I watched one of those trauma life in the ER. A lady had a face lift done and apon returning home she started to bleed then her whole cheek and neck swelled she went to the hospital and they said the artery was bleeding slowly and when they went to put hot packs on her it blew thru her neck incision and they ran and got compression dressing basically any dressing since the doctor now was holding the wound closed with her fingers. then they got it stopped and dressed it tightly her cheek and neck were still swelling and starting to press on her air way so they had to scramble to make sure it didn't move further so they marked it with a marker and got a hold of her surgeon but by now from her surgery that morning to 7pm he left the country on vacation and only a resident was there she gets there and started undoing the dressing she started squirting blood all over the doctor luckily the ER doctor had a brain and already had her receiving blood and more on back up cause of her. the doctor got mad and left to change. Luckily, the ER doctor was so upset that she called the doctor back and said to get one of his partners in to fix this problem. he had no partners. she had already paged the surgeon in the hospital for the ER he got there right when the resident got back and he looked at her and said call the O.R were on our way up luckily for them especially that ER doctor or she wouldn't have made it and she did but had to go back in to fix complications from the whole thing. what a mess, and as an old trauma nurse, I've seen things but never like that.
Even when i was a size 6 which was too skinny for my frame (you could see all of my ribs in my back!) i always had big upper arms like bat wings from the time i was a child. It def is hereditary fat pockets because my mom has and grandmom had upper arms like i do. I hate it, nothing fits properly and it gives the illusion that i am much heavier than i really am. But i am poor and sick (broken nervous system) so surgery will never be an option for me, not even those other treatments. This was very informative, Dr. Youn, I never realized that pockets of fat were hereditary. If i had known this 30 years ago, i wouldn’t have wasted my time trying to reduce that area with creams and exercise. Because nothing worked. They did get a little smaller with overall weight loss, but the PROPORTIONS never changed.
in high school our english teacher had the same condiion. we used to make fun of her. one time she heard us and we got detention. i am 30 now, i still feel bad for behaving like that :(
Sit down and write a letter or card to that teacher, saying just that-that you're deeply sorry that the teenage you did those things. It could mean so much to help heal their heart from being bullied.
I had an Uncle everyone used to call Tough Bob. He once had major surgery without anesthesia. They later changed his nickname to Screaming Bob. Im not sure why.
None of what we are shown explains why the doctor couldn't use something like conscious sedation for her nervous client if there was a concern about using general anesthetic. The risk of complications is less than using a general. My guess why the doctor didn't use any sedation was because of a pre-existing physical condition that would have compromised the woman's health, (perhaps her breathing is compromised) and the doctor left it unsaid for HIPAA reasons. This woman had every right not to have whatever her reasons were advertised to millions of strangers on TH-cam. Whatever her reasoning, the patient must have been uncomfortable with sharing these particular details so it is incumbent upon us to accept it and wish her well and happiness for her future!
I’m a doctor (do clearances for surgery, not the actual surgery) and a lot of my patients will opt for the local versus the general because it saves about fifteen hundred to two thousand dollars. There are very few complications to general anesthesia and if they were present, I would not clear the patient for an elective liposuction of this amount.
This looks like Lipedema! I was treated for Lipedema in 3 different liposuction sessions. I was fully awake for all three...but nobody slapped me. 😂 My doctor was brilliant and my results are excellent! He had me hold muscles contracted in the areas he was working so he could manually feel the fat vs muscle. (It was quite a workout for both of us!) I wouldn't have been able to do that had I been under general anesthesia and I believe my results are better because of it. It was surprisingly not as painful as it would seem. I have no regrets! Lipedema sucks and now I have a new life. Its wonderful to see this woman in the video getting the new life she deserves too. ❤
My Aunt had it. A size 0 on top, if not smaller. You could see bones in her chest. A size 20 or bigger on the bottom. As a guess. She has been gone close to 40 years now, but I remember asking my mother about it back in the 60s. Mom just said it happened to some people in the family. But even as a child I knew something was wrong. I also remember she barely ate. She was the sweetest woman ever.
I agree that it does look like lipedema. I haven’t had surgery. I am still looking to find someone where I live that knows about it, and I can trust to help me through the process. Praying this person makes a good recovery from her surgery.
I've heard women mention their saddlebags, but I never knew they truly existed! I'd thought it was just an expression. I'm so thrilled for this patient. What an amazing result.

Couldn’t agree more about the anesthetic. If she was medically cleared for an elective liposuction of this amount, she would be cleared for at least conscious sedation. So the most common reason to avoid it would be cost.
Yea sedation or general anesthetic is expensive and it'll add to the medical bill wherever you go. I still remember my anesthetic was 300-400$ just so they could do surgery on my ankle

I am so happy for her! I had liposuction done on my stomach and was awake. I was given pills to calm me prior to the anesthetic injections which helped tremendously. I prefer not to have general anesthesia because I don't have a good history with it. I'll avoid it at all costs. I love that she's so thrilled!
Same. Had it on saddlebags and stomach. The sedative helps to keep you calm. Tummy could use some more attention, but the saddlebags turned out great! When it’s hereditary, not much else you can do…🫢
Me too. I was awake, but no pain. They gave me low dose Demerol under my tongue with a tic tac before the procedure and local pain shots. General anesthesia always make me barf my guts out.

I did do lipo on my saddle bags without anesthesia. It wasn't bad, and it was kind of cool to watch it all happen. This was before I had a child, and it was a small, stubborn area. Having my lips done with filler was way worse, lol. However, I had a tummy tuck a month ago with muscle repair and lipo of my flanks, and you better believe I would not do that awake lol. Being awake during my c-section was the last surgery I care to ever be awake for!
It’s not really a problem other than she doesn’t like it. Plastic surgery is for anyone who wants it but it’s vanity unless it’s reconstructive in my opinion. This is vanity. Not that that’s a bad thing

I've just had a malignant tumor removed from my chest area. It was done under local anaesthetic but the tumor was bigger and deeper than anticipated. They local anaesthetic was wholly insufficient and 10 minutes in I started dfeeling each cut they made. I had to request anaesthetic 3 times. I'm a tough bastard but I NEVER want to feel that pain again!
I am so happy for this patient. I too have saddlebags but nothing like this. I am 5-10 and got down to a size 4/6 and I still had them but greatly reduced. I no longer starve myself to maintain that size and finally accepted that it is what it is. We don’t get to pick and choose our genetics. I choose to look at what I do have and appreciate.
This lady has lipedema. It’s sad no doctor diagnosed it and there was no mention of it in this video. I had four liposuctions to correct it with a total of 28l of fat removed. It would’ve been insane to do that under local anesthetic. I feel sorry for that poor woman and that terrible doctor putting her through that. That woman would’ve needed way more lipo than that. The condition will progress and cause severe pain.

@@VeganVampire.that’s actually not that many procedures. 4 liposuctions is pretty average properly treat it properly. It wasn’t about the looks. I was in a lot of pain and had trouble walking for more than a few minutes. I could barely lift my arms. The surgeries gave me back my life.
I have fat arms and it's hereditary so I understand how she feels. I don't wear spaghetti strap shirt because of this. Glad she had something done. She is beautiful before and after.
As someone with an anxiety disorder, I'm really unhappy with how the surgeon handled this entire procedure. We obviously don't know the whole story, but I feel like they could've prepared this poor young woman better than they did. My heart hurt for her as she said she was scared. I would want to be completely under. I'm so glad she's happy with the results!
Fat deposits are definitely genetic. Everyone in my family has a gut. Even the people with almost no fat, it’s all in their stomach. It’s annoying. Even when I was actually pretty skinny, people thought I was pregnant.
Me too, and I'm super skinny so I got SculpSure and it worked! It melted my gut fat and now even if my weight fluctuates by a couple pounds my stomach stays flat! I can't believe it! Totally worth it.
You and me both. I f'n hate it!! Normally I have a really fast metabolism and it makes me look too thin, but when I do manage to gain weight it's usually from a medication. So the rest of my body will look normal except for my stomach which is where most of the weight goes, making me look pregnant. It's horrible. It's impossible for me to lose the stomach fat without losing the weight in areas I really need it. Again, I hate it.
It’s expensive but there’s a clinic in my city that does two for one deals a couple times a year so I was able to stack the treatments and get them half off. It’s still a lot. I did it twice and the total cost for me was around $1400, BUT there is no amount of money I could have ever spent at any gym to get this result. My only other option was liposuction.

Can you get it on just one side? I have a weird lump like hers but just on one side. It's started to be very uncomfortable and a he's. I keep catching it on doors etc. I'm too ashamed to ask a dr as I'm overweight.
When I get a root canal or filling I get a local anesthetic, I always dig my nails into my palm or hand, I hardly feel a pinch in my gums. Now I understand why.
The medical term for this, is LIPEDEMA. It is a pathology characterized by non healthy, abnormal growth of fat on the limbs (never in the abdomen, thorax etc). This fat grows with time, and its growth is NOT RELATED with obesity of general weight gain. Unfortunately it is poorly though in medical school, but it is a pathology discovered back in 1996 and it was recognized and defined as a pathology by the WHO in 2016. It is a real disease since these fat cells aren’t like normal healthy cells, they don’t produce the hormones the normal fat cells produce, they keep replicating in number (not only increasing in size) and they are surrounded by fibrotic tissue. It is a pathology, non related with obesity, it has a inherited character and is associates with comorbidity and other pathologies (such as lymphedema). It also has 5 stages and different subtypes (according with the area and limb that is affected)
Actually, the European doctors, especially German/Austrian/Poliish were describing it at the beginning of the 20th Century. German surgeons are the most experienced at dealing with it because the health service there recognises it and treatment for advanced (Stage 3) is free for taxpayers
My saddlebags are muuuuch smaller, but they are already quite painful, fx if I go running without compression leggings they will hurt with every step and vibration. When I look at my mum, they are 100% inherited - I hope they will not get bigger

Where I am at it costs exactly $0 dollars to have a procedure done without anaesthesia and the exact same amount to do it with. I've always opted for without, because I take it really badly (had it once), so even things that are typically done with anaesthesia I chose to do without. FAR less traumatic for me. For some people it can be various health risks or preexisting conditions. So no, it's not just money.
It's also possible the pt picked it, some people can be very scared of "going under". She might have medication allergies or a health condition she didn't want shared publicly.
I'm so glad I came upon your channel, Dr. Youn. I am much older than this young lady, 67...and am soon having hip and knee procedures. Not joint replacement. I have noticed due to my sedentary lifestyle, I am developing saddle bags. I would NEVER have any such procedure without general anesthetic. However, I must agree, the doctor did an excellent job. I'm happy she got such a positive result. I hope mine is as successful.
I've known a couple older women that look like this. I know they were embarrassed and there was nothing they could do on their own. They weren't from a generation that would even think of talking to a doctor about this type of thing and many people shamed them. I bet some doctors even told them to lose weight. I don't know why you would shame anyone to start with but in theses 2 cases, they were average weight people and it was obvious their situation was not due to them being overweight
Were you prescribed Gabapentin (the drug from 👹🔥HELL)? While I was taking it, I was in classes trying to learn new stuff for my job. It made me totally stupid and I couldn't learn. 🤯 I didn't figure out why until after I had stopped taking the meds! 🤬☮️

I had an Aunt who had a mastectomy under a local anesthetic. Not sure if it was a nerve block or what but she sure did. Never complained about it. Well she complained about hospital staff but that was her nature. Lmao! Tough as nails!

I had an awake lipo surgery. The idea of the needle/canula is the scariest part. But I didn't feel anything the entire time. I went through Squplt. I was in and out in less than 3hrs.
This lady looks amazing after the liposuction! I have never saddle bags that big ! Poor lady she tried hard to walk them off. Our family is all thin dads side & moms all tall & thin . My GMA and mom & me , got baggy upper arms after pregnancy but GMA & I just exercised & it went away. My mom got an operation to fix her arms. I started getting a under chin when I was on a medication for a while ,but it left with the 20 pounds that I gained.
I had a biopsy done on a mass once and I was aboviosuly kept awake since it a minor procedure, but even that much was hell for me. Granted, it doesn’t help that I have a fear of needles but still. So I can’t imagine having this done without being put under, that poor woman. She’s a real trooper and I’m happy the results turned out well great for her!
I love how cavalier the surgeon is. I honestly believe doctors and surgeons need to be put through what patients go through so that they have more empathy. They should also watch the movie "The Doctor" with William Hurt.
Why not use IV conscious sedation? I’ve had procedures such as cataract surgery, dental extractions, carpal tunnel release, colonoscopies, etc. done this way and it’s great! Much less risky than general anesthesia.

I just had a total knee replacement. Quite a few of the other patients were getting hip or knees replaced awake! I'm so glad I had a medical issue that meant I had to be knocked out.
I have them in my thighs and knee.My mom and dad didn't have it, my aunt's did.Cancer treatment from chemo & radiation has made it go down some but it's not gone.
That is likely to be Lipoedema. 11% of women are affected by it and it can occur on hips, thighs, upper arms and lower legs. Occasionally, it can occur in up to 32 different areas of the body. If you have lumpy, hard, painful fat in these areas, please don't blame yourself. It's NOT lifestyle fat. It's NOT your fault. It is often of genetic background and is also hormone affected and begins to develop during puberty, pregnancy and menopause. it doesn't go away. The only way to remove it currently is by liposuction. It continues to grow, become fibrotic and causes pain, heat retention and disability - along with horrible judgement from society. Some Health Insurances pay for its removal, many dont.
Could this surgeon do an epidural vs. doing the local like she is? I'd assume the epidural would be less painful (I've had one for the birth of my son) compared to multiple pokes with a large needle?
I am terrified of general anesthesia. Seriously, being unconscious and it this state where you are just switched off is scarier to me than having it done... Also local anesthetics work very well with me. I usually need smaller dose than other people and it keeps me numb for longer.
In elementary school there was this girl named Tracy D. Her mom was a volunteer yard duty teacher during recess and lunch. Her mom had bigger hips and saddle bags than this lady. I always wondered how she was able to walk around and stand for so long.
Someone mentioned this might be lipedema, and I'm inclined to agree. There's not a lot of awareness about it so for everyone's information, lipedema is a condition that about 11% of women have, where fat isn't "fluffy" like regular adipose tissue, but instead is hard, has fibrous connective tissue within it, and collects in lumpy curdles/lobules on the body that are often painful and bruise easily. Usually it's the legs that are most affected, and look disproportionately large compared to the torso, but arms can have it too. It can also affect specific parts of the legs (i.e. both thighs and calves, thighs but not calves, etc.) Unfortunately it's almost *impossible* for people with lipedema to lose fat with traditional diet and exercise, so if nothing that's "supposed to work" has worked for you, you may want to check for the possibility of lipedema. The only way to be rid of the fat is to undergo liposuction, but you CAN adjust your diet to avoid adding *more* lipedemic fat onto your frame.
This reminds me of the hellish pain I went through getting EIGHT Wisdom Teeth removed under Local(I was a minor for the next month so my mom made this decision without my consent)Still hurt. A lot. Bonus: I have actual PTSD, and haven’t seen a dentist in years. I do my damndest to make sure I don’t have too.
@@jax99888 good question.I didn't know until I was fully awake in that Sadistic O Surgeons chair how weird that was. It runs on my mom's side of the family. She had the top 4 taken out when she was younger( not a kid..i think in her 30s), and had the bottom 4 taken out a few months ago(at 67). Her bottom teeth are extremely crowded. I didn't ask, but I kinda wonder if she's going to get Invisiline or something.
@@jax99888 just some further info about why my mom did this too me, meanwhile getting her bottom ones removed under General A. Her mom, my Bubushka died of heart failure maybe 2 weeks before I had this procedure. Even though Bub was 87, she blamed the Doctors and the Anesthesiologist for her death( one of those people that needs to blame something bad on anyone) . So despite begging, l had a total of 18 numbing shots top and bottom. I still felt pain in my mouth. Ended up with nerve damage in a small area of my right cheek. This Spawn of Satan Surgeon was humming a tune the whole time, while telling me to stop shaking, and that 'i looked like I'd seen a ghost'. My mom insisted on being in the room. I kept trying to say 'hes hurting me' but with that much lidocaine, or Novacane it just comes out like drool filled garble. Sorry. TMI. I don't have many opportunities to even bring that Hell up,😆.
I had mine removed under local anesthetic by choice, as I’d never been under before, and I didn’t want to fast for that long (seriously, it was all about food). One had to be removed earlier because the half cut gum kept getting infected. That guy pre-numbed the injection site with that banana tasting stuff, and I didn’t feel the shot at all. I felt a slight jolt when the tooth was pulled, but it was so little I didn’t flinch. The guy who did the other three turned my mouth into Swiss cheese with the needle, and I got that same jolt from the lower one. The injection holes hurt worse afterwards than the tooth sockets! How are some people so good at giving shots and some so awful at it?
I have had a couple of surgeries under local anesthesia. The crazy one was a c-section when they forgot to move the mirror out of the way so I got to watch. OB put me under when I reminded him to use a Pfannenstiel incision. 😂
She has lipoedema, how did no one diagnose her ?? I have it and have a very similar shape to her. I have to have 3 liposuction surgeries with a lipoedema specialist surgeon, totalling at least $60,000. It’s not even covered by Medicare. She went to just a general liposuction place, so it’s going to grow back and worse.
This should be done under general anesthesia. Liposuction is using a blunt trochar that has openings in the sides that are sharp, to shear the fat to be suctioned out.
My mom has a NOTABLE case of Lipedema (another form of genetic fat disorder). Her Arms, Legs, Butt, and Stomach are all affected - arms and legs especially. The fat can not be dieted or exercised away At All, and the surgeries are hysterically expensive and are (maybe) covered on a case-by- case basis by insurance. The disorder primarily shows up in women, and aside from a great-grandma or aunt, nobody has seen this stuff in about 3 generations. Mom's been off sugar, bread, and fruit for several years - currently on Carnivore, with only meat and cheese and eggs with basic seasonings - her double chin has mostly gone, but everything else is there. The last several years, she has only been able to sleep sitting in a chair like a recliner - no laying down, no or very wide arms on the chair. I arguably have the beginnings of the disorder also. There are few surgeons in the US (the world, really - but only about 5 or 6 nation wide) that handle this issue specifically. We're in California, and the surgeon closest to us is several hours away, very expensive, and (contrary to our initial consult) apparently has begun requiring travelling patients to stay local for upwards of two weeks, including a night or two in a special facility. Staying local to Beverly Hills for upwards of two weeks - for several surgeries over the course of a year or two. It adds WAY up... As for the disorder, the biggest issue is that so few people know about it. My mom had to find out from internet research. None of her GP's over the years, her bariatric surgeon (tried the LapBand years ago)... nobody knew this stuff existed. They just stuck with 'you're fat! If you lose weight, you'll feel better!' - with no viable examples of HOW. I've had similar experiences.
I just had a consultation about getting lipo yesterday, and I told them I was terrified about being awake (which is all they offer). This video didn't help my fear.😅
8 years ago I got pneumonia and an infection at the hospital. I had to get an ultrasound guided thoracintesis while fully awake and alert. Basically they make an incision in my back and insert a tube to drain the fluid out of my lung. It sucked.
When I'm at the dentist I have my hands as fists with my nails pointed at my palms and when it hurts I just bury the nails in my palms more and more to simulate bigger pain that I have under my control tho and that helps me stay calm. It is weird but that is indeed how it works and if you get panicky or just really dislike the pain of dentist's appointments I recommend you try it.
Is it true that if you get a tummy tuck and you gain weight and the weight doesn't go back to your stomach but to other places, it starts to look weird where the fat goes? Is that true?
I am amazed how obsessed people are with plastic surgery, ignoring all the risks. For whom do they do that? "Oh, my breasts are to small. My butt is too flappy (how about exercise?) .My nose is to wide. My lips are to slim." Who put those pictures into their minds?who told them what is wrong with them? Why do they believe that? I myself am a bit too chubby, but with a moderate diet and a little exercise its correctable. The only procedure I could imagine is a facelift (because I am well over 50 now ) but I doubt I will do it. Perhaps it's a Korean and American thing. Here in Germany fewer people go under the knife, and if they do, they usually talk openly about it, because the folks around them notice anyway.
I'm glad the doctor worked with her awake. Even though she is healthy probably enough for general anesthesia. As a mom, i'm sure she couldn't imagine putting her own life at risk for a cosmetic procedure. Too many healthy moms have passed from mommy make overs. I'm sure that even though it was painful and uncomfortable she had peace of mind
@@CherryJuli true who knows her allergies and health conditions the tumescent used in lipo usually contains a certain amount of lidocaine (anesthetic) but even lidocaine in higher doses can affect blood pressure maybe she didn't want to deal with more side effects , the side effects of pain are known variables
Liposuction of this amount carries far more risk on it's own than anaesthesia does. If she was that concerned, she would not have gotten the procedure done at all.
I had a recent colonoscopy without anaesthesia. I once operated on my own finger without surgery; I purposely inserted a needle on my own eye socket as a demonstration since none of my nursing students volunteered. I had a biopsy, a D&C, and dental work without painkillers. I have a freaky high pain tolerance. It hurts, yes, but I am not bothered by it. I broke the 8th largest bone in my body, the radius, and all I felt was a deep and dull ache. I was on vacation and got up the next morning and kept doing vacay activities with nothing more than ibuprofen. P S. I also had red hair as a child. That explains a lot since redheadedness is gene linked to pain tolerance.
When i had my lipo, my primary Dr wanted the surgeon to do local anesthesia. I guess i was braver than i thought if i went along with my primary dr's preference. Plastic surgeon said nope and did general anesthesia. It was great, a blink of an eye, done.
My arms have always been extremely large because of genetics. It's exactly like you described, its the last place to lose weight. I lost 100lbs 2021 and my arms are still very large. Even a brachioplasty likeky won't fix them. They will still be large for my size.
Ugh, I’d never do a surgery like this without general anesthesia. I broke my hand when I was 13 and my middle finger was *really* crooked after. I had to see a hand specialist and he had to numb up my hand to mess with my middle finger so it would go back into its regular place. It SUCKED. I really could feel the local anesthesia go in and to this date (I’m 27 now), it’s the shot that caused me the most pain. They had to give me lots of numbing stuff for my hand and I felt random sharp pains throughout my hand as it stretched out my skin. Nope, nope, no way. I swore he gave me more than one shot, but apparently he only gave me one.
Local anesthetics really do hurt. I got bonked on the top of my head by a still moving chainsaw blade when I was a kid and needed stitches. There was no crying until I got those dang shots!
You can take out more. It just becomes more dangerous. I had 11.5l of pure fat removed in one liposuction and 8.5l in another. But you will have side effects from that.
I remember getting a root canal on my lunch break when I worked in retail. His office was in the building across the street from my job. As long as I’m numbed I’m fine. Always better to just grit your teeth (figuratively) and get it over with. That’s my process for everything.

I watched one of those trauma life in the ER. A lady had a face lift done and apon returning home she started to bleed then her whole cheek and neck swelled she went to the hospital and they said the artery was bleeding slowly and when they went to put hot packs on her it blew thru her neck incision and they ran and got compression dressing basically any dressing since the doctor now was holding the wound closed with her fingers. then they got it stopped and dressed it tightly her cheek and neck were still swelling and starting to press on her air way so they had to scramble to make sure it didn't move further so they marked it with a marker and got a hold of her surgeon but by now from her surgery that morning to 7pm he left the country on vacation and only a resident was there she gets there and started undoing the dressing she started squirting blood all over the doctor luckily the ER doctor had a brain and already had her receiving blood and more on back up cause of her. the doctor got mad and left to change. Luckily, the ER doctor was so upset that she called the doctor back and said to get one of his partners in to fix this problem. he had no partners. she had already paged the surgeon in the hospital for the ER he got there right when the resident got back and he looked at her and said call the O.R were on our way up luckily for them especially that ER doctor or she wouldn't have made it and she did but had to go back in to fix complications from the whole thing. what a mess, and as an old trauma nurse, I've seen things but never like that.
Even when i was a size 6 which was too skinny for my frame (you could see all of my ribs in my back!) i always had big upper arms like bat wings from the time i was a child. It def is hereditary fat pockets because my mom has and grandmom had upper arms like i do. I hate it, nothing fits properly and it gives the illusion that i am much heavier than i really am. But i am poor and sick (broken nervous system) so surgery will never be an option for me, not even those other treatments. This was very informative, Dr. Youn, I never realized that pockets of fat were hereditary. If i had known this 30 years ago, i wouldn’t have wasted my time trying to reduce that area with creams and exercise. Because nothing worked. They did get a little smaller with overall weight loss, but the PROPORTIONS never changed.
in high school our english teacher had the same condiion. we used to make fun of her. one time she heard us and we got detention. i am 30 now, i still feel bad for behaving like that :(

None of what we are shown explains why the doctor couldn't use something like conscious sedation for her nervous client if there was a concern about using general anesthetic. The risk of complications is less than using a general.
My guess why the doctor didn't use any sedation was because of a pre-existing physical condition that would have compromised the woman's health, (perhaps her breathing is compromised) and the doctor left it unsaid for HIPAA reasons. This woman had every right not to have whatever her reasons were advertised to millions of strangers on TH-cam. Whatever her reasoning, the patient must have been uncomfortable with sharing these particular details so it is incumbent upon us to accept it and wish her well and happiness for her future!
The preexisting condition was probably obesity +💉.
My guess is that this doctor just does it like that.
I’m a doctor (do clearances for surgery, not the actual surgery) and a lot of my patients will opt for the local versus the general because it saves about fifteen hundred to two thousand dollars. There are very few complications to general anesthesia and if they were present, I would not clear the patient for an elective liposuction of this amount.
I think it's because this was done in a clinic and not a hospital setting. They are limited with how to do certain procedures.

This looks like Lipedema! I was treated for Lipedema in 3 different liposuction sessions. I was fully awake for all three...but nobody slapped me. 😂 My doctor was brilliant and my results are excellent! He had me hold muscles contracted in the areas he was working so he could manually feel the fat vs muscle. (It was quite a workout for both of us!) I wouldn't have been able to do that had I been under general anesthesia and I believe my results are better because of it. It was surprisingly not as painful as it would seem. I have no regrets! Lipedema sucks and now I have a new life. Its wonderful to see this woman in the video getting the new life she deserves too. ❤

She has been gone close to 40 years now, but I remember asking my mother about it back in the 60s. Mom just said it happened to some people in the family.
But even as a child I knew something was wrong. I also remember she barely ate. She was the sweetest woman ever.
I agree that it does look like lipedema. I haven’t had surgery. I am still looking to find someone where I live that knows about it, and I can trust to help me through the process.

Couldn’t agree more about the anesthetic. If she was medically cleared for an elective liposuction of this amount, she would be cleared for at least conscious sedation. So the most common reason to avoid it would be cost.
Yea sedation or general anesthetic is expensive and it'll add to the medical bill wherever you go. I still remember my anesthetic was 300-400$ just so they could do surgery on my ankle

I am so happy for her! I had liposuction done on my stomach and was awake. I was given pills to calm me prior to the anesthetic injections which helped tremendously. I prefer not to have general anesthesia because I don't have a good history with it. I'll avoid it at all costs. I love that she's so thrilled!

Me too. I was awake, but no pain. They gave me low dose Demerol under my tongue with a tic tac before the procedure and local pain shots. General anesthesia always make me barf my guts out.
My primary wanted local but plastic surgeon didn't agree, so I went under. I think it was propofol.

I did do lipo on my saddle bags without anesthesia. It wasn't bad, and it was kind of cool to watch it all happen. This was before I had a child, and it was a small, stubborn area. Having my lips done with filler was way worse, lol. However, I had a tummy tuck a month ago with muscle repair and lipo of my flanks, and you better believe I would not do that awake lol. Being awake during my c-section was the last surgery I care to ever be awake for!

I've just had a malignant tumor removed from my chest area. It was done under local anaesthetic but the tumor was bigger and deeper than anticipated. They local anaesthetic was wholly insufficient and 10 minutes in I started dfeeling each cut they made. I had to request anaesthetic 3 times. I'm a tough bastard but I NEVER want to feel that pain again!

I am so happy for this patient. I too have saddlebags but nothing like this. I am 5-10 and got down to a size 4/6 and I still had them but greatly reduced. I no longer starve myself to maintain that size and finally accepted that it is what it is. We don’t get to pick and choose our genetics. I choose to look at what I do have and appreciate.
This lady has lipedema. It’s sad no doctor diagnosed it and there was no mention of it in this video. I had four liposuctions to correct it with a total of 28l of fat removed. It would’ve been insane to do that under local anesthetic. I feel sorry for that poor woman and that terrible doctor putting her through that.

@@VeganVampire.that’s actually not that many procedures. 4 liposuctions is pretty average properly treat it properly. It wasn’t about the looks. I was in a lot of pain and had trouble walking for more than a few minutes. I could barely lift my arms. The surgeries gave me back my life.

I’m pretty sure I have lipedema. Even my doctor said the only way to remove the fat in my legs was surgically.

As someone with an anxiety disorder, I'm really unhappy with how the surgeon handled this entire procedure. We obviously don't know the whole story, but I feel like they could've prepared this poor young woman better than they did. My heart hurt for her as she said she was scared. I would want to be completely under. I'm so glad she's happy with the results!

Fat deposits are definitely genetic. Everyone in my family has a gut. Even the people with almost no fat, it’s all in their stomach. It’s annoying. Even when I was actually pretty skinny, people thought I was pregnant.

You and me both. I f'n hate it!! Normally I have a really fast metabolism and it makes me look too thin, but when I do manage to gain weight it's usually from a medication. So the rest of my body will look normal except for my stomach which is where most of the weight goes, making me look pregnant. It's horrible. It's impossible for me to lose the stomach fat without losing the weight in areas I really need it. Again, I hate it.

It’s expensive but there’s a clinic in my city that does two for one deals a couple times a year so I was able to stack the treatments and get them half off. It’s still a lot. I did it twice and the total cost for me was around $1400, BUT there is no amount of money I could have ever spent at any gym to get this result. My only other option was liposuction.

Can you get it on just one side? I have a weird lump like hers but just on one side. It's started to be very uncomfortable and a he's. I keep catching it on doors etc. I'm too ashamed to ask a dr as I'm overweight.

When I get a root canal or filling I get a local anesthetic, I always dig my nails into my palm or hand, I hardly feel a pinch in my gums. Now I understand why.
The medical term for this, is LIPEDEMA. It is a pathology characterized by non healthy, abnormal growth of fat on the limbs (never in the abdomen, thorax etc). This fat grows with time, and its growth is NOT RELATED with obesity of general weight gain.
Unfortunately it is poorly though in medical school, but it is a pathology discovered back in 1996 and it was recognized and defined as a pathology by the WHO in 2016. It is a real disease since these fat cells aren’t like normal healthy cells, they don’t produce the hormones the normal fat cells produce, they keep replicating in number (not only increasing in size) and they are surrounded by fibrotic tissue.
It is a pathology, non related with obesity, it has a inherited character and is associates with comorbidity and other pathologies (such as lymphedema). It also has 5 stages and different subtypes (according with the area and limb that is affected)
The only treatment for lipedema is liposuction
Actually, the European doctors, especially German/Austrian/Poliish were describing it at the beginning of the 20th Century. German surgeons are the most experienced at dealing with it because the health service there recognises it and treatment for advanced (Stage 3) is free for taxpayers

Where I am at it costs exactly $0 dollars to have a procedure done without anaesthesia and the exact same amount to do it with. I've always opted for without, because I take it really badly (had it once), so even things that are typically done with anaesthesia I chose to do without. FAR less traumatic for me. For some people it can be various health risks or preexisting conditions. So no, it's not just money.

I have noticed due to my sedentary lifestyle, I am developing saddle bags. I would NEVER have any such procedure without general anesthetic. However, I must agree, the doctor did an excellent job. I'm happy she got such a positive result. I hope mine is as successful.
I've known a couple older women that look like this. I know they were embarrassed and there was nothing they could do on their own. They weren't from a generation that would even think of talking to a doctor about this type of thing and many people shamed them. I bet some doctors even told them to lose weight. I don't know why you would shame anyone to start with but in theses 2 cases, they were average weight people and it was obvious their situation was not due to them being overweight

My dentist uses the tapping method when giving injections and it makes the whole process so much less painful.
I had an Aunt who had a mastectomy under a local anesthetic. Not sure if it was a nerve block or what but she sure did. Never complained about it. Well she complained about hospital staff but that was her nature. Lmao! Tough as nails!

I had an awake lipo surgery. The idea of the needle/canula is the scariest part. But I didn't feel anything the entire time. I went through Squplt. I was in and out in less than 3hrs.
This lady looks amazing after the liposuction! I have never saddle bags that big ! Poor lady she tried hard to walk them off. Our family is all thin dads side & moms all tall & thin . My GMA and mom & me , got baggy upper arms after pregnancy but GMA & I just exercised & it went away. My mom got an operation to fix her arms. I started getting a under chin when I was on a medication for a while ,but it left with the 20 pounds that I gained.

I had a biopsy done on a mass once and I was aboviosuly kept awake since it a minor procedure, but even that much was hell for me. Granted, it doesn’t help that I have a fear of needles but still. So I can’t imagine having this done without being put under, that poor woman. She’s a real trooper and I’m happy the results turned out well great for her!

You can do nerve blocks. I had Fraxel repair done under local with nerve blocks and it works out ok.
My mom had a friend who had saddlebags. Even when she had lap band surgery and was at her thinnest she still had them.
Why not use IV conscious sedation? I’ve had procedures such as cataract surgery, dental extractions, carpal tunnel release, colonoscopies, etc. done this way and it’s great! Much less risky than general anesthesia.

I just had a total knee replacement. Quite a few of the other patients were getting hip or knees replaced awake! I'm so glad I had a medical issue that meant I had to be knocked out.
I have them in my thighs and knee.My mom and dad didn't have it, my aunt's did.Cancer treatment from chemo & radiation has made it go down some but it's not gone.
That is likely to be Lipoedema. 11% of women are affected by it and it can occur on hips, thighs, upper arms and lower legs. Occasionally, it can occur in up to 32 different areas of the body. If you have lumpy, hard, painful fat in these areas, please don't blame yourself. It's NOT lifestyle fat. It's NOT your fault. It is often of genetic background and is also hormone affected and begins to develop during puberty, pregnancy and menopause. it doesn't go away. The only way to remove it currently is by liposuction. It continues to grow, become fibrotic and causes pain, heat retention and disability - along with horrible judgement from society. Some Health Insurances pay for its removal, many dont.
Could this surgeon do an epidural vs. doing the local like she is? I'd assume the epidural would be less painful (I've had one for the birth of my son) compared to multiple pokes with a large needle?

I am terrified of general anesthesia. Seriously, being unconscious and it this state where you are just switched off is scarier to me than having it done... Also local anesthetics work very well with me. I usually need smaller dose than other people and it keeps me numb for longer.
In elementary school there was this girl named Tracy D. Her mom was a volunteer yard duty teacher during recess and lunch. Her mom had bigger hips and saddle bags than this lady. I always wondered how she was able to walk around and stand for so long.
Someone mentioned this might be lipedema, and I'm inclined to agree. There's not a lot of awareness about it so for everyone's information, lipedema is a condition that about 11% of women have, where fat isn't "fluffy" like regular adipose tissue, but instead is hard, has fibrous connective tissue within it, and collects in lumpy curdles/lobules on the body that are often painful and bruise easily. Usually it's the legs that are most affected, and look disproportionately large compared to the torso, but arms can have it too. It can also affect specific parts of the legs (i.e. both thighs and calves, thighs but not calves, etc.)
Unfortunately it's almost *impossible* for people with lipedema to lose fat with traditional diet and exercise, so if nothing that's "supposed to work" has worked for you, you may want to check for the possibility of lipedema. The only way to be rid of the fat is to undergo liposuction, but you CAN adjust your diet to avoid adding *more* lipedemic fat onto your frame.

It runs on my mom's side of the family. She had the top 4 taken out when she was younger( not a kid..i think in her 30s), and had the bottom 4 taken out a few months ago(at 67). Her bottom teeth are extremely crowded. I didn't ask, but I kinda wonder if she's going to get Invisiline or something.
@@jax99888 just some further info about why my mom did this too me, meanwhile getting her bottom ones removed under General A. Her mom, my Bubushka died of heart failure maybe 2 weeks before I had this procedure. Even though Bub was 87, she blamed the Doctors and the Anesthesiologist for her death( one of those people that needs to blame something bad on anyone) . So despite begging, l had a total of 18 numbing shots top and bottom. I still felt pain in my mouth. Ended up with nerve damage in a small area of my right cheek. This Spawn of Satan Surgeon was humming a tune the whole time, while telling me to stop shaking, and that 'i looked like I'd seen a ghost'. My mom insisted on being in the room. I kept trying to say 'hes hurting me' but with that much lidocaine, or Novacane it just comes out like drool filled garble.

I had mine removed under local anesthetic by choice, as I’d never been under before, and I didn’t want to fast for that long (seriously, it was all about food). One had to be removed earlier because the half cut gum kept getting infected. That guy pre-numbed the injection site with that banana tasting stuff, and I didn’t feel the shot at all. I felt a slight jolt when the tooth was pulled, but it was so little I didn’t flinch. The guy who did the other three turned my mouth into Swiss cheese with the needle, and I got that same jolt from the lower one. The injection holes hurt worse afterwards than the tooth sockets!
How are some people so good at giving shots and some so awful at it?
As a nurse, doing this’under a local seems barbaric to me. Trying to think of a medical reason why she can’t have a general or conscious sedation.
I have had a couple of surgeries under local anesthesia. The crazy one was a c-section when they forgot to move the mirror out of the way so I got to watch. OB put me under when I reminded him to use a Pfannenstiel incision. 😂
She has lipoedema, how did no one diagnose her ?? I have it and have a very similar shape to her. I have to have 3 liposuction surgeries with a lipoedema specialist surgeon, totalling at least $60,000. It’s not even covered by Medicare.
She went to just a general liposuction place, so it’s going to grow back and worse.

This should be done under general anesthesia. Liposuction is using a blunt trochar that has openings in the sides that are sharp, to shear the fat to be suctioned out.

My mom has a NOTABLE case of Lipedema (another form of genetic fat disorder). Her Arms, Legs, Butt, and Stomach are all affected - arms and legs especially. The fat can not be dieted or exercised away At All, and the surgeries are hysterically expensive and are (maybe) covered on a case-by- case basis by insurance. The disorder primarily shows up in women, and aside from a great-grandma or aunt, nobody has seen this stuff in about 3 generations. Mom's been off sugar, bread, and fruit for several years - currently on Carnivore, with only meat and cheese and eggs with basic seasonings - her double chin has mostly gone, but everything else is there. The last several years, she has only been able to sleep sitting in a chair like a recliner - no laying down, no or very wide arms on the chair. I arguably have the beginnings of the disorder also. There are few surgeons in the US (the world, really - but only about 5 or 6 nation wide) that handle this issue specifically. We're in California, and the surgeon closest to us is several hours away, very expensive, and (contrary to our initial consult) apparently has begun requiring travelling patients to stay local for upwards of two weeks, including a night or two in a special facility. Staying local to Beverly Hills for upwards of two weeks - for several surgeries over the course of a year or two. It adds WAY up...
As for the disorder, the biggest issue is that so few people know about it. My mom had to find out from internet research. None of her GP's over the years, her bariatric surgeon (tried the LapBand years ago)... nobody knew this stuff existed. They just stuck with 'you're fat! If you lose weight, you'll feel better!' - with no viable examples of HOW. I've had similar experiences.
I just had a consultation about getting lipo yesterday, and I told them I was terrified about being awake (which is all they offer). This video didn't help my fear.😅

8 years ago I got pneumonia and an infection at the hospital. I had to get an ultrasound guided thoracintesis while fully awake and alert. Basically they make an incision in my back and insert a tube to drain the fluid out of my lung. It sucked.
That nerve stimulation trick is great for little kids. Clumsy as your body changes so rapidly and it helps the pain stop faster

When I'm at the dentist I have my hands as fists with my nails pointed at my palms and when it hurts I just bury the nails in my palms more and more to simulate bigger pain that I have under my control tho and that helps me stay calm. It is weird but that is indeed how it works and if you get panicky or just really dislike the pain of dentist's appointments I recommend you try it.
Is it true that if you get a tummy tuck and you gain weight and the weight doesn't go back to your stomach but to other places, it starts to look weird where the fat goes? Is that true?

I am amazed how obsessed people are with plastic surgery, ignoring all the risks. For whom do they do that? "Oh, my breasts are to small. My butt is too flappy (how about exercise?) .My nose is to wide. My lips are to slim."
Who put those pictures into their minds?who told them what is wrong with them? Why do they believe that?
I myself am a bit too chubby, but with a moderate diet and a little exercise its correctable. The only procedure I could imagine is a facelift (because I am well over 50 now ) but I doubt I will do it.
Perhaps it's a Korean and American thing. Here in Germany fewer people go under the knife, and if they do, they usually talk openly about it, because the folks around them notice anyway.
I'm glad the doctor worked with her awake. Even though she is healthy probably enough for general anesthesia. As a mom, i'm sure she couldn't imagine putting her own life at risk for a cosmetic procedure. Too many healthy moms have passed from mommy make overs. I'm sure that even though it was painful and uncomfortable she had peace of mind
You can do liposuction under anesthesia that isn’t as deep as it would be for a heart surgery for example.
@@CherryJuli true who knows her allergies and health conditions the tumescent used in lipo usually contains a certain amount of lidocaine (anesthetic) but even lidocaine in higher doses can affect blood pressure maybe she didn't want to deal with more side effects , the side effects of pain are known variables
Liposuction of this amount carries far more risk on it's own than anaesthesia does. If she was that concerned, she would not have gotten the procedure done at all.
I had a recent colonoscopy without anaesthesia. I once operated on my own finger without surgery; I purposely inserted a needle on my own eye socket as a demonstration since none of my nursing students volunteered.
I had a biopsy, a D&C, and dental work without painkillers. I have a freaky high pain tolerance. It hurts, yes, but I am not bothered by it.
I broke the 8th largest bone in my body, the radius, and all I felt was a deep and dull ache. I was on vacation and got up the next morning and kept doing vacay activities with nothing more than ibuprofen.
P S. I also had red hair as a child. That explains a lot since redheadedness is gene linked to pain tolerance.
When i had my lipo, my primary Dr wanted the surgeon to do local anesthesia. I guess i was braver than i thought if i went along with my primary dr's preference. Plastic surgeon said nope and did general anesthesia. It was great, a blink of an eye, done.
My arms have always been extremely large because of genetics. It's exactly like you described, its the last place to lose weight. I lost 100lbs 2021 and my arms are still very large. Even a brachioplasty likeky won't fix them. They will still be large for my size.
Ugh, I’d never do a surgery like this without general anesthesia. I broke my hand when I was 13 and my middle finger was *really* crooked after. I had to see a hand specialist and he had to numb up my hand to mess with my middle finger so it would go back into its regular place. It SUCKED. I really could feel the local anesthesia go in and to this date (I’m 27 now), it’s the shot that caused me the most pain. They had to give me lots of numbing stuff for my hand and I felt random sharp pains throughout my hand as it stretched out my skin. Nope, nope, no way. I swore he gave me more than one shot, but apparently he only gave me one.
Local anesthetics really do hurt. I got bonked on the top of my head by a still moving chainsaw blade when I was a kid and needed stitches. There was no crying until I got those dang shots!
If you can only take out 4ltrs a day cab you do the surgery from like 10am-2am
You can take out more. It just becomes more dangerous. I had 11.5l of pure fat removed in one liposuction and 8.5l in another. But you will have side effects from that.
My mom, a surgical RN at the time, used to pinch under our arms right before injecting the needle. It worked for me. Not so much for my sister.
try an epidural. I had my knee done that way and loved it, was able to watch on the monitor.

I’m one of those people who gets every procedure without anesthesia, including dental work. Getting a face lift soon and yup, staying awake.

I remember getting a root canal on my lunch break when I worked in retail. His office was in the building across the street from my job. As long as I’m numbed I’m fine. Always better to just grit your teeth (figuratively) and get it over with. That’s my process for everything.
I think I saw this show and she specifically didn’t want anesthesia cause she’s terrified of anesthesia. This doctor does conscious plastic surgery for people who want to opt out of anesthesia
They also said in the show, that healing would take less time.
