyone have recommendations? Does pain get worse before it gets better?

What is it if it’s a punctual pain on the standing leg side when back kicking (ushiro geri) in the boney sacroiliac joint area? Did 2 corticosteroids in the SI joint to no avail, did no 0 sports for 3 months to no avail. When I come back to kicking it comes back immediately and gets the same worse in a few weeks. Also cannot squat to the floor heels down anymore cuz gives immediate back pain and gives sciatica leg block if I continue doing it ignoring the pain for few days.So it sucks, I can no longer back kick, squat below 90% and run, which btw is mildly painful to the lower back witch each leap and intensifies with each day I run. Been to 3 doctors to no avail… I have 36 yrs if it helps… basically what ai believe now there is no cure but just the body is broken down and that’s it end of story for training karate or athletics or weights… is bye bye. Basically I’m like a 70yr old guy at 36 due to my 5 days/week training since I was 20. That is my sad conclusion anyway….

In one of your videos, I heard you say you're not a fan of stretching. Or you don't believe in it. but from my personal experience growing up in life and people that I've seen. They stretch every morning and night twice a day and are extremely flexible. David Goggins runs ultra marathons and stretches 2 hours a day. My friend Rob does yoga consistently and can wrap his legs around his head. He's 48 years old. I've seen old women who are extremely flexible stretch multiple times a day well into their 90s. So why is it you don't recommend stretching?

Short story, any benefit from stretching can be had from full range of motion strength training, which adds HUGE value to health compared with stretching alone. For what it's worth stretching is the only thing that makes my back pain better, I'm sure drugs would make me care less but I don't have time for that. However, I pair mobility exercises with stability exercises. And my lower back pain is coming from my hips getting too stiff. I don't think I've actually got anything wrong with my back but by God, that muscle in the back getting tight had more than once, taken my breath away. I do know the more I move into movements that hurt, it hurts less. And I don't have any tingling or shooting pain.
Edited to add: My SI joint is pissed. I'm on the right track with mobility and stability exercises.

I can't do the exercise as too much pain in getting up. Or sitting. Or standing up. Or walking. What do I do to stop this excruciating pain?
I feel severe lower back pain when trying to stand or walk fully upright. The pain worsens the more I bend backward, improving from hunching forward. The pain is also severe when trying to stand back up after bending down to lift something. It is a very sharp pain right between my pelvic bones on my spine. I feel almost no lower back pain when sitting or laying down. I know rest is important however I also know not enough activity also weakens the supporting muscles.

Basically gradual progression back into normal strength exercise. Calm stuff down, build stuff back up! I likely won't make a video specifically on labral tear rehab since at this time my specialty and my focus online is back pain and sciatica.

Please help as I cannot pin point the exact cause of my pain (I have it around for 1 year). My symptom started when a kid sat on my back unexpectedly while I was doing push-ups. The pain used to be short-sharp pain that lasted less than a second and disappear. Now there is major relief but the pain still exists when I do the following tasks mentioned below
Symptoms:
I get pain when I do side kick, pain on left lower back when I kick with right leg
No pain while streching
Can pin point the exact location of pain
Slight pain while standing on one leg, my left leg*
No pain on bending, but feel slight sensitivity, similar in case of squats, deadlift etc
I haven't visited doctor because the pain is not sharp and I can perform my daily tasks easily and even strength train but it's uncomfortable nonetheless

Can you explain to me why you say that core bracing is not a long term strategy? for example, if you are flexion intolerant and bending forward, or creating shear are your pain triggers, wouldn't promoting spinal flexibility be detrimental in the long term because you continue to push your pain triggers?
i just cant see how anything but spinal stiffiness and core bracing is beneficial if you have an discogenic injury.
